2 What is My UMC Utrecht? As a patient of the UMC Utrecht you have access to the patient portal called My UMC Utrecht. In this portal you can view information in your medical file and test results, send an e-consult and request a repeat prescription. This brochure explains how My UMC Utrecht works and what the various components are. Go to mijn.umcutrecht.nl for a detailed manual. My UMC Utrecht enables you to access parts of your medical file. Your doctor updates your medical records as a reference for themselves and for others involved in your care. You might therefore come across information in your patient portal that is difficult for you to understand, such as medical terms. Please contact your doctor if you have any questions about the contents of your patient portal. Patient portal My UMC Utrecht 2
3 What do you need? To use My UMC Utrecht, you need: a computer or tablet with Internet access a DigiD with SMS function a mobile phone What is DigiD? DigiD is short for digital identity. It is a shared system that enables the authorities to verify your identity on the Internet. You can apply for your DigiD at Your DigiD gives you access to a growing number of government agencies. Applying for DigiD If you do not have a DigiD, you can apply for one via To use My UMC Utrecht, you must request a DigiD with SMS function; this is safer to use. If you already have a DigiD, but without the SMS function, you can view and update your DigiD data via After you have applied for or updated your DigiD, you will receive an activation code through the post. This can take up to five days. You can activate your DigiD with this activation code. You will also need your username and password to activate your DigiD. 4 Logging into My UMC Utrecht Step 1 Go to and click on Login at the top right, or go to mijn.umcutrecht.nl. Doctor: I think it's important that patients can peruse parts of their medical records. But they might get more than they bargained for, like results they don t really understand or treatment reports containing medical terminology, sometimes not quite entirely completed. Patients should be aware of that. If necessary, the doctor can explain things the next time they see the patient. Patient portal My UMC Utrecht 4
5 Step 2 The following screen will be displayed. Here you will find more details, a comprehensive manual and answers to frequently asked questions. To log into My UMC Utrecht, click on Log in with DigiD. Step 3 The following screen will be displayed. Click on Login again. Patient portal My UMC Utrecht 5
6 Step 4 You have opened a DigiD input screen. Here you can log in with your DigiD username and password. You will immediately receive a text message containing a code on your mobile phone. Once you enter that code, you are logged into My UMC Utrecht. Logging in as an authorised person Parents and guardians can be given access to their child s portal. Administrators and mentors can be given access to the portal of the patient they represent. Access is granted upon submission of an authorisation. Authorised persons can use their own DigiD with SMS function to access medical records, send e- consults and request repeat prescriptions. The authorised person can see and do everything in My UMC Utrecht that the patient can. Should you wish to log in to the portal of your child or the patient you represent, you must first request authorisation from the care provider. On your next visit to the outpatient clinic, you must bring along a number of things that are required for the authorisation. More information on this can be found on our website. Go to click on Login at the top right of your screen and then go to Log in as authorised person. Logging in To log in as an authorised person, follow steps 1 through 4 as described under Logging into My UMC Utrecht. Log in with your own DigiD username and password. After logging in, you will see for whom you are authorised (see picture below). You can be authorised for several people. Click on the name to gain access to the portal of the relevant person. Switching to your own portal To switch back to your own portal (if you are not only an authorised person, but also a patient at the UMC Utrecht) click on your name at the top of the screen. This brings you back to your own patient portal. Logging out You can log out by clicking on Logout at the top right of your screen. If there is no activity in your My UMC Utrecht for 15 minutes, you will automatically be logged out. Everything that has not been sent or saved will be lost. Patient portal My UMC Utrecht 6
7 Components of My UMC Utrecht The following is a brief explanation of each component in My UMC Utrecht. The components you can access vary depending on the unit where you are being treated. You might therefore come across descriptions of components in this brochure that do not occur in your patient portal. Detailed information can be found in the manual, which is posted on my.umcutrecht.nl and in your patient portal under Explanation of My UMC Utrecht. 8 File Treatment Reports Under Treatment Reports you can read in your file what the care provider has written about your outpatient visit, hospitalisation or telephone consultation. You can read about your medical symptoms, whether you are to increase or decrease the dosage of your medicines or whether you have to undergo an examination or do certain exercises. Correspondence regarding patient Here you will find various kinds of correspondence about your medical situation for example, letters you receive yourself, but also letters from the UMC Utrecht to your general practitioner or another healthcare provider. Medicines Here you will find the list of medicines you are currently taking as far as the doctors at the UMC Utrecht are aware. You can request a repeat prescription for some medicines via My UMC Utrecht in consultation with your doctor. Medication you have been prescribed but no longer take is listed under History. Measurements Here you can view the results of the various measurements that have been taken. Test results listed here include your heart rate, blood pressure, respiratory rate, temperature, height, weight, BMI, pain score and oxygen percentage. Test results In this component you can view the results of various tests that have been done, such as blood tests, urine tests, tissue tests or gastrointestinal tests. Reports on imaging tests (such as an ultrasound or MRI) can be found here but no visual material (photos or scans). Test results immediately available Please note that all test results can be read immediately. Your healthcare provider will tell you during your next appointment what the test results entail. You decide whether you want to see the test results beforehand. Should you have any concerns, please contact the outpatient clinic where you are receiving treatment. Never change your medication intake yourself based on your results, unless agreed with your doctor. Tasks (questionnaires) Have you been requested to complete a questionnaire or keep a diary? These requests can be found under Tasks. Here you will find the questionnaires that still need to be completed and those you have already completed. In some instances, you can still make changes. In that case, a pen is displayed on the screen. When you click on the i (in red) you will receive additional information regarding the questionnaire. For instance, you will see how long after saving the questionnaire definitely changes can still be made ( Changeable ). You can also see under Draft duration how long after clicking on Save temporarily you can still make changes. Patient portal My UMC Utrecht 8
9 Agenda Under Agenda, you can view a list of your appointments, admissions, operations and treatments that are scheduled in the UMC Utrecht. You can make an appointment for some specialisms via My UMC Utrecht; this is indicated by the Schedule appointment button. If you have an appointment in the UMC Utrecht, please bring the following: Your valid ID Your insurance card or health card A current medication list, which can be obtained from your pharmacy E-consult You can use an e-consult to put questions to your care provider as well as to inform your care provider of any side effects you might suffer from your medication, for instance. Your e-consult is usually received at the reception desk of the outpatient clinic. The attendant will send your question to the appropriate care provider, who will answer your question. You can expect a response within three working days. An e- consult is not suitable for emergencies. Instead, please telephone the outpatient clinic where you are being treated or your general practitioner. Patient portal My UMC Utrecht 9
10 You can be notified by when you receive a new e-consult in your portal. If you wish to receive notification, please tick the appropriate box in your personal particulars which you can find under patient data. Doctor: Patients are given more personal control if that s what they want. For instance, I make arrangements with some patients to come back for a check-up when they feel it is necessary. In between times, we consult with each other via e-consults in their patient portal. Patients are pleased with that and so am I! Patient data Personal particulars Here you will find your particulars as we know them, such as your address and the details of your general practitioner and your insurer. It is important that this data is correct, so check it carefully. Please enter your address and pharmacy yourself. Please inform us of any changes: by means of a form on Go to contact & route and click on change patient data. at the desk of the outpatient clinic or at Central Admissions at the main entrance of the UMC Utrecht. Here you can also see (under Authorised persons ) who can access the portal on your behalf. Treatment team Here you will find the details of your treatment team. Personal notes Here you can make your own notes. For instance, you can make a note of the questions you want to ask your healthcare provider during your next visit or you can keep a list of your symptoms. All care providers who have access to your file after providing treatment can view these notes. In principle, your healthcare provider will only access your notes if you have agreed upon this with each other. Information Here you will find general information about your visit to the hospital and about your healthcare rights. Some departments include additional information here. Patient: I mostly use my portal for reference purposes and to retrieve things later on. I prefer to get my test results from my doctor, because then I will be able to understand exactly what the matter is. Patient portal My UMC Utrecht 10
11 Protecting your data The UMC Utrecht respects patient privacy. Here you can read how we protect your privacy and your data and how you can help in this respect. We record your details We like to keep track of how often My UMC Utrecht is used. We therefore register all your logins. The data you enter into My UMC Utrecht is stored in your medical file. Any e-consult you send your care provider is stored in your UMC Utrecht file. The answer you receive from your healthcare provider is also stored there. E-consults can be viewed by the healthcare providers within the UMC Utrecht who have access to your file. Using My UMC Utrecht safely We treat your digital data with care, in accordance with the Dutch Personal Data Protection Act. You too can help to protect the use of My UMC Utrecht: Log out when you are no longer actively using My UMC Utrecht and then close the browser. Take care with printouts you make of your data. Your DigiD username and password are confidential. Save them in a safe place. Do not save your DigiD username and password in your computer, even though many computer programs tell you to do so, because anyone who has access to your computer will also be able to find your login name and password. Use a screen saver to prevent passers-by seeing your data when you are away from your computer. If you have Wi-Fi, make sure your Internet modem or router is encrypted. This will prevent unauthorised use of your Internet connection. The manual of your device explains how you can do this. Patient: I have been taking the same medication for years. You can also contact the help desk. The help desk is located near the main entrance to the UMC Utrecht, next to the Central Admissions desk. The help desk can be reached by telephone Mondays to Fridays between 9 am and 5 pm.
