1 Welcome to the 12 th Grade Career Internship Program! You have entered into a new phase of your senior year that will prove to be a worthwhile and purposeful experience leading you toward a future career. This booklet is designed to help you navigate the process of getting started in your 12th Grade Career Internship. Please read the description and eligibility requirements carefully. Follow the easy 4 step process by submitting all forms and documents on time. Step 1: Step 2: Step 3: Teacher Sign Out, Student Internship Application, and Montclair High School Internship Site Agreement. Due by April 17, Official documentation required two weeks before start of option May 4, 2009 Received when Step 1 is completed. Ongoing assessment documents to be submitted to 12 th Grade Career Internship Coordinator at weekly seminars. Step 4: Final Presentation/Exhibition week of June 15, 2009.

2 DESCRIPTION The 12 th Grade Career Internship Program allows high school seniors the opportunity to intern with local companies, government or faith based organizations, schools, or other community entities. It is the student s responsibility to find a reliable, appropriate internship that is career focused, related to post high school education or training, or community service oriented. These career internships are designed to provide the student with diversified learning experiences that bridge the gap between school and employment or higher education and training. Students participate in unpaid, structured, and supervised activities as set forth in the training plan. The training plan is designed collaboratively by the school district and/or the internship site. ELIGIBILITY The requirements for the Program are as follows: Successful passing of the High School Proficiency Assessment (HSPA) or SRA Framework Graduation requirements and credits have been met Satisfactory Academic Standing Satisfactory Discipline Record Textbook obligations have been met 15 or less absences in a full Year course by May 18, or less absences in a Semester course by May 18, 2009 EXAMPLES OF POSSIBLE 12 th GRADE CAREER INTERNSHIPS Career Internships in a career focused, work related site Service tied to high school and/or past high school courses Technical courses taken outside of school Dual enrollment in college courses Mentorships with teachers at the elementary, middle or high school level

3 TIME PERIOD FOR SENIOR OPTION Begins after all appropriate documentation has been submitted and approved during Semester 2 with actual release time from classes Marking Period 4, May 18, The students will participate as a student learner with the approved internship site agency for four (4) hours a day, five (5) days a week. The end date for the internship is June 19, FORMS REQUIRED All the forms provided herein are in compliance with federal and state labor laws, regulations, child labor laws, and education administrative code governing Career Internships. Step 1: The application and Training Plan are to be submitted for review and acceptance into the 12 th Grade Career Internship Program as Step 1 in the process. 1. Teacher Sign Out 2. Montclair High School/Internship Site Agreement Step 2: The following forms are submitted after the student is notified that he/she will participate in 12 th Grade Career Internship Program 1. Agreement (2 page document) 2. Insurance Statement 3. Parent/Guardian Responsibilities 4. Code of Conduct Step 3: The following forms are used during the internship for assessment purposes. Please be sure to read each form carefully and submit the completed forms to the 12 th Grade Career Internship Program Coordinator in a timely manner. 1. Assessment of Skill Mastery (one for student, one for business mentor) 2. Time Sheets 3. Journal Form (use this form or create your own) 4. Feedback (one for student, one for business mentor) Step 4: Presentation/Exhibition Criteria Form

4 WEEKLY SEMINARS Students will participate in a weekly seminar at 1:45pm every Thursday in the LGI. May 18, 2009 Real Life, Real Skills Getting Along with Others May 25, 2009 Personal Finances June 1, 2009 Choosing the Right Career Career Options June 8, 2009 My First Semester at College June 15, 2009 Presentations/Exhibitions Weekly Time Sheets, Journal Forms, and Intern Mentor Evaluation Forms will be handed in on each of these dates. FINAL PROJECT/EXHIBITION At the conclusion of the Career Internship Program, during the week of June 15, 2009, students will return to the high school to present a final project/exhibition showcasing their career internship experience. Students will present to a committee of school officials that will include the 12 th Grade Career Internship Program Coordinator, Guidance Counselor, Parent Volunteers, Teacher, and Principals. Please plan for at least a 5 minute presentation. Internship Site representatives may voluntarily attend the presentation as well. Suggested presentations for the final project include: Presentation using technology such as PowerPoint or other software Collage or Poster Portfolio that includes work samples, photographs, and other items that highlight the internship Research paper explaining the internship experience and some important aspects of the field Dramatic production such as a play enacting the internship site Essay on future career path using the internship experience as reference SUMMARY Through the 12 th Grade Career Internship Program, students may connect their classroom learning to the world of work and ultimately, their future careers. Students can explore tentative career choices, gain awareness of employment opportunities, and assess their own interests and abilities while participating in actual on the job activities. The New Jersey Business & Industry Association (NJBIA) and the NJ Departments of Education and Labor support the NJ Department of Education s 12 th Grade Career Internship Program. All forms contained herein are in compliance with federal and state laws, regulations, and administrative code. Forms will be kept on file with the 12 th Grade Career Internship Program Coordinator. Time Sheets and Journals in addition to 12 th Grade Career Internship Coordinator s contact with business mentors will keep the school abreast of the student s accomplishments and adherence to the Training Plan. The 12 th Grade Career Internship Program culminates with the final project/exhibition and a forum for evaluation and feedback on the internships.

9 Montclair High School 12 th Grade Career Internship Program Montclair High School/Internship Site Agreement We understand that this unpaid Career Internship is not employment and that the student is not entitled to wages or a promise of employment at the completion of the Career Internship, as per the NJ Department of Labor Child Labor Laws, Subchapter 18, 12: , School to Work Program. We have reviewed and are in agreement with the description, goal(s), learning activities, and final project. If the learning activities are accomplished prior to the proposed end date, we agree either to develop a new training plan indicating new learning activities or to conclude the unpaid Career Internship. We understand that if the Career Intern remains on the work site after the completion of the learning activities, then the Career Intern will be considered an employee under the New Jersey Child Labor Laws and will be entitled to compensation from that point forward. We agree to complete all required paperwork and maintain all required documentation for this unpaid Career Internship. The district Program Coordinator and the Internship Site Mentor agree to conduct regular monitoring of this Career Internship in accordance with the 12 th Grade Option Unpaid career Internship Agreement.
