13 ACHA Guidelines Recommendations for Institutional Pre-Matriculation Immunizations The following recommendations are provided to colleges and universities to facilitate the implementation of comprehensive institutional pre-matriculation immunization policy. Vaccine-preventable diseases continue to occur on American campuses. In response to changing epidemiology and the introduction of new vaccines, the Vaccine-Preventable Diseases Task Force monitors age-appropriate public health recommendations and updates this document accordingly. The task force recognizes that many colleges and universities are mandated by state law to require certain vaccinations for matriculating students. States and educational institutions may require fewer or more vaccines, while some may only recommend certain vaccinations. This document is intended as a guideline that is consistent with the Advisory Committee on Immunization Practices (ACIP) recommendations. Tuberculosis screening is complex and guidelines are currently under revision by the CDC. The sample record includes a suggested TB screening guideline, which will be updated when available from the CDC. A recommendation related to the influenza vaccine is not included in the guidelines for a PIR. However, the ACIP and/or the ACHA recommendation regarding influenza vaccination is included as a footnote that follows guidelines. Vaccine Age Indicated Major Indications* Major Precautions* Measles, Mumps, 1st dose at age All entering college Pregnancy; history of Rubella (MMR) months or later; 2nd dose students born after anaphylactic reaction to at age 4-6 years or later eggs or neomycin; immunosuppression. History of a neurologic Pertussis with DTaP or DTP, booster hypersensitivity Diphtheria, tetanus at age years with Td, reaction following a toxoid, and acellular then every 10 years. previous dose (whole-cell) pertussis: DTaP (DTP) - Diphtheria and tetanus toxoid (Td) Polio Vaccine Primary series in childhood IPV for certain OPV should not be - Inactivated (IPV) with IPV alone, OPV alone, international travelers. given to - Oral poliovirus (OPV) or IPV/OPV sequentially immunocompromised booster only if needed for or HIV antibody travel after age 18 years. positive persons. *Refer to the appropriate Advisory Committee on Immunization Practices (ACIP) recommendations for more details.

14 Vaccine Age Indicated Major Indications* Major Precautions* Varicella Childhood, adolescence, All entering college students Pregnancy young adulthood. without a history of the disease or without age appropriate immunization or with a negative antibody titer (two doses at least one month apart, if over age 13 years). Hepatitis B Series of 3 doses (given All college students. None Vaccine at 0, 1-2 mo., and 6-12 mo.) prior to college entry Meningococcal quadrivalent polysaccharide vaccine: Recommendation Over 2 years, repeat every Certain high risk groups None for vaccination 3-5 yrs. including persons with terminal complement deficiencies or those with asplenia. Research or laboratory personnel who may be exposed to aerosolized meningococci. Recommendation for information and access to meningococcal polysaccharide vaccine: ACIP has revised its recommendation for use of the meningococcal polysaccharide vaccine in college students. The vaccine continues to be indicated in the control of outbreaks (serogroups C, Y, W-135, and A); however, the revised recommendation is based on recent evidence that college freshman living in dormitories or residence halls are at modestly increased risk of meningococcal disease relative to other persons their age. Though a final recommendation awaits publication in MMWR in the near future, the major points of the recommendation are summarized below. The ACIP has recommended that practitioners who provide medical care to freshmen, particularly those who live in or plan to live in dormitories: Should inform students and their parents about meningococcal disease and the benefits of vaccination. Should provide the vaccine or make it easily available to those freshmen who wish to reduce their risk of disease. Should provide the vaccine to other undergraduates under 25 years who wish to reduce their risk. The ACIP has recommended that colleges and universities provide information about meningococcal disease and the availability of safe and effective vaccine to freshmen, particularly those who plan to live in dormitories or residence halls. Public health agencies should serve as a technical resource regarding meningococcal disease and vaccine for colleges and providers, including as a source of information regarding how to obtain vaccine.

15 Refer to the appropriate Advisory Committee on Immunization Practices (ACIP) recommendations for more details. Footnotes Other recommendations: 1. ACIP and ACHA encourage all college students (particularly those at high risk of complications from influenza or those living in dormitories) to receive influenza vaccine annually during autumn, to minimize disruption of routine activities during campus-wide epidemics. 2. Immunization requirements and recommendations for international travel may vary, depending on personal medical history and travel destination. Anyone anticipating international travel should contact a health care provider for specific information.
