1 PMP: Spring 2016 Checklist of Required Items for your upcoming studies with Navitas at UMass Dartmouth: Please be sure that we have the correct address in our records by ing Initial Invoice for Pre-Master s Pathways Program Payment due by January 4 Payment of your initial invoice is due within 14 days of receiving your F-1 visa, or at least 2 weeks prior to arrival, whichever is earlier. Please note that you will not be enrolled in classes until your invoice has been paid in full. Payment in full is due by Monday, January 4 in order to register you for Spring Academic Year 2015/2016 Housing Contract return by Tuesday, December 22 Please return your Housing Contract (by ) to Navitas at UMass Dartmouth by Tuesday, December 22. Your housing on-campus cannot be secured without the housing contract that is included in this packet. A FAQ sheet has also been included in this packet which provides helpful information about living in campus accommodation. Please note that this Academic Year Housing Contract is pro-rated to include only the Spring 2016 semester. Students who elect to stay during the Summer, will be required to sign a Summer housing contract before the end of the Spring semester. UMass Dartmouth Health Forms return by Tuesday, December 22 Please submit your completed health forms, with accurate records for all required immunizations, to Navitas at UMass Dartmouth by Tuesday, December 22. Students in the United States are not allowed to live on-campus or attend classes without these immunizations, so it is important that these are sent to us prior to your arrival on campus. If you have questions about the required immunizations, please feel free to contact us. Please these forms to us once completed, we do not need originals. Travel Itinerary return by Monday, January 4 As soon as you have received confirmation of your visa, please begin making travel reservations to arrive to Dartmouth, Massachusetts on Wednesday, January 20! Please note that students living on campus will not be allowed to check in to the dorms until Wednesday, January 20. If you will arrive before this date, please plan to make temporary accommodation arrangements off-campus as early check-ins cannot be arranged. If you require an Airport Pickup from Boston or Providence, you must complete the online Airport Pickup Request by Monday, January 4 with your full flight itinerary. The approximate cost of the airport pickup service is $160 USD (varies based on total number of students who request this service). Please complete the online arrival form as soon as you have booked your flights, and us your flight itinerary (even if you don t need airport pickup): International Student Orientation begins on Thursday, January 21 Orientation, which is mandatory for all new students, begins on Thursday, January 21 and continues on Friday, January 22 so you must plan to arrive in Dartmouth, Massachusetts by Thursday. All students will be required to check in with the Navitas staff to report their arrival on Wednesday, January 20. The campus residence halls will be open for move-in between 5:30pm-8:00pm on Wednesday, January 20 and Thursday, January 21. Please note that if you arrive before Jan 20 or after 8:00pm on either date, you will need to make arrangements to stay at a hotel until the residence halls are open. Early residence hall move-ins cannot be arranged for students who arrive prior to January 20. Pre-Departure Guide An online pre-departure guide and other important updates will be ed to you prior to your arrival, so it is important that we have the correct address on file and that you check your often. 2 PMP Housing Contract FAQs What you need to know before returning your contract to Navitas at UMass Dartmouth The University Housing Contract is a legally binding contract by signing and returning the housing contract to Navitas at UMass Dartmouth you are legally and financially responsible to fulfill the terms of the contract in accordance with the Terms and Conditions. The contract will be voided if you do not enroll with Navitas; however contract releases are very difficult to process once you are enrolled. Please review the UMass Dartmouth Housing Terms and Conditions at: Duration of Contract The University follows an Academic Year schedule. The academic year begins in late August with the start of the Fall semester and ends in early May at the close of the Spring semester. By signing an Academic Year contract for an August start you are committed to live on campus for both the Fall and Spring semesters. By signing an academic year contract for a January start you are committed to on-campus housing for the duration of the Spring semester (Jan May). You will later have the option to re-sign a contract if you wish to live on campus during the following academic year. Staying on campus during the school breaks Students who need accommodation on campus during the breaks (when school is not in session) must complete an application form prior to each break, and will be subject to additional fees if they have an Academic Year contract. Students on a 10 or 12 month contract must also complete an application form prior to the break, but will not be subject to additional fees. Students with an Academic Year contract, who plan to leave campus during the short-term breaks, can leave their belongings in the room at no additional charge. Semester breaks include Thanksgiving Break (late November), Winter Break (mid- December - late January), Spring Break (mid-march) and Summer Break (mid-may August) 10/12 Month Contracts Signing a 10 month contract allows students to stay on campus during the Thanksgiving, Winter and Spring Breaks with a 40% discount on the weekly rate (included in the contract rate). Signing a 12 month contract allows students to stay on campus during the school breaks as well as during the summer. Please us if you would like more information on a 10 or 12 month contract. Summer Housing Applications - Students joining Navitas for a Summer Pre-Master s Program (May start) should submit a summer housing application to live on campus during the summer. PMP Students can choose to live on campus just for the duration of the Navitas program (10 weeks), with the option to move off-campus at the end of the PMP program in August, or to sign an Academic Year contract to continue living on campus during the first year of your graduate program for the Academic Year (Sept May). You will have the option to sign an Academic Year contract to renew your campus housing for the Fall and Spring semesters, however you will likely need to change rooms at the start of the Fall semester. Accommodation charges The housing and dining rates on the Academic Year contracts are for the full academic year, however housing charges are invoiced per semester. The rate is per student, not per room or apartment, so this cost is not shared among other students sharing a room or apartment. Additionally, students living on-campus must pay a residential technology fee each semester. Please note that summer housing is charged based on a flat rate fee for either the summer term only or the summer term plus the 3 week break in August. Please note that housing/dining rates listed on the contract are subject to change without notice and typically increase 3-7% annually, with rate increases taking effect in July. Room Assignments While the housing office will make every effort to assign you a room matching your preferred room type, please know that room assignments are made based on availability of rooms and placement in the room type you mark on the housing contract cannot be guaranteed. Living with other students is a learning experience that adds value to your college education, so we hope that you will approach the opportunity to live with other students with an open-mind. Meal Plans Students living in the traditional first and second year residence halls are required to have a minimum dining plan of 10 meals per week, as there are no kitchen facilities in the students rooms. Students living in the Woodlands apartments or Cedar Dell townhouses have the option to select a meal plan, but are not required to do so. There are two options (traditional meal plans or block plans), so you can select a weekly allowance or a semester allowance depending on the number of meals you plan to use in the campus dining locations. Declining Balance Plans work similar to a pre-paid debit card, and can be purchased in increments of $500, $750, $1000, and $1250. Important Note: Cancellation of or changes to your meal plan will not be allowed after arrival, so you must us prior to your arrival if you wish to change or cancel your meal plan selection from that marked on your housing contract. 3 PMP Housing Contract FAQs Tips to help you complete your housing contract What is Woodlands? - Woodlands community features six residential buildings of apartment-style accommodation for upper-class and graduate students. The apartments are fully furnished and provide single bedrooms with a bed, desk, chair, and built-in closets. The living room area has a couch, chair, and table. The kitchen area of the apartment is equipped with a stove, refrigerator/freezer, ample cabinet space, barstool chairs, and a dining alcove. Most of the apartment units are four-bedroom with two full baths (4bd/2bth); several are two bedrooms/two full baths (2 bd/ 2 bth). *Most Navitas Pre-Master s students will live in Woodlands, 4 bd/ 2bth apartments; however these spaces fill up quickly so please submit your Housing Contract as soon as possible! What is Cedar Dell? - Cedar Dell is an on-campus living community comprised of several townhouse residences for upper-class students. Each townhouse offer six single bedrooms, furnished with a bed, desk, desk chair, dresser, and closet; a shared kitchen with appliances: oven, cook top, full-sized refrigerator/freezer, and ample cabinet space; a living room with couch and coffee tables; a dining area and one half-bath down stairs and one full bathroom upstairs. Cedar Dell also features a centrally-located student community center. What is a Traditional Residence Hall? Traditional residence halls are dormitory style residences, typically occupied by first-year undergraduate students, in which students live with one to three students per room and share a bathroom between same-gender residents living on the same floor of the residence hall. Students living in a Traditional Residence Hall are required to have a minimum dining plan of 10 meals/week. What is Oak Glen or Pine Dale? Oak Glen and Pine Dale are names of two dormitory style residence halls on campus, similar to the traditional residence halls, in which second year students typically live. Each residence hall has a shared common area, kitchen facilities and laundry room for students living in the building to share. Students living in these halls are required to have a minimum dining plan of 10 meals/week. Single room in a Traditional Residence Hall A room occupied by one student, equipped with a bed frame, mattress, desk, and closet/dresser. Students in a single room of a traditional hall will share a bathroom with other students in the building. Please note that single rooms are very limited in availability. Double room in a Traditional Residence Hall or Oak Glen/Pine Dale Hall One room occupied by two students, typically undergraduates. Each student will have their own bed frame, mattress, desk and closet/dresser to use. Triple room in a Traditional Residence Hall or Oak Glen/Pine Dale Hall One room occupied by three students, typically undergraduate students. Each student will have their own bed frame, mattress, desk and closet/dresser. Am I required to live on campus? Students in the Pre-Master s Program are not required to live on campus; however we strongly encourage all students to live on campus, especially for their first semester/year until they become better acquainted with the local area. Public transportation is very limited and students who live off campus frequently face difficulty in getting to and from campus for classes and university activities, especially as classes often run from 6:00pm-9:30pm. If you are looking into options off-campus, please plan accordingly. You can see local listings for off-campus housing through this site: Please Submit your Housing Contract to Navitas at UMass Dartmouth At least 4 weeks prior to your arrival on campus. Your housing cannot be secured without this form, so it is important that we receive it at least 4 weeks prior to your arrival on campus. Students, who return housing contracts close to the deadline date, may be placed in temporary room assignments for the first several weeks of the semester. To increase the likelihood of receiving a room assignment in your preferred room type, please submit your housing contract at least 6 8 weeks prior to arrival. 5 Health Form Requirements: Please note that the enclosed medical forms must be filled out in English and the immunization records must be translated into English as well. All medical forms MUST be submitted before the student is actually on campus at UMass Dartmouth. Any student who has not turned in their medical forms, with completed immunization requirements, will not be enrolled in classes. You are required to have a physician fill out your immunization records on page 2 and return your completed health forms, including all 4 pages, to Navitas at UMass Dartmouth prior to your arrival. These forms are mandatory and failure to complete them will prevent you from being able to move on-campus and register in classes. Students in the United States are NOT permitted to attend a University without these forms! Before returning your health forms, please check the following: Your immunization history must be completed by a doctor! Immunization records for the following must be accurate, complete, and provided from your doctor or health care facility, page 2 should NOT be completed by the student/parent/agent: 1) M.M.R. Measles, Mumps & Rubella (2 doses are required at least one month apart). The first dose must be given at least 12 months from date of birth. 2) TDAP Tetanus, Diphtheria and Pertussis (must be given within the last 5 years) TD and/or DTP is not acceptable in place of the TDAP vaccine. 3) Hepatitis B Vaccine (3 doses required) *Dose 1 and dose 2 must be at least one month apart *Dose 3 must be four months after dose 1 and at least two months after dose 2 4) Varicella Vaccine (2 immunizations required at least 28 days apart) Or positive history of disease (chickenpox) and recorded date 5) Meningococcal Vaccine or signed waiver form All pages must be included, including the Meningococcal Waiver if you have not had the vaccine. Tuberculosis Screening: If you have answered Yes to questions 2, 3, or 4 on page 4 of the health forms, it is required that you provide Tuberculin (PPD/ Mantoux) skin test results. The TB skin test must be done within the last 12 months, and results must be read hours after the test is applied. Note: Students with a positive TB test must provide an x-ray report and documentation that verifies a conversation has taken place with your doctor regarding recommending INH medication regimes. If you answer NO to all of the questions on page 4 you are still required to submit complete immunization records! 12 Navitas at UMass Dartmouth Arrival Confirmation & Airport Pickup Form Arrival Information All students should complete this form to indicate travel plans at least two weeks prior to their expected arrival. All new students must attend the mandatory orientation program, so it is important that you arrive to Dartmouth on or before the scheduled check-in day. You have the option of arranging your own personal transportation to reach UMass Dartmouth or you can take our scheduled airport service, which is approximately $160 USD per person*. If you request an airport pickup, requests must be submitted with your full flight details at least one week in advance of your flight. Students requesting an airport pickup should plan to arrive at Boston Logan International Airport (BOS) or Providence T.F. Green airport (PVD). If you are not requesting an airport pickup service, we ask that you still submit this arrival confirmation form to inform us of your travel plans, and please plan to check in at the Navitas office on campus between 9:00am and 5:00pm on the designated check-in date. Important Note: Students living on-campus, who will arrive to Dartmouth, MA prior to the designated check-in date or after 8:00pm on the check in day, will be required to stay at a local hotel until you can check in to the university residence halls. Early check-ins, prior to the designated check-in date, cannot be arranged. Please refer to the online Pre-Departure Guide for more information and recommendations for local hotels. *Please be advised that this is estimated cost and the final charges will depend on the total number of airport pickups requested. When possible, we will try to arrange group pick-ups to reduce the cost. If you fail to show up within an hour after your plane has landed, or notify us of any changes or cancellation in advance, full fees will be charged for no show. Additional fees apply for late notification of flight changes or delay. Surcharge applies for delivery of excess baggage to your accommodation.
