Read More on Page 4 Busy mom Carmen Gonzalez chose our Diamond Neighborhoods Family Health Center as the Primary Care Medical Home for her son Aaron (shown here doing a check up of his own on Dr. Hippen). Like many San Diegans, Carmen recently lost her job and is looking for work. Aaron s dad has steady work as a mechanic, but his job doesn t provide health insurance for the family. Thankfully, Carmen found our health center so her active preschooler gets all of the healthcare services he needs!

Thanks to the San Diego Breastfeeding Coalition and Rest Haven Children s Fund, Family Health Centers of San Diego now has a new breast pump loan program! Through the program, we can loan 10 multi-user electric pumps to new moms. Breast milk is best for babies, and now we can better support new breast feeding moms who can not afford to purchase the pumps on their own. 4 Recognition Designated Primary Care Medical Home Since our founding in 1970, Family Health Centers of San Diego has been dedicated to providing coordinated care to our patients. This summer, The Joint Commission formally designated Family Health Centers of San Diego as a Primary Care Medical Home (PCMH). The PCMH criteria demonstrates an organization s commitment to ensuring their patients receive timely and well-coordinated treatment. The criteria promotes increased patient satisfaction, improved patient outcomes and reduced overall costs to the health care system. The PCMH model exemplifies the new federal health care reform efforts to improve health outcomes and insure the coordination, quality and efficiency of health care services. Physician Assistant Lori Saunders holds Leondrus White, who came to the health center with his mom Leantonette LeeLee White. LeeLee also received case management and support services through our Black Infant Health Program during her pregnancy - just one part of the comprehensive service system that makes up our Primary Care Medical Home system of care. Family Health Centers of San Diego is honored to receive this external validation of our care delivery system. Our dedicated clinical and support teams work together to coordinate high quality patient care and provide an exceptional patient experience. 5 Site Expansion NEW Health Center at Rice Elementary in Chula Vista Great news! We just received a federal school based health center grant to establish a health center on the campus of Rice Elementary in Chula Vista! The new health center will open in 2012 through a partnership with the Chula Vista Elementary School District. It will provide healthcare services to Rice students, helping them stay healthy and in school, as well as their families and local community. Everyone is excited about the new health center at Rice Elementary in Chula Vista! Watch City Heights Grow We are excited about the progress of our new Family Health Centers of San Diego s City Heights Family Health Center. Each day of construction we move closer to our new and expanded health center. The center will provide comprehensive healthcare and support services that will include: Adult Clinic with 11 Exam Rooms Pediatric Clinic with 12 Exam Rooms Women s Clinic with 6 Exam Rooms Dental Clinic with 7 Operatories Developmental Services for Children, Birth to 5 Years Old HIV Prevention, Outreach and Testing Radiology Full-Service Laboratory Multi-Purpose Community Room First Floor Parking Patient Care Support Team Focused on Preventing and Managing Chronic Disease Family Health Centers of San Diego s City Heights Family Health Center is well on its way to completion. Beneficiary: Term used to describe a person who benefits from a trust, will, IRA, or other assets. Estate: All property you own, in any form, that will someday pass to loved ones and/or charitable organizations. Trust: A structure that outlines ownership and management of your property. Trustee: The individual or entity charged with managing trust assets on a day-today basis. In most cases, this is you. Successor Trustee: The individual or entity charged with taking over management of trust assets if the trustee becomes incapacitated or dies. Will: Instructions to guide distribution of your property or estate. When used in tandem with a revocable living trust, it normally directs the distribution of personal property. Executor: Person or entity chosen to carry out the terms of your will; sometimes referred to as a personal representative. Power of Attorney: A legal document empowering others to take action on your behalf. Durable Powers of Attorney are used for the purpose of empowering others if you become incapacitated. Advance Health Care Directive: A document that declares your wishes outlining medical care and life support. This document is also used to appoint someone to make medical decisions on your behalf if you are unable to make these decisions for yourself. Please always consult with your attorney or professional advisor as you take steps to plan your estate now.
