1 What is MyEChart? Frequently Asked Questions MyEChart is Skagit Regional Clinics secure patient portal. It gives you tools to efficiently communicate with your health care provider s office, including the ability to send secure s, request an appointment, view portions of your medical record, including lab results, request prescription refills and other tools to help manage your health at anytime. Who can sign up? Patients and their legal guardians can sign up for MyEChart. Legal guardians will have full access to their child s account until the patient turns 13 years old. Why does access change when the child turns 13 years old? Based on Washington statues, in cases where certain medical conditions apply, children may obtain medical treatment without the permission of their parent or legal guardian. Upon the child s thirteenth birthday, the legal guardian s access will expire and the child will need to complete the enrollment process to activate their own account. Once enrollment is complete, the child can grant full or partial rights to the legal guardians on their account. How do I sign up? You may sign up at your provider s office or by visiting and clicking the Enroll Now button. Please read and accept the terms and conditions, then complete the enrollment form. Our staff will verify the information that you provide and will follow up with a phone call if there are any questions or discrepancies. Within 48 hours, you will receive a Welcome in the personal account you provided during enrollment.

2 How will I know that my information will remain private? MyEChart is located on a secure server and the connection between your computer and the server is encrypted and secured. For more information, we encourage you to review the Privacy Policy on the MyEChart log-in page. Can I contact my health care provider through the portal? Yes. You can send a secure message to your provider through MyEChart. You can also send a message to another member of your care team, such as your provider s medical assistant. To send a message, select Compose Message from Mail tab at the top of the homepage. Fill out the appropriate fields. Then submit your message. How quickly can I expect a response to a message I sent to my provider s office? Please allow 72 hours to receive a response to your message. Please note that messages are not monitored over the weekend. How do I schedule an appointment with my doctor? To send an appointment request to your provider s office, go to the Schedule tab at the top of the homepage and select Request Appointment. Fill out the required information and click Submit. You will receive an appointment notification in your MyEChart Inbox. How can I access my complete medical record? MyEChart provides you with access to portions of your medical record, including medications, immunizations, procedures, lab results and more. You may also contact our Health Information Management department through MyEChart to request an electronic copy of your complete medical record. How can I access a family member s MyEChart account? In order to access a family member s account, you must complete a proxy form available in our Health Information Management department. This form must be completed in person in order to verify that access is appropriate. Questions about this process can be directed to Medical Records at

3 I have been locked out of my account. How can I unlock it? Accounts become locked after three unsuccessful login attempts; however, your account will automatically unlock after 30 minutes. I have forgotten my User Name/ Password. How can I retrieve it? Click on the link that says Don t remember your user name or password? on the portal log-in page (see screenshot below). Follow the instructions provided to retrieve your log-in information. I have a new address. How can I change it? Log into MyEChart. Click on the Settings tab at the top of the homepage. Click My Information and scroll down to addresses. Update your address by selecting Edit. You may also add an address to the account by clicking Add. At your next appointment, please ask the receptionist to update your address. How do I request my Personal Health Record? Click on the My Chart tab on the top of the homepage. Select Request Health Record. Then click Submit. A response will be sent to your MyEChart inbox.

4 Why is my lab result not showing up in MyEChart? Lab results will appear in MyEChart after your provider has reviewed and signed off. on the result. Why doesn t my recent diagnostic test (CT scan, MRI, etc.) appear in MyEChart? Diagnostic tests are not automatically sent to the MyEChart; however, your provider can send it to your portal account following his or her review of the test or procedure. If you would like your results sent to your portal account, please send your provider a message through the portal stating your request. Why doesn t the information about my recent hospital stay appear in MyEChart? Information from visits to Skagit Valley Hospital (including those who receive care at the Regional Cancer Care Center, Wound Healing and Rehabilitation departments) is available through the Skagit Valley Hospital Patient Portal. The SVH Patient Portal allows patients to access heath summaries of inpatient visits, discharge instructions, list of hospital and clinic visits and more. To enroll in the SVH Patient Portal, please visit: Patients/Patient-Portals. The Skagit Valley Hospital Portal and MyEChart provide access to different information and require separate enrollment for those patients who have received care at both facilities. How do I send a message to my doctor? To send a secure message to your provider or care team, go to the Mail tab and select Compose Message. Fill out the needed information and click Submit. Where can I find the forms that my providers send me? When your provider sends you any forms through MyEChart, you can find those on the homepage, in your Inbox. You can also view your Inbox by clicking on the Mail tab and selecting Inbox. Any correspondence from your provider will be listed there. Can I access MyEChart from my smart phone? Yes. You can log in and use MyEChart from anywhere with access to the Internet, including your smart phone. The same information and tools are available whether you access MyEChart through a desk top computer, laptop, tablet or smart phone.

5 Can I use one account for SRC and North Cascade Eye Associates? Yes, you can use one portal account for both practices. To do this, go to the settings tab, and select Manage Practices. Enter the security token provided to you by the additional practice, and enter your address. Once this is done, you are able to log into one portal account and select which practice you want to view.
