2 2 Surrey County Council Care and support for adults in Surrey 3 Introduction We understand that there are times when you may need extra support to do every day things. This can be due to age, a health condition, disability or changing home circumstance, or you may be a carer in need of some help or a break from caring. Often other family members can help out, or friends or neighbours who live nearby, but when help is not available there are a number of options for you to consider. This booklet is here to inform you about various options available to help you fulfil a safe and independent life. Contact details for the services mentioned are listed at the end. Helping you find information and advice The Surrey Information Point website can help you to search for local care and support services, including organisations, community and faith groups who do a great deal to support people in their own communities. These organisations can offer assistance with a range of services from transport to cooking, cleaning and shopping. If you d prefer to talk to someone, you could visit one of the local information Hubs and speak directly to someone who will help you understand the options open to you. There are currently eight hubs in Surrey, all in local centres and open and accessible to everyone - please see below for details of finding the one nearest you. Alternatively, you can contact Age UK Surrey to discuss options.
3 4 Surrey County Council Care and support for adults in Surrey 5 Helping you remain independent Feeling safe Helping you stay happy and independent in your own home is important. Sometimes this can be done by simply adding a piece of equipment or making some adjustments to your home. Our online equipment assessment tool has been designed with you in mind and helps you find the right equipment to suit you, or the person you care for. It will also tell you who will stock the item/s you need. One Stop Surrey is a great new service from a number of organisations which you can use to get the right kind of support or service you need at home to live and age well. Visit the website for a wide range of information, from how to access domestic help or a befriending scheme to prevent loneliness, to finding out how to get free loft insulation. Everyone should feel safe in their own home and having a telecare community alarm system can help. Just knowing that there s someone at the end of the phone if you need them, 24 hours a day, seven days a week, will give peace of mind to you and your family. Protecting you from fire is really important and Surrey Fire and Rescue Service offer older and vulnerable residents a FREE home fire safety visit. Get in touch with them to arrange a visit. If you or someone you know in a vulnerable position is at risk of abuse, please report it to the Adult Social Care helpline, or in an emergency dial 999. Did you know that your local borough and district council can help too? They provide a range of support and services such as community transport, and equipment and adaptations for the home to help people move around safely. You will find their details on page 12 of this booklet. People experiencing memory loss can also benefit from advice and support services. The local Wellbeing centres are provided by a number of borough and district councils in convenient locations across Surrey, to offer specialist support and advice, and techniques and equipment that will prompt you to remember important information such as dates, routines and when to take medication. 4 6 Surrey County Council Care and support for adults in Surrey 7 Support for carers of all ages Help with financial matters Carers look after family, partners or friends in need of help because they are ill, frail or have a disability. The care they provide is unpaid. If this sounds like you, then you are a carer and your local carers support scheme is there to help you. Visit Surrey Information Point for local details and see what support people in a caring role can get. Alternatively, please contact Action for Carers (Surrey). You can also request a carer s information pack from the Adult Social Care helpline. If there are any children in the household affected by the caring situation, they may be a young carer. Surrey Young Carers supports young carers under 18, and offer a wide range of care and support options aimed at young people. Carers of all ages have a right to an assessment of their needs. To find out more, please contact the Adult Social Care helpline. Many people need help accessing extra money or benefits to help them with their situation. GetWise is a partnership made up of a number of organisations that can help you maximise your income. Some residents may benefit from independent financial advice around care choices, especially those who already pay for their own care without council support. Making a life changing decision can be daunting, especially if you aren t aware of all the options available to you. We strongly recommend speaking to an independent financial adviser, who will be able to provide impartial advice and help you navigate through the complexities of care funding. So we ve teamed up with the Society of Later Life Advisors (SOLLA). Visit their website or call their phoneline for local Surrey advisers. Please note that some services they provide are free, others are chargeable. Keeping up to date There are various ways in which you can keep up-to-date. If you have a registered disability, signup to the Surrey Disability Register. As well as offering a host of benefits, such as discounts and opportunities to have your say, they send out a twice yearly newsletter packed full of useful information. The register can also identify people who may need support in the event of an emergency, such as extreme weather or flooding. You can also subscribe to our Living and Ageing Well e-newsletter too, just drop us an to and we ll add you to the distribution list.
5 8 Surrey County Council Care and support for adults in Surrey 9 When and how Adult Social Care helps Will I have to pay? There s a huge choice of support available provided by many organisations in Surrey. The Adult Social Care service at the county council is here to support residents who have longer term and more complex needs that prevent them from doing routine things each day, and carers. We are also here to provide short term support to help you get back to full fitness or mobility after a hospital stay or if you are recovering from a mental illness. If we find you need our support we can speak to you and find out about the areas that matter most to you and would make the greatest impact on your life and wellbeing. You then have the option to choose the support (with or without our help) that will allow you to achieve the goals that matter most to you. We regularly review these changes to make sure that you are getting the most out of our services. We understand that some people may need additional support when making these decisions, which may be due to mental capacity or communication difficulties. In these circumstances we are happy to arrange for an advocate who will assist in sharing your views. You do not have to pay for an assessment of your care and support needs but you may have to pay towards the support you receive. The amount you pay will depend on your savings and income. We will carry out a financial assessment to work out if you need to pay a contribution. We will also check that you are receiving the right benefits. Some services, such as equipment and minor adaptations are free of charge. Carers are not charged for services they receive directly. More information is available in our Paying for Care leaflet. In 2016 a care cap will be introduced by the government which may affect the way you pay for your care. Find out more about this at surreycc.gov.uk/careact or call the Adult Social Care helpline. The support you choose may be provided by another organisation or person. We prefer to put you in charge of arranging your own support by giving you a direct payment. A direct payment is an amount of money to spend on support or services of your choice to meet your eligible social care needs. We will usually provide you with an account to make managing the direct payment easy. If you cannot manage a direct payment, we will discuss other options with you.

Contact details for the services mentioned If you have concerns about the decision we have taken, we would ask you, in the first instance, to contact the person (or team) you have been dealing with to let them know what your concerns are and ask them to review the decision.
