1 -ATHLETIC TRAINING/SPORTS MEDICINE SERVICES- SCOPE OF PROPOSED SERVICES: Halifax Area School District 3940 Peters Mountain Road Halifax, Pennsylvania The Halifax Area School District wishes to secure professional services for the and school years in accordance with the specifications for Athletic Training/Sports Medicine. Alternates for school years , and will also be considered. CRITERIA FOR PROPOSAL ACCEPTANCE: Proposals will be evaluated by the Halifax Area School District and will be ranked with respect to criteria specifically developed to examine the technical competence and suitability of prospective providers. The contract will be awarded to the most qualified and responsible proposal for the most reasonable cost. In order to qualify as responsible, a prospective provider must meet the following criteria as it relates to this request for proposal: 1. Possess the adequate professional, technical and financial resources for performance of required services or have the ability to obtain such resources as required during performance of said services. 2. Possess the necessary experience and technical skills in the field. 3. Prove a satisfactory record of performance of services for other organizations. 4. Propose compensation that is considered fair and equitable to the level of effort to be expended. 5. Address all elements of the request of the proposal as specified in this document. 2 CONTENTS OF PROPOSAL: The proposal shall contain the following information: -An overall introduction to the proposal, including a statement of understanding with regards to the services to be provided. -An explanation of the role of the provider, as it relates to the Halifax Area School District, including the division of work between the provider and the Board. The provider s response must address the general responsibilities as outlined in this request for proposal and all additional general responsibilities the provider wishes to include. -Define specifically what type of data, information, and/or other assistance will be expected from the Board or school administration (in addition to the stipulations listed in Duties of the School District ) and describe how this will be integrated into the proposed services. -Costs, including a breakdown between any billable expenses and direct contract costs. -Identification and background information of key personnel contributing to services. -A list of clients for whom the proposer has performed services similar to those described in this request for proposal. -The proposal shall be signed by an individual authorized to bind the Provider and shall contain a statement to the effect that proposal is a firm offer. It should include the name, title, address, and telephone number of the individual(s) with authority to negotiate and contractually bind the provider and who may be contracted during the period of proposal evaluation. -The provider must submit a statement that they are an equal opportunity and affirmative action employer, if applicable. CONTRACT PRICE: The cost of service should be presented on the attached Price Proposal Form. Pricing should represent what the Board can expect to pay, upon completion of all assignments during the contract period as described in the specifications. At the time of negotiations of the contract, a payment schedule will be agreed upon between the Board and the professional service provider.
3 JOB DESCRIPTION: Athletic Training/Sports Medicine personnel will be responsible for the coordination and administration of a comprehensive health care program (1000 hours total) for all Halifax student athletes. Primary duties on the field will commence with the first day of pre-season athletics for the fall sports and continue through the end of the school year. The athletic training/sports medicine staff will also be available for consultation in the off-season regarding injury rehabilitation, strength training and conditioning program development and nutritional counseling. Service coordination will be provided on a year round basis. The proposed sports medicine program will include: methods for prevention of injuries, strength training and conditioning consultation, immediate care of acute injuries, continuing care of chronic injuries and rehabilitation of injuries, under the direction of the School District s physician, the athletic training staff or the athletes personal physician. QUALIFICATIONS: The athletic trainers will have appropriate training in sports medicine, be certified by the National Athletic Trainers Association, and maintain the Association s standards. HALIFAX ATHLETIC PROGRAM: The following sports are offered in the Halifax Area School District: Varsity Junior Varsity Middle School Football X Cheerleading X X Girls Soccer X Boys Soccer X Volleyball X Golf (Coop at Mbg) Wrestling (Coop at Hfx) X X Girls Basketball X X Boys Basketball X X Swimming Archery X Baseball X Softball X Track (Coop at Mbg) Strength
4 GENERAL RESPONSIBILITIES: The athletic training/sports medicine program Provider must address the following expectations: -Develop a plan, in cooperation with the Athletic Director, for providing athletic training/sports medical coverage at all interscholastic events and practices. -Provide services for any School District team that qualifies for postseason playoffs. -Create and maintain a regular schedule in the training room: Monday through Friday. Saturday hours will be determined by game and practice schedules. -Assist the School District s physician, upon request with summer, winter and spring physicals. -Advise the School District as to the supplies and training equipment required for the School District s athletic program. -Assist the School District s coaching staff in the design and implementation of a student athletic trainer program. staff. -Provide continuing education programs to the School District s coaching -Attend preseason coaches meetings, and upon request, PTO and school board meetings. -Coordinate and provide injury follow up and evaluations for School District s team physician. -Monitor athletic injuries and develop injury prevention training and programs with the advice and consent of School District s team physician. -Ensure any agent, employee, or contractor of the provider maintains a valid driver s license. -Ensure any agent, employee, or contractor of the provider, having unsupervised contact with students, submits for review the appropriate clearances and background checks. -Carry professional liability insurance and deliver to the School District a certificate for said insurance, which shall name the District as an additional insured and shall provide not less than thirty (30) days prior written notice of cancellation to the School District.
5 DUTIES OF THE SCHOOL DISTRICT: -The School District shall coordinate the establishment of an athletic training room(s) at the School District s facilities, as per mutual consent of the Provider and the School District. -The School District will provide safe parking and unlimited access to any training room used to provide the athletic training. -The School District will provide all reasonable supplies to enable the Provider to provide the athletic training services under this Agreement. -The School District will provide an ambulance in accordance with PIAA guidelines for football games. -The School District will assist the Athletic Director in the implementation of appropriate documentation for participation in organized sporting events, including, but not limited to, required forms by the School District s league, and/or the PIAA. These records shall remain the joint property of the School District and the Athletic Training Services Provider, with the School District s Athletic Director being the keepers of the records and responsible party in the completion of these records. -The School District will list the Provider on appropriate vehicle insurance policies for the purpose of operation of appropriate motor vehicles. -The School District will provide the Provider with communication capabilities with all staff (i.e., cell phone, computer, walkie talkie). -The School District will provide the Provider with any and all changes to athletic schedules no later than 24 hours prior to the rescheduled contest. -The School District will provide the Provider with all necessary teaching materials to help promote and provide a student athletic trainer s program for the School District. -The School District will provide a team physician. -The School District, at its sole cost and expense, shall obtain and maintain general liability insurance. The School District shall deliver to the Provider a certificate for said insurance, which shall name the provider as an additional insured and provide not less than thirty (30) days prior written notice of cancellation to the Provider.
