2 Cass County Health, Human & Veterans Services Job Description JOB TITLE: Registered Nurse JOB CLASSIFICATIONS: Grade Level 30 DEPARTMENT: Public Health Division CURRENT EMPLOYEE: Vacant SUPERVISOR: Director of Public Health * Exempt * Bargaining Unit Eligible Purpose of Position Registered Nurses provide professional nursing care services with emphasis on service to patients requiring care related to public health programs, performs professional nursing in accordance with prescribed rules, regulations, professional practices, and special orders from physicians; may supervise licensed practical nurses and/or home health aides, and performs related work as assigned by a public health nurse. Work Performed/Job Functions A. Assure the quality and accessibility of health services i. Assessment of client s individual needs and level of service required through coordination of linking people to needed personal health service providers and assure the provision of health care when otherwise unavailable. ii. Skilled Nursing: Provides direct skilled nursing service to clients as recommended by physician. Nursing functions may include physical assessment of all ages, assessing vital signs and weights, checking fetal heart tones, mental/behavior assessments, medication management, diabetic management, disease management, screening of child growth and development, high risk pregnancy, use of therapeutic communication, and providing anticipatory guidance. Providing hands of technical skills required by a registered nurse including blood draws or other lab work, catheter care, ostomy care wound and dressing changes, infusion therapy, computation of doses, and injections. Obtaining and documenting physician orders and providing service according to physician plan of care for clients. Skilled nursing includes home health care in a Medicare Certified Agency that requires meeting State and Federal Requirements including the Outcome and Assessment Information Set (OASIS) requirements. Requires weekend coverage rotations for home care visits that may be necessary to meet client s needs due to their health condition iii. Correctional Health Nursing: Assessment of inmates medical care needs including providing health assessment as required for clients with chronic health needs, mantoux screening for tuberculosis; reviews, refills, and arranges for medication required within correctional facility; responsible for disposal and destruction of unused medications; communicate with and obtaining physician orders that affect inmates medical needs, arranging for medically necessary care outside of correctional facility. Training jailers in medication administration for certification and communicating medical needs of inmate during the stay in correctional facility. Transferring client medical care to other correctional facilitates when necessary. Responsible to follow correctional health policies and recommend changes and revisions as necessary. Advises jailers and Jail Administrator on medical needs of inmates. iv. Family Planning: Provide family planning services both in office and at clinic sites. Provide public information and outreach to targeted populations. Review client s health history and report significant findings to health care practitioner. Obtain vital signs, weight and lab work for pregnancy testing, hemoglobin and STI testing. Assist health practitioner during exam as needed. Provide counseling and referral and follow up as needed. Dispense and provide education on birth control method including action of, side effects and danger signs. Track clients for subsequent exams, follow ups and birth control pickup and/or administration. Follow
3 Family Planning grant requirements including documentation and charting procedures and general practice standards. v. Senior Health Clinics: Providing foot care for seniors, diabetic and anticoagulant therapy clients, medication consultation and education, blood sugar screening for seniors in community located clinics. Promote health of seniors by individual counseling. vi. Information, Outreach and Referral: Intake of phone calls in public health agency and while in community linking clients with other community resources including Head Start, Meals on Wheels, Lifeline, Help Me Grow, Family Centers, Faith in Action, and other community resources that may assisting in meeting their identified needs. B. Promote healthy communities and healthy behavior i. Teaching the client basic principles of healthy living applying information from related fields such as nutrition, dental hygiene and safety. ii. Promote healthy growth, development, aging and management of chronic diseases across lifespan. iii. Women, Infant and Children (WIC) Clinics: Compute financial eligibility, provide nutrition and breast feeding assessment and education, prescribe food, and referral to health services and social services as needed. Assure compliance with MN WIC policy and procedure as well as Federal Regulations utilizing HuBERT. Coordinate programs as needed. iv. Car Seats: Provide home visits to instruct parents on appropriate use of car safety seats for infants and children. v. Child & Teen Check up Clinics: Assessing children for potential or actual health problems by performing physical and developmental assessments. Interprets the screening results and makes appropriate referrals for diagnosis and treatment. vi. Early Childhood Screening: Works in collaboration with the school district to conduct health screenings for 3 4 year olds across the county. vii. Health promotion through information and education of community presentations. viii. Provides school health consultation services and contracted services as needed within school setting. ix. Health Promotion Grants: Variety of grants may require Registered Nurse to work toward grant requirements and towards action plans to fulfill the duty requirements such as areas of tobacco or obesity. x. Identify and address the needs of vulnerable populations. xi. Teaches the client basic principles of healthful living applying information from related fields such as nutrition, dental hygiene, safety, etc. Instructs caregivers to recognize early signs of disease and deviations from normal health patterns. C. Assure an adequate local public health infrastructure i. Many Registered Nurse responsibilities are mandated and regulated by regulatory agencies such as Department of Health & Human Services (DHS) and the Minnesota Department of Health (MDH). Carrying out these responsibilities must be done following the guidelines of the Minnesota Nurse Practice Act. ii. Enforce laws and regulations that protect health and ensure safety such as vulnerable child/adult protection reporting, clandestine drug lab ordinances, public health nuisance, and surveillance and disease investigation. iii. Provides input in program planning, the development of agency goals and the evaluation of the agency's programs and policies. iv. Collaborate with other entities including local, regional and state partners such as MDH, other PH departments, & public and private agencies. v. Facilitate experiences with students based on objectives developed by the school and student while serving as preceptor to nursing students. D. Prevent the spread of infectious disease i. Work with medical providers and other community partners to facilitate disease reporting and address problems with compliance. Provide Immunization Practice Improvement (IPI) visits to local clinics.
4 ii. Immunization Clinics: Assess immunization needs of clients; promote immunizations, and administer immunizations as indicated at community and office immunization clinics. Utilize Minnesota Immunization Information Connection (MIIC) to determine immunization needs and practice standards. iii. Assist in reporting communicable diseases and potential disease outbreaks. iv. Participate in surveillance and monitoring activity to detect patterns of unusual events and implement appropriate actions. Use data to develop strategies and plans to detect and respond to infectious disease problems and outbreaks within jurisdiction/region. v. Provide support to latent tuberculosis follow as needed including follow up screening for tuberculosis through mantoux testing. vi. When surveillance detects an imminent threat of infectious disease outbreak or epidemic, implement appropriate local disease control programs, including but not limited to mass treatment clinics, mass immunizations clinics, and isolation and quarantine. vii. Disease prevention and control maintain awareness of state epidemiological information through participate in conference calls, observe local trends. Respond as needed to prevent disease. E. Protect against environmental health hazards i. Conduct public health nuisance investigation in compliance with MN Statute 145A.04 public health nuisance requirements. ii. Provide information on environmental health hazards and monitor the community for significant and emerging environmental health threats, and develop strategies to address these threats. iii. Tobacco Compliance Check Program: Recruiting and training youth, coordinating the tobacco compliance checks site visits, working with the Auditor, Sheriff and County attorney to assure compliance with the ordinance, payment of fines, serving of suspensions and setting hearings as requested per Tobacco Ordinance. F. Prepare for and respond to disasters, and assist communities in recovery i. Conduct or participate in ongoing assessments to identify potential public health threats, local hazards, and the workforce capacity to respond. ii. Receive and evaluate Health Alert Network (HAN) information on a 24/7 basis from MDH. Health Alerts are utilized to exchange information during any emergency involving a public health response, including environmental threats, natural disasters, or acts of terrorism. Information is shared locally with community partners in response. iii. Provide leadership for public health preparedness activities in the community, by developing and maintaining relationships with partners at the local, regional, and state level. iv. Develop, exercise, and periodically review response plans for public health emergencies. Participate in periodic local preparedness drills. v. Participate in surveillance and monitoring activities, and implement appropriate response activities. vi. Provide current 24/7 contact information to public health department and be aware of roles and responsibilities during a public health emergency. vii. Work with Cass County Emergency Manager to define public health roles and response required by public health in emergency plans in event of disaster. G. General Responsibilities in all areas of nursing: i. Uncovers health problems through observation, interviews and analysis of records and initiates appropriate action. ii. Communicates/consults with the physician, nursing team, home health aides, social worker, and other involved caregivers to coordinate health care plans to meet client needs. iii. Maintains medical file documentation. Documentation is objective and factual, reflecting the nursing process: nursing assessments, diagnoses, interventions, plans of care, and evaluation of nursing interventions. Assure documentation, both written and computerized, meet legal, program, and agency requirements; complete appropriate agency and program forms in a timely manner, submit necessary forms to outside agencies as specified by program requirements. Complete all forms for billing purposes and agency management and operations in a timely fashion including dailies to report time work by activity. Follow data privacy practices (and HIPAA) as established by local, state and, federal agencies. Maintain records in good order so required documentation is available and
5 accessible. Maintain ongoing progress notes and communications with other health care providers and agencies as appropriate. iv. Community Organizations, Partners, Collaboration and Coalition Building: Represents the 1. Encourages community development by initiating or participating in action to address specific community needs. 2. Consult and serve in leadership roles on various committees or with community agencies such as clinics, community organizations, and schools as assigned. 3. Identify current services, gaps in services, and funding sources provided within the community for specific group focus. Partner with other community organizations on a systems level to coordinate services to meet identified needs. Develop written materials, educate, and problem solve with the community regarding services available and how to access those services. 4. Within the interagency framework, develop, evaluate and modify programs to meet identified community needs. 5. Build relationships and network with other agencies to advocate for target population, facilitate provision of cost effective coordinated services, and eliminate duplication of service thereby maximizing community resources. 6. Empower individuals, families, communities, or systems to mobilize their resources to become self sufficient in meeting their health and wellness goals. v. Registered Nurses are responsible to help cover staffing shortages and leaves by providing nursing services as needed for all areas of public health noted above. vi. All other duties that are assigned by management. Performs professional duties and responsibilities in accordance with PELRA (Public Employers Labor Relations Association) requirements. Minimum Training, Experience or Certification At a minimum should have a high school degree or its equivalent, plus specialized knowledge gained through a Diploma or Associate Degree in nursing (no related field allowed) from an accredited college and successfully pass the State Board of Examinations for Registered Nurse license. Additionally, must maintain current Registered Nurse License in Minnesota and CPR Certification. Essential duties require current, valid driver s license. At time of hire, preference for at least one year of public health nursing experience or home care.
