Prove, by diploma or transcript, conferral of the equivalent of at least an American bachelor s degree. 2. A $125 application fee is required of all applicants. The application fee will be collected at the time of your first registration. (Do Not Send the Application Fee in With Your Application). Students currently enrolled on the graduate level at another CUNY will have their application fee waived provided proof is shown at the time of application. 3. Permanent residents of the United States will be required to present their Alien Registration Card to the Admissions Office in order to be considered for New York State tuition rates 4. Students who present education credentials in a language other than English must also supply official translations of the documents. Deadlines for students with international documents will not be extended. 5. If the application and supporting material are acceptable, registration materials will be sent (usually two weeks prior to the start of registration) with further instructions. This material and usually an additional copy of the academic record must be presented by you to the departmental advisor to obtain course approval. 6. Please leave the completed and signed form with the required documentation in Room 203 (North Building), or mail to: Admissions/Non Matriculated, Hunter College, 695 Park Avenue, N.Y All required documents must be submitted along with the application. Applications will be returned if required documentation does not arrive with the application. Note: - The School of Education does not offer non-matriculated admission. - The School of Social Work admits eligible non-matriculants on a first come first served basis. The number of available spaces varies each semester. The program may close prior to the printed deadline. - Acceptance as a non-matriculant does not imply admissibility as a matriculated student in a future semester nor does it imply approval to take a specific course. - Students are advised to consult the current Graduate Catalog, and Schedule of Classes for specific information, procedures and regulations. The Schedule of Classes is usually not available until approximately two weeks prior to the start of each registration period. - Non-Matriculated students are not eligible for financial aid. - Non-Matriculated students are not eligible for student visas. - Ethnicity information is being collected to meet federal reporting requirements. It is confidential and will not be release except in the form of statistical summaries in which individuals are not identified. 5 Hunter College Immunization Requirements and Instructions: You must submit your: 1. Hunter College Immunization form (below). Only complete Part 1 and Part Immunization record showing 2 MMR vaccinations or a titer (If you have had a titer, you must also provide dated lab results for your titer.)ensure that your name appears on every page of your immunization record. This is especially important when submitting a lifetime immunization card. Submit to our Partnership Program Coordinator no later than August 1, 2014 to pp@hunter.cuny.edu. Failure to submit by August 1 may result in registration restrictions if you are accepted into the program. Forms that are not legible cannot be processed, and this will delay your registration. Your immunization record must show proof of the following: OR MEASLES (Rubeola) - Two doses of Live Measles Virus Vaccine at least 28 days apart: on or after 1st birthday and dated 1968 or later. MUMPS - One dose of Live Mumps Virus Vaccine: on or after 1st birthday and dated 1969 or later. RUBELLA (German Measles) - One dose of Live Rubella Virus Vaccine: on or after 1st birthday and dated 1969 or later. MMR Vaccination (Measles, Mumps, Rubella - 2 doses after 01/01/1972 Dose #1 must be given at age 12 months or later Dose #2 must be given after 15 months of age - OR at least 28 days after first vaccine OR Date healthcare provider diagnosed measles and/or mumps disease. Disease of rubella is NOT acceptable proof of immunity. There must be proof of vaccine or titers. Proof of these vaccines should be provided in one of the following ways: 1. Have your health care provider fill out and stamp the Hunter College Immunization Form (attached), 2. Provide a stamped copy of your health care provider's immunization form, 3. Provide a copy of your lifetime immunization card (childhood immunization card), or 4. Provide an official stamped copy of immunization records from a former high school, college or university. Additionally, all students must indicate that they have been notified of the Meningococcal Meningitis Public Health Law 2167 and have appropriate response on record with the college. This response is recorded in Part 2 of the Hunter Immunization Form submitted prior to the first semester. The Meningococcal Meningitis vaccine is not mandatory, we only require proof that you have been notified and given information about the vaccine. Failure to submit this form will delay your registration. Any foreign vaccine-related immunization records must be translated into English prior to submission to the Office of Health Services. Failure to do so may delay your registration.
6 We strongly advise against sending forms via as is not a secure form of communication and these forms contain personal information. Exemptions Medical Exemption - A note from your licensed physician/health care provider is required indicating the medical reason for contraindication. This statement must be written on the provider's stationary with stamp, license number, and signature. Religious Exemption - Submit a written statement (forms are available in the Office of Health Services) of sincere and genuine religious objection to immunizations.

8 Measles, Mumps and Rubella immunization requirements HUNTER COLLEGE Office of Health Services 695 Park Avenue 307 North Building New York, NY As of the Spring of 1990, Hunter College has required that all students comply with New York State s Public Health Law #2165. This law requires that college students born on or after January 1, 1957, present proof of immunizations or laboratory results indicating immunity against measles, mumps and rubella. Proof of age must be submitted for those students born prior to Meningococcal Disease Exemptions Documentation of medical contraindications, provided by a physician. As of August 15, 2003, Hunter College requires that all students comply with New York State Public Health Law (NYS PHL) #2167. This law requires institutions, including colleges and universities, to distribute information about meningococcal disease to all students meeting the enrollment criteria, whether they live on or off campus. Hunter College, CUNY, is required to maintain a record of the following for each student: A response to receipt of meningococcal disease and vaccine information signed by the student or student s parent or guardian. This must include information on the availability and cost of meningococcal meningitis vaccine (Menomune TM ). and either A record of meningococcal meningitis immunization within the past 10 years. or An acknowledgement of meningococcal disease risks and refusal of meningococcal meningitis immunization signed by the student or student s parent or guardian. Meningitis is rare. However, when it strikes, its flu-like symptoms make diagnosis difficult. If not treated early, meningitis can lead to swelling of the fluid surrounding the brain and spinal column, as well as severe and permanent disabilities, such as hearing loss, brain damage, seizures, limb amputation, and even death. Cases of meningitis among teens and young adults 15 to 24 years of age have more than doubled since The disease strikes about 3,000 Americans each year and claims about 3,000 lives. Between 100 and 125 meningitis cases occur on college campuses, and as many as 15 students will die from the disease. A vaccine is available that protects against four types of the bacteria that cause meningitis in the United States, types A, C, Y, and W-135. These types account for nearly two-thirds of meningitis cases among college students. Vaccines are available from a primary care physician or visit the CDC Travel Clinic web site for clinics offering the meningitis vaccine.
