3 Introduction Many thanks to the families and professionals involved in shaping this directory, which has been produced for families with disabled children and young people by the Integrated Services for Disabled children of the London Borough of Tower Hamlets. Universal, targeted and specialist services - what does this mean? At Tower Hamlets, we aim to meet the needs of disabled children through universal and inclusive settings. However the need or disability may also require targeted provision. The two categories are described below: Universal Services: Universal services seek, in conjunction with parents and families, to meet all the needs of children and young people so that they are happy and healthy and able to learn and develop securely. Universal services are provided as a right to all children, including those with targeted and specialist needs. Examples include maternity services at birth, health visiting, the chance to use children s centres in early years, schools, extended schools services, youth services and Connexions in their teenage years. Targeted and Specialist Services: Services for children and their families with additional and vulnerable needs that go beyond what is on offer in the range of universal services are known as targeted services. Examples include behaviour support or additional help with learning in school, extra support for parents during the early years stage of their children, such as the Portage Team, or targeted youth support service. Specialist services are where the needs of the child and their family are significant to need intensive or statutory intervention to ensure their needs are being met. More about this Directory The directory sets out a range of short breaks for children and young people with disabilities, which are fun and stimulating allowing them to enjoy new experience, form new friendships and become more independent. Short breaks also offer breaks to families from the demands of caring responsibilities. The directory also lists other support services aimed at improving the personal development of children and young people and helping families to reach their full potential in caring for children with disabilities. The directory is not an exhaustive list of services, however it attempts to bring together information that is handy for parents. The needs of a majority of disabled children are met within universal services without the need for an assessment. Families can access certain short breaks directly or can ask for an assessment if there is a change in circumstances or if more than one service is required. A short break can be accessed by one of the following: 1. The Self-referral form 2. Common Assessment Framework (CAF) 3. 4 Applying for a Short Break You can apply for a short break service for your child by completing the short breaks self-referral form which can be obtained from the Short Breaks Co-ordinator by calling or short.breaks@towerhamlets.gov.uk To request an assessment or access more than one specialist service, you can contact the Children with Disabilities Team on Assessments are completed by a professional. This takes into account the child s development, parent capacity and family and environmental factors, as families may have the same needs but manage differently. 4
5 tower core hamlets offer Services for disabled children in the borough are provided by integrated teams across health and Children, Schools & Families for effective joint working to meet the needs of disabled children with a timely and coordinated approach. The Core Offer is intended for parents, children and young people locally, so that families with disabled children know how the Core Offer will be in their area. There are three components of the basic offer as set out below. The bullet points describe the local offer to families. 1. 6 Tower Hamlets FIS is a free and impartial information service for mums, dads, carers of children and young people from birth to 19 years. The FIS provides information, advice and assistance on a wide range of services from helping to find your nearest children s centre or youth club to exploring what funding is available for childcare. Information officers are available to offer help. You can access information through the online Family Services Directory. This directory allows you to search for local and national services for families with children and young people. 7 Service Key Specialist Short Break funding is targeted at two groups of disabled children at the higher end of need. Nationally the two groups are identified as group A and group B and the services listed on this directory indicate their target group as A, B or for both. A: Children and young people with Autistic Spectrum Disorder (who have severe learning disabilities or behaviour which is challenging) OR those children and young people whose challenging behaviour is associated with other impairments such as severe learning disabilities. B: Children and young people with complex health needs including those with physical and/or learning disabilities, those who require palliative care (nursing care) and those with associated sensory impairments. A&B: Both of the above. The services listed will also indicate whether they can be accessed directly or whether a referral and assessment is needed and the keys are as follows: 3 Self-referral. 9 A range of specialist early years services are available for pre-school children with additional needs to improve outcomes for them and help with their physical and emotional development. Nursery and Play Services Children s Centres (age birth - years) A&B 3 Children's centres offer high quality inclusive services to families with children under 5 through a range of family support and early learning activities. Children's Centres also work in partnership with a range of agencies and organisations to deliver advice and support on health and wellbeing, employment, training and volunteering. Throughout all our provision we support children, parents and carers with additional needs to access services and contribute to the development of children's centres across the borough. 10 Day Care Providers (age birth - 5 years) Day Care by John Smith Children s Centre (age birth - 5 years)a&b W Provide a day care facility of play sessions for 22 children under five years old and 40% of take-up is by children with additional needs. Referral must be made by the Child Development Team (CDT)- see below for information on CDT. They will have a named Inclusion Coordinator who is responsible for working with parents to plan for children's needs. They are supported by Tower Hamlets Early Years Inclusion Team, who provide advice and support to include disabled children and children with additional needs. For specific provider details contact FIS on All day care settings have a named Inclusion Coordinator and are supported by Tower Hamlet's Early Years Inclusion Team to include disabled children and children with additional needs. A&B W This is a support group for young children with additional needs. The group offers therapy and play sessions which are run fortnightly on Fridays from 10.00am pm, term time only. The group is co-ordinated by health staff from the Child Development Team (CDT) but takes place at the Whitehorse Play Centre. 11 Tower Hamlets Opportunities Group (THOG) (age 2-5 years) A&B 3 Provide fully inclusive play sessions for children with additional needs. Sessions take place from 9.30am pm Monday - Friday and up to 36 places are available for the sessions each morning although this number may differ each day depending on the availability of space. There is a fee of 50p per session. As well as offering a playgroup, THOG provide an outreach programme where staff members regularly visit families with children with additional needs to help their access to supervised play in a safe, secure and stimulating environment. Service is for all the family thus offering a break from the demands of living with a child with special needs. Visits are pre planned, booked in advance at a mutually agreed time during the school day. Serves around 30 families but this may vary. Special Sensory Play (pre-school children) A&B 3 A weekly play session for pre school children with special needs, their pre school siblings and parents, with access to sensory experiences plus support, both for adults and the children. Sessions take place on Wednesdays 1.00pm pm during term time at The Toyhouse Centre. Sessions are for an average of 6 families but this may vary. Short Breaks: Playschemes and Activities Easter Scheme (age 5-19 years) A&B 3 Offer a secure place for a period of eight days over the holiday period providing structured on and off site activity programme for children with physical and learning disabilities and/or autism. Examples of activities provided are arts and craft, therapy pool, sensory room sports, cinema tips and visits to local places. Provide breakfast, snacks and hot lunch as well as door to door transport. Scheme runs Monday to Friday from 9.00am pm and offers 50 places. February/October half-term Scheme (age 5-19 years) A&B 3 Offer a secure place for a period of five days over the half-term period providing structured on and off site activity programme for children with physical and learning disabilities and/or autism. Examples of activities provided are arts and craft, therapy pool, sensory room sports, cinema tips and visits to local places. Provide breakfast, snacks and hot lunch as well as door to door transport. Scheme runs Monday to Friday from 9.00am pm and offers 50 places. Summer Scheme (age 5-19 years) A&B 3 Provide a four week summer holiday scheme in an inclusive setting for children with physical and learning disabilities and/or autism. Service offers a full on and off activity programme and transport is provided to meet needs. Exciting activities offered include site visits and day trips, swimming, cinema trips, cooking, arts and crafts, sailing and wall climbing. Breakfast, snacks and hot lunch are also provided. The scheme runs Monday to Friday from 9.00am pm and offers 120 places. Summer Holiday Scheme with Phoenix (for Phoenix pupils only) A 3 Provide a three week summer holiday scheme in an inclusive setting for children living in Tower Hamlets and attending Phoenix School. A range of indoor and outdoor activities are organised such as arts and craft, play sessions and outings to attractions. Breakfast, snacks and hot lunch provided as well as transport to meet needs. 30 places are available for each scheme although this may differ each year. Directory of Services for Disabled Children 13
14 Autistic Leisure Group (age 5-19 years) A 3 Provide stimulating activities to encourage independence, integration, creativity with social and cultural events for children and young people with autism. Door to door transport provided. Service available on Wednesdays from 6.00pm pm and 6 places are available for each session. After School Club (age 8-19 years) B 3 Provide positive activities for children and young people with complex needs. Service available for two age groups: year olds (senior group) and 8-13 year olds (junior group). The club for the senior group operates Monday to Friday from 3.30pm pm and has 12 places available for each session. The club for the junior group operates on Fridays from 3.30pm pm and has 12 places available for each session. Children and young people are given the opportunity to engage in a range of activities such as glass painting, photography, music and dance, pottery, sporting activities and cooking. After School Deaf Club (age 5-25 years) B 3 Children and young people have access to artistic material, IT provision and other activities. Service offered to two age groups: 5-12 year olds (junior group) and year olds (senior group). The club for the junior group operates Monday to Friday from 4.00pm pm and 15 places are available for each session. The club for the senior group operates on Mondays, Tuesdays, Thursdays and Fridays from 6.30pm pm and places are available for each session. Service also provides holiday schemes at Easter time, for 1 week, and during the summer, for 3 weeks. Service is also available to their siblings, carers and family members. Provide an environment that is safe, secure, stable and that ensures children are suitably cared for. Places are available for up to 20 children who will have access to a 3 hour activity session per week. Befrienders offer one to one support to each child enabling the child/young person to get out and about and enjoy a range of activities which may include attending a youth club, a visit to a leisure centre, going out for a meal, a shopping trip or staying at home engaging in indoor activities. 15 Aut and About (age 8-19 years) A 3 A Saturday community based service for children and young people aged 8-13 years (junior group) and years (senior group) with autism, offering group based indoor and outdoor activities from 10.30am pm, term time only. Service takes place fortnightly and serves 20 children/young people and takes place at the Atlee Youth and Community Centre, 5 Thrawl Street, London E1 6RT. Barnardo s, Gladstone Place, 1 Ewart Place, Bow, London E3 5EQ Tel: Family Link (age birth - 18 years) A&B 3 Offer high quality support to disabled children and young people by offering play and positive activities to meet an individual s needs. A support worker directly engages in indoor and outdoor leisure activities with the child or young person and their family during agreed hours. The support will be based either in the family home or local community. Each session will take place after school, college or at the weekend. A session lasts for up to 4 hours or less and takes place at the same time each week. Service is for children and young people and around 50 volunteers are trained to become befrienders. The youth clubs and after school clubs include football coaching, drama club, dance club and a range of activities such as pool, video games and table tennis. The Inclusion Workers also offer autism-specific support and training to mainstream youth clubs and after schools clubs to help them fully support children and young people with autism. 16 Saturday Sibling Group (age 8-11 years) A&B 3 This group is for pupils with siblings in other schools. The service aims to enable pupils to interact and play together, build new skills and work as a team. A session lasts from 9.30am 1.00pm and during this time a variety of activities is offered such as arts and crafts, sports, swimming, music and cooking. The pupils have a snack time and then take the school bus home escorted by a member of staff. The sessions are held on Saturdays, once a month. Easter play scheme operates for 4 or 5 days for children aged 7-10 years during the Easter break and 25 places are available (for Stephen Hawking School pupils only). The summer scheme lasts for three weeks during the summer period and has places available for 45 pupils (aged 2 to 10 years) with priority given to pupils of Stephen Hawking School. Examples of activities organised are swimming, outings such as a trip to the London Zoo, cooking and story telling. Services by Scope can be accessed through the self-referral process for short breaks however places are limited. Their services can also be accessed via direct payment which requires an assessment. To access Scope services through direct payment, please contact the Children with Disabilities Team on and the Duty Worker will direct you to the right person who will guide you through the assessment process. 17 Short Breaks: Family Activities Family Holiday Break (age birth - 19 years) A&B 3 Provide five weeks of short breaks per year to allow 5 families (with up to 6 individuals per family/ family friends) to have a 5 day supported holiday break. An additional break may take place during the Easter break or other school break. The service includes self-catering facilities, food pack including halal meat, transport, swimming, sensory theatre, adventure playground, junior playground, animal farm, gym, basketball, football, tennis, cycling activities, radio and recording studios. The holiday takes place in Dorton House, Wilderness Avenue, Seal, Sevenoaks, Kent (TN15 0EB). A range of activities are offered including exciting sensory tent. The session offers children a range of possibilities and adults an opportunity to build supportive networks with others. There is a book store for borrowing relevant books on living with a child with special needs. All Children can borrow toys from the Toy Library to extend the play to the whole family in the home throughout the week. 18 Overnight Short Breaks Weekend Breaks (age 7-17 years) B W This is a short break for children and young people with disabilities offering a range of activities that are educational, social, leisure and recreational. The service runs 270 weekend blocks during all school terms and the weekend block starts from Friday 4.00pm to Monday 10.00am. 6 children are allocated a slot per weekend. Transport is provided. Holiday Breaks (age 7-17 years) Service provides overnight short breaks for a maximum of 7 days to children and young people with a disability offering a variety of recreational activities according to children s wishes and choice. The breaks are provided for 7 disabled children and/or young people at any one time. After School Club (age 7-17 years) B W B 3W Runs every Tuesday from 3.30pm pm providing indoor activities such as arts and crafts, music and play therapy. The service provides for 8 children/ young people. The after school club may also be accessed by young people without the need for an assessment. Day Care (age 7-17 years) B W Day Care weekend short break is provided to prepare a child for overnight short breaks over a specified period. The weekend service runs on Saturdays and Sundays from 9.00am pm on each day. Day Care also provides a programme of activities that contribute to a healthy lifestyle from 4.00pm pm on Tuesdays and Fridays and a minimum of 22 children are served each day. Weekend Breaks for Children with ASD (age 7-17 years) A W The service offers overnight short break for children at the higher end of the spectrum of autism for one weekend a month. A minimum of 5 places is available for the weekend offering meaningful social and recreational activities. 19 Overnight Short Break at 1 Discovery House (age 7-17 years) A W This is an overnight service offering holiday and weekend breaks, day care and after school clubs to children and young people with autism or challenging behaviour. The service allows young people an opportunity to engage in a range of stimulating activities such as sports and outings to support the inclusion, independence and empowerment of children and young people benefiting from the service. Up to 5 places are available for each weekend and school holiday overnight service. The After School Club may be accessed directly without the need for an assessment. Children are allocated up to 21 nights per year, seven are to be taken during weekends or school holidays, seven during term times and the remaining seven as occupancy allows. Parents/ carers have the option to stay with the children in one of the family flats. 20 Family Support Services Early Intervention for Children with Autism (age birth - 5 years) A 3 A family centred approach to autism that offers parent groups and individual support work (1:1 sessions). Service includes coaching, modelling, video-reviews and some direct work with the child with the aim to increase effective involvement from parents. Service for families with children with developmental delays including communication, behaviour problems, attachment difficulties or autistic/pre-autistic behaviours. The service caters for children and their parents/carers a year. All parents or carers interested in receiving support from MindBuilders are advised to attend an Introductory Session which is organised once a month. This will help parents and carers gain an insight into the work of MindBuilders and lead to further support via 1:1 sessions. Support is offered at post-diagnostic stage as well as at other times of need. The Family Support Worker runs a Help! programme (post diagnostic programme) or Help!2 workshop (on a specific ASD-related topic) each term. The Family Support Worker works 9.30am 5.30pm Monday Friday. 23 Other Social/Sport Activities and Services Positive Activities for Young People (PAYP) Programme (age 8-19 years) A&B 3 PAYP supports young people (with priority given to year olds) by offering a range of activities during school holidays. By engaging them in positive activities, the programme encourages young people to engage with their community and make valuable use of their free time during holiday periods. Some provisions cater for the needs of young people with learning difficulties/ disabilities (LDD) or Special Educational Needs (SEN). Where there is concern about a significant mental health difficulty there is a dedicated Neurodevelopmental Care Pathway.
