1 McGill Sport Medicine Clinic Level 1&2 Varsity Costs, Services, Coverage (Revised June 26, 2010) 1. Philosophy Athletics and Recreation is committed to providing men s & women s basketball, cross country, hockey, soccer, rugby, track & field, swimming, men s football, lacrosse and women s volleyball, with priority service in the McGill Sport Medicine and Winsor Clinics. 2. Staff Assignments The Executive Director of Athletics appoints all full-time therapy personnel. The Sport Medicine Clinic Manager appoints all part-time therapy personnel. The Associate Medical Director appoints all physicians. 3. Practice/Game Coverage Winsor Clinic: A certified athletic therapist will be on duty in Winsor Clinic, Monday to Friday, from 4:00-8:00 pm as well as Saturday and Sunday from 1:00-5:00pm. (August to March). Services include injury triage, minor rehabilitation, student therapist supervision as well as liaison with varsity coaches. In-Season Practices: A student therapist will attend in-season practices. A staff therapist will attend all football practices. In-Season Games: A student therapist will attend in-season and away games. In the event that a student therapist is unable to attend away games, arrangements for therapy services will be made with the host institution. A staff therapist will attend all home games. A staff doctor will attend all football home and away games as well as home men s hockey games.
2 Conference Playoff and National Championships: Upon request and when possible, the services of a staff therapist will be appointed by the Sport Medicine Clinic Manager for conference playoffs and national championship events. Out-Of-Season: Upon request, special arrangements can be made for out-of-season medical coverage of sanctioned, special events. Teams will be charged for the additional coverage. International Travel: Upon request, special arrangements can be made for international coverage of sanctioned, special events. Teams will be charged for the additional coverage as well as be responsible for the therapists and/or doctors travel expenses. 4. Pre-Participation Physicals (PPP) All Level 1&2 varsity athletes are required to complete a PPP Questionnaire no earlier than July 19, 2010 and no later than August 6, These forms will be reviewed by each team s staff therapist. If deemed necessary, athletes with medical concerns will be required to complete their medical exam Pre Participation Physical prior to the commencement of their season. All Level 1&2 rookies, plus all football and men s hockey athletes must complete a PPP, at a current cost of $39.00, prior to the commencement of their season., The PPP must be completed by McGill clinic physicians on Athletics and Recreation medical forms. Staff therapists in conjunction with the head coaches will be responsible to ensure that all athletes have acquired pre-season medical clearance. Final Team Roster All athletes on the final team roster must be present on the day of their scheduled Pre Participation Physicals to: Receive a full physical (rookies) or choose to see the team doctor (veterans) Present a valid provincial health card or Blue Cross card (not copies) and a valid McGill ID card Sign administrative policy forms Pay their medical and therapy fees (see cost chart below)
3 Failure to Attend: Athletes unable to attend their scheduled PPP must contact the Clinic Manager in order to re-book on the next scheduled PPP date.. Penalty: If a first-year athlete or veteran misses their PPP without prior notice, a $39.00 fine is levied. Special Note: In all cases, any athlete that misses their PPP will be deemed ineligible to participate until their PPP has been completed. 5. Medical Appointments Therapy and doctor appointments in the Sport Medicine Clinic are on an appointment only basis. Athletes who do not have their original health card (copy not accepted) or who fail to attend their scheduled appointment with a physician will be charged the regular no show fee of $50. As with all patients, not showing up for a therapist s appointment, arriving late or canceling less than 24 hours in advance will result in a $45.00 charge (full cost of the treatment). This cost is not covered by Sunlife Insurance or the Therapy Fee Fund. 6. Sunlife Health Insurance Coverage According to the McGill SSMU and PGSS plans (covered by Sunlife), undergraduate and graduate students are covered for $20 per treatment, up to and including 20 physiotherapy or athletic therapy sessions. A. For Varsity athletes covered by SSMU or PGSS: 1. Athletes are permitted to draw 20 treatments from the Therapy Fund. 2. After 20 treatments, if an athlete switches to another type of licensed therapist, they are required to pay $25 per visit. Sunlife will cover the balance up to 20 treatments of that particular form of therapy. If an athlete chooses to remain with the same license, they are required to pay full price ($45). 3. After 40 treatments, athletes are required to pay full price. Athletes who require more than the allowed number of treatments (20) are encouraged to acquire supplemental health insurance. Athletes are urged NOT to opt out of the physiotherapy/athletic therapy portion of the Sunlife Insurance policy paid at the time of academic course registration.

For athletes who opt out of the SSMU or PGSS plans OR those who are covered by Blue Cross (international students) They must pay $20 per treatment up to 20 treatments, since the clinic cannot directly bill private insurance companies or Blue Cross. The remaining $25 is still deducted from the Therapy Fee fund up to 20 treatments. Athletes who do not have access to supplemental health insurance will be given a home program from the Winsor Clinic staff, when possible. 7. Therapy Fee Fund Costs: $97.00 Football, men s hockey & rugby $87.00 Men s and women s basketball, cross country, soccer, T&F, men s lacrosse and women s rugby. $77.00 Swimming, women s volleyball Refund: Services: Players who quit or are released from a team may request a Therapy Fee Fund refund. This can occur up to and including the day of the team s first official game, and only if the player has not received treatment during their pre-season. 1. Minor rehabilitation in Winsor Clinic 2. Payment of treatment balance not covered by Sunlife Insurance. 3. Administration of the rehabilitation claims. 8. Injury Reporting/Return To Play Guidelines Athletes should report all injuries to their team therapist or the on-duty staff therapist in Winsor Clinic. Staff Therapists are responsible to triage athlete injuries, as well as coordinate doctor and therapist appointments. It is the team therapist s responsibility to report an athlete s playing status to their respective coaches. 9. Athletes Playing Status Due to Injury A McGill Sport Medicine Clinic staff doctor will make all decisions regarding an athlete s playing status due to injury. In the event of a dispute, the Associate Medical Director will make the final medical decision.
5 The Sport Medicine Clinic and Intercollegiate Managers will jointly resolve any disputed administrative concerns. This may be done in conjunction with the Associate Medical Director and Executive Athletics Director (if required). 10. Consent a) General I agree to undertake a pre-participation physical exam in order to ensure that I am fit to train and compete. I am aware that some information may require clarification or follow-up with my treating doctor and physio/athletic therapists, and agree to the release of relevant information to these people. I am aware that medical fitness issues may be discussed with my coach. I understand that the information contained in this form is otherwise confidential and can only be released with my consent. I hereby state that, to the best of my knowledge, my answers to the above questions are complete and true. b) External Consultation I understand that if I choose to seek outside medical advice for an injury or medical condition that interferes with my ability to play, I will have to be reevaluated by a McGill Sport Medicine Clinic doctor before being cleared to returnto-play. I understand that in order to facilitate this re-evaluation process, it is my responsibility to provide the McGill Sport Medicine Clinic doctor the test results, reports and/or images from the external sources. If there is a discrepancy of opinion concerning return-to-play between an outside physician and a McGill Sport Medicine Clinic doctor, I understand that the McGill Sport Medicine Clinic doctor s opinion will prevail.
