General Summary of Function/Purpose of Position This is a senior Registered Professional Nurse (RN12) position which requires advanced clinical decision making skills and judgment. The RN12 is the first and sometimes only health care professional to access and/or treat correctional residents. It is the RN 12 who determines the need for and timeliness of an appropriate referral to the physician. S/He is the only health care professional expected to independently perform the full range of duties inside of the correctional setting. In any and all settings, the RN 12 is expected to initiate and sustain life-saving care and measures as appropriate, as well as perform the full range of clinical nursing duties. The complexity of the work exceeds written, established methods and procedures often available and, therefore, the RN12 is expected to exercise considerable independent judgment in selecting the most appropriate course of action. The RN12 works as an emergency clinic nurse and as an ambulatory clinic nurse. This position is located 100% within the secure perimeter of a correctional facility and is a drug and alcohol test designated position. Page 1

2 15. Please describe the assigned duties, percent of time spent performing each duty, and what is done to complete each duty. Duty 1 List the duties from most important to least important. The total percentage of all duties performed must equal 100 percent. General Summary of Duty 1 % of Time 30 The RN12, as the senior level Registered Nurse, is always the frontline intervener in any medical or psychiatric emergency. S/He provides life-sustaining, life stabilizing emergency medical or psychiatric treatment in the event of a patient crisis or medical emergency. S/He uses emergency standing orders to use emergency prescription medication and treatment in life-threatening situations. Makes appropriate and timely referrals or recommendations for patient care to a physician based on professional judgment and knowledge of scientific rationale. Advanced clinical judgment skills are utilized in patient care situations in which the patient status or outcome is unpredictable. Psychiatric emergencies can include individuals expressing lethal suicidal behaviors and/or those experiencing acute psychotic episodes. Makes recommendations for suicide prevention and/or physical restraint when necessary and recommends offsite health care consultations and/or transport. The senior level Registered Nurse is the only health care provider who performs work outside the clinical setting, ie. In the housing unit, segregation unit, protective environment units, etc. Performs triage of prisoner complaints and physical assessment or prisoners as indicated. Provides immediate first aid, CPR, or other nursing interventions as indicated. Administers IV fluids, O2 as indicated. Performs testing as indicated (EKG, PO2 s,etc.) Performs venipunctures for lab, IV s as indicated. Determines disposition of prisoner based on assessment/evaluation of condition. Initiates transfer to another facility as needed for appropriate treatment. Provides for continuity of care by notifying appropriate providers. Maintains confidentiality of health information as required by law. Recommends suicide prevention interventions as indicated. Maintains universal precautions in the performance of duties. Duty 2 General Summary of Duty 2 % of Time 30 The senior level Registered Nurse is responsible for the total health care delivery and establishment of a therapeutic relationship with acutely or chronically ill medical, surgical and psychiatric patients. Makes appropriate and timely referrals and recommendations for patient care to the physician, psychiatrist or psychologist, bases on professional judgment and knowledge of scientific principles. Provides ongoing individualized group and individual patient education. Shares and receives information from other health care providers. Consults with other health care providers or staff as necessary to meet health care needed of prisoners. Refers prisoners to other health care providers as needed. Provides routine care for annual health screening, and chronic care clinics. Initiates appointments for prisoners as indicated by their complaints/conditions. Utilizes existing policy, procedures, protocols, or guidelines for provision of nursing care. Provides education to prisoners regarding health promotions, health interventions and illness prevention. 3 Duty 3 General Summary of Duty 3 % of Time 25 The senior level Registered Nurse documents in the health record in a professional, legal manner. Completes a variety of documents and forms used in the clinical complex. Completes necessary documents and reports in a professional manner. Conducts or attends in-service training programs and maintains professional knowledge base. Leads or attends staff meetings and committee meetings as assigned. Makes recommendations for changes in the clinic. As assigned, acts as a lead worker and clinical resource to other RN s when necessary. Performs related duties as assigned. 1. Completes accurate documentation of encounters in the electronic medical health record and appropriate logs. 2. Maintains confidentiality of health information. 3. Provides accurate information to other health care providers as staff to maintain continuity of care.\ 4. Attends and participates in in-service training as assigned. 5. Attends and participates in meetings or serves on committees as assigned. 6. Maintains continuing education units for maintenance of individual licensure. 7. Participates in continuous quality improvement activities in the workplace. Duty 4 General Summary of Duty 4 % of Time 10 Draws blood, prepares and obtains laboratory specimens. Performs routine lab tests, ie., hemoccult studies, and dipstick urinalysis. Starts and maintains IV lines, steri-strips lacerations, perform visual acuity exams, dressing changes, venipuncture, EKG, intradermal TB tests, etc. inventories supplies, recommends ordering of supplies and equipment. Participates in all institutional security procedures. Performs venipunctures to obtain lab specimens or for IV line insertion. Obtains lab specimens as indicated, prepares for transport to the laboratory. Performs visual acuity exams. Assesses lacerations for severity and applies steri-strips as indicated. Performs dressing changes using accepted wound care protocol. Conducts testing as appropriate (EKG, PO2, TB Mantoux testing, etc.) Maintains inventory of supplies, performs equipment checks. Makes referrals for equipment purchases, replacement, or repair. Page 3

4 Duty 5 General Summary of Duty 5 % of Time 5 Safety and Security Complies with all policies and procedures including safety and security. Participates in all institutional security procedure and practices. Receives report at the start of an assigned shift from Registered Nurse departing an assigned shift and gives report to the on-coming RN for the next shift. Complete written and verbal reports as required, including participation in reporting of Critical Incident using MDOC Critical Incident Witness Report Form (CAJ-571) and submit. Checks the Emergency and Critical Tool list at the start of and departure from assigned shift and reports missing tools to the Shift Commander, Health Unit Manager, and Supervisor. Hazards associated with work inside of a correctional facility. Considerable bending and lifting of heavy objects and patients is a normal part of this position. Work involves shift, weekend and holiday assignments. Potential for exposure to a communicable disease. Contact with convicted felons with 100% of time being spent within the secure perimeter of a Correctional Facility. The incumbent has regular unsupervised access to and direct contact with prisoners. This is a drug and alcohol randomly-tested position. 19. 7 26. What are the minimum education and experience qualifications needed to perform the essential functions of this position? EDUCATION: Graduate of an approved school of nursing. EXPERIENCE: One year of professional nursing experience. One year of corrections health care experience preferred. KNOWLEDGE, SKILLS, AND ABILITIES: Emotional stability and good interpersonal skills. Increased knowledge and skills in the application of professional nursing techniques. Strong verbal and writing skills. Advanced physical assessment skills. Ability to work independently and as described in Civil Service Class Series. CERTIFICATES, LICENSES, REGISTRATIONS: Licensed as a Registered Nurse in the State of Michigan.
