College Way; Bend, Oregon Instructions for Department Clearance to Register for NUR 095 Term: Winter 2014 THE PRE-REGISTRATION PROCESS AND DEPARTMENTAL CLEARANCE IS REQUIRED EACH TIME THAT YOU ATTEMPT TO REGISTER FOR NURSING 095 (NURSING ASSISTANT CLASS) Carefully read the following information, sign the agreement page and return the signed agreement page to the nursing department Administrative Assistant at least 3 business days prior to your assigned registration date, for the term in which you are trying to register. Departmental Approval will be given only after the signed agreement is received by Nursing Department Administrative Assistant. This does not guarantee that you will be able to register as registration is on a first come first serve basis. It is your responsibility to register for the class. We do not register you in the class. Completely read this packet and understand the requirements of the NUR 095 class. Sign the Agreement Page and give it to the Nursing Department Administrative in the Nursing Department Office, prior to the start of registration for the term for which you are trying to register. Giving this packet to any other person may result in a delay in preapproval, beyond your registration date. Make sure that you have adviser clearance, and confirm that Departmental Clearance for Nursing 095 is visible on your Can I Register Page. NOTE: New students need to not only apply to the college, but also need to complete placement testing or send in transcripts, and follow all Getting Started ( steps prior to registration into the Nursing 95 course. Register for NUR 095 at your scheduled time. It is crucial that you follow the instructions in this packet and meet the required deadlines. Any registered student that does not meet the required deadlines will be withdrawn from class and their seat will be given to a waitlisted student. If you have questions, you may call the Nursing Department Administrative Assistant at (541) All communication will be done through your college account; therefore, it is very important that you check it often. We do not accept lack of communication, including checking your college , as an excuse for lateness. 2 FOR STUDENTS WHO HAVE SUCCESSFULLY REGISTERED IN NUR 095 Step 1 - Criminal History Check [Due Date: Wednesday, December 11, 2013] The Oregon State Board of Nursing requires criminal history background checks on all students admitted to the Nursing Assistant Program. Individuals who have committed crimes, or have a history of arrests that will not allow them to attend clinical, or to be certified, will be denied admission to NUR 095. If you have an arrest record, check the ORS number on your court documents against the crimes listed on the DHS crimes list at You may not register for the nursing assistant (NUR 095) program until your criminal record has been cleared of any listed crimes. Talk to the Nursing Assistant Program Director prior to registering for NUR 095 if you are unsure of your status; -OR- If you do not have an arrest history, the criminal history check must be initiated after successful registration and no later than the indicated due date: On a computer, access enter the program code HJKHW , complete and pay for the criminal history background check with a credit or debit card. The cost is ~$ This is the only criminal history background check accepted by Central Oregon Community College. Step 2 - Immunizations [Due Date: Wednesday, December 11, 2013] Registered students must provide copies of all required immunizations to the Nursing Department Administrative Assistant no later than the indicated due date: Hepatitis B Vaccine : Provided official documentation of three Hepatitis B vaccinations and results of Hepatitis B surface antibody test (titer) showing immunity no later than due date indicated; -OR - Hepatitis B Vaccine series in progress: First dose must be completed no later than the indicated due date and second dose received one month after first dose; -OR- If due, the third Hepatitis B vaccine dose or titer must be done prior to clinical attendance. MMR Vaccine (measles, mumps, rubella): Provide official documentation of two Measles, Mumps, Rubella (MMR) vaccinations, at least 4 weeks apart; -OR- If born before 1957 provide documentation of one MMR vaccination; -OR- Vaccine series in progress: Provide documentation of first dose prior to indicated due date; second dose at one month and completed prior to clinical component of program; -OR- Provide laboratory evidence of immunity: Dated copy of measles, mumps and rubella titer report with results must be included in documentation packet. Varicella (Chickenpox): Provide documentation of 2 doses of Varicella vaccine, 4 weeks apart; -OR- Serological evidence of immunity (titer) to Varicella; -OR- Documentation of the 1 st vaccine and documentation of the second dose within the first 4 weeks of the term. Tetanus, Diphtheria, Pertussis: Provide documentation of a one-time dose of Tdap as an adult

3 2 Step TB Testing (PPD) or blood draw (IGRA): Provide documentation of a blood test (IGRA), either Quantiferon Gold or T-Spot, within the past the past 12 months. OR- Documentation of two negative TB tests, administered 14 days apart, and read within 48 hours of placement by the administering agency. If both are negative, nothing further needs to be done. OR- If either test is positive, provide documentation of the TB tests, a chest x-ray and an evaluation by a physician. OR- Students with a past positive TB test must provide documentation of the positive TB test and, if not already completed, provide documentation of a baseline chest x-ray prior to the deadline. and- All follow up care must complete prior to the first day of class Please note: A TB test cannot be placed within 30 days of receiving the MMR vaccine. Plan accordingly. Influenza vaccine: An influenza vaccine is required for students who will be attending NUR 095, fall or winter quarter. Medical or religious exemption: If you have had a life-threatening allergic reaction to a vaccine or any component of a vaccine, you must provide documentation, on letterhead, from your health care provider. See the Nursing Department Administrative Assistant for documentation of immunization exemption for medical or religious reasons. Step 3-Healthcare Provider CPR: [Due Date: Wednesday, December 11, 2013] CPR cards must be current and valid through the end of the term. CPR cards must comply with Oregon State Board of Nursing rules. Pay attention to the type of card required from each provider. No other card will be accepted. Copy of both sides of a signed, current American Heart Association Healthcare Provider CPR/AED Copy of both sides of a signed, current American Red Cross AED/CPR for the Professional Rescuer Copy of both sides of a signed, current National Safety Council BLS for Healthcare and Professional Rescuers card Step 4 Class Attendance: The instructor will NOT accept required paperwork on the first day of class. You will be withdrawn from class and your seat given to a waitlisted student, if required paperwork is not turned in to the Nursing Department Administrative Assistant by indicated due dates. This is non-negotiable. Attend mandatory orientation on the first day of class and arrive on time to retain your seat in NUR 095.

6 Letter of Agreement for Departmental Approval Winter 2014 My initials and signature indicate that I have read, understand and will comply with the requirements for the Nursing Assistant Class if I am SUCCESSFUL in registering for NUR 095. I also understand that if I do not meet the outlined requirements for class attendance that I will be administratively withdrawn from the class. _ After successful registration into Nursing 095, I will apply for a Criminal History Background with Verified Credentials, on or before the date indicated on the checklist for the term. This is the only criminal history accepted by COCC. I understand that I must be prepared to do the criminal history in the first 2 hours of class if I am accepted from the wait list. I understand that I must pass the criminal history check, as outlined in the Preapproval checklist, to maintain my seat in the class. I understand that the results of my criminal history are kept confidential and only reviewed by the Nursing Assistant Program Director and appropriate college Administrators. I will provide photocopies of documentation that I have met requirements for the following immunizations and titers on or before the indicated due date: Hepatitis B and/or titer MMR and/or titer Varicella Vaccine and/or titer Tdap 2 step TB test (PPD) and/or chest x-ray OR blood draw (IGRA) Influenza vaccine (fall or winter term students only) If I am on the wait list I understand that I must have all the listed immunizations and bring copies of documentation of immunizations, titers and x-rays to the Nursing Department Administrative Assistant on or before the scheduled deadline. I will obtain a current, valid: American Heart Association Healthcare Provider CPR/AED National Safety Council BLS for Healthcare and Professional Rescuers OR- American Red Cross AED/CPR for the Professional Rescuer card and I will provide a photocopy of both sides of the signed card to the Nursing Department Administrative Assistant in HCC 257 or 357 on or before the indicated due date. I understand that I must attend the mandatory orientation on the first day of class to retain my seat in the program or if I am waitlisted, to be considered for an open seat. I understand that the nursing department will not accept document originals and copies cannot be made in the department office. I understand that the classroom instructor will not accept either photocopies or originals of the required documents in the classroom. I understand that I will be administratively withdrawn from the course, and the open seat will be given to a waitlisted student, if I do not meet all pre-registration requirements for departmental approval and turn in photocopies of all required documents to the Nursing Department Administrative Assistant in HCC 257 or 357 on or before the indicated due dates.
