1 PARK COUNTY POSITION DESCRIPTION I. POSITION IDENTIFICATION Work Unit: Health Supervisor: Nursing Director Current Classification: Pay Grade: 16 Salary Range $ $22.00 Title: Public Health Outreach Education Non-Exempt Specialist FULL time II. ASSIGNED DUTIES AND TASKS Position overview: PUBLIC HEALTH HOME VISITING AND EDUCATION PROGRAM: Position is responsible for fulfilling the Public Health Mission to high needs individuals, pregnant women, their infants and children in Park County. The case manager is responsible for overseeing the Public Health Home Visiting program. The case manager works cooperatively with Parents as Teachers personnel, nursing director, local physicians and multiple local agencies, to ensure the program goals and objectives are met. The outreach manager, along with the nursing director, is responsible to see that contract agreements are carried out and quality assurance audits are performed. IMMUNIZATION PROGRAM: Position is responsible to perform duties stated in DPHHS Immunization contract: maintaining Immunization Registry record input; sending reminders for immunizations due, performing assessments of vaccination coverage for Park County s 2 year old population and other duties as contract states. PHEP: As a part of the Public Health Emergency Preparedness team, this position will provide education to families regarding emergency preparedness. FETAL INFANT CHILD MATERNAL MORTALITY REVIEW COORDINATOR: This position is responsible to assure all fetal, infant, child and maternal deaths are reviewed by the FICMMR committee and preventability issues addressed. TOBACCO PREVENTION SPECIALIST: For 50% of this position, the Outreach Specialist will serve as The Tobacco Use Prevention Specialist (TUPS) for Park County and is responsible for the planning, implementation, and evaluation of Park County Tobacco Use Prevention Program activities to meet the goals and objectives of the Annual Work Plan in coordination with the Montana Tobacco Use Prevention Program (MTUPP) grant. All duties below are essential functions unless otherwise indicated. Duties of the position are not all inclusive and subject to change. A. PUBLIC HEALTH HOME VISITING PROGRAM 1. To maintain a mechanism for identifying high-risk individuals through referrals from Parents as Teachers, Livingston Memorial Hospital, Community Health Partners, WIC, local physicians and programs serving high risk families; serves as a liaison to these agencies. 2. Following receipt of referral, to screen to determine urgency, scope and action required, responds to referral within five days, and opens referrals to service when appropriate, refers client to other agencies as appropriate. 1

To provide core maternal and child health services through public health home visiting to pregnant women, non-pregnant women of child bearing age, infants younger than one year of age, children and adolescents under age 22, or children with special health care needs. PHHV program will utilize Parents As Teachers as the evidenced based program model. 4. To perform comprehensive health assessment at initial visit using forms designated in PHHV contract and conduct a reassessment at appropriate intervals. 5. To make referrals to appropriate community resources and follows up to see that client has established care with a primary care provider, received services, and assists the client with application process or acquiring additional specialized service when needed. 6. To provide at least 75% of the home visits in the client s home, with no more than 25% of visits conducted outside the home; if home environment deemed unsafe, provide visits in office or alternate location, documenting reason for out of home visit. 7. To maintain written or phone contact with health care providers and other agencies who also have involvement with the client. 8. To prepare a written consultation and places information on chart within one week of visit, assessment or re-assessment. 9. To provide health education as needed. To collect, develop, compile and distribute resource materials; teaches information to client. 10. To complete accurate and timely charting and record keeping on client s chart. Maintains record of all contacts for annual MCH report. Submits all data to granting organizations, DPHHS and Nursing Director as directed in contract. 11. Maintains strict confidentiality per HIPAA guidelines at all times. 12. Attends mandatory county in-services and trainings/meetings as directed by MCH Block Grant contract. 13. To be available by phone to clients as much as possible during regular business hours. B. IMMUNIZATION PROGRAM ACTITIVITIES 1. To use the immunization data from the IMTRAX registry evaluate records for missing vaccinations and send out reminder cards to children involved in the PHHVP. 2. To perform audit of month old children s vaccination records to assess vaccination coverage in Park County. 3. To provide education and outreach to improve adolescent vaccination rates and assess outcomes. C. DISASTER AND EMERGENCY PREPAREDNESS ACTIVITIES 1. To complete FEMA IS-700, ICS-100 online courses within 6 months of employment and ICS-200, ICS-300 course within one year of employment. 2. To enroll online in DPHHS TCC and logs trainings into established account. 3. To participate in trainings and exercises as requested by supervisor. 2

3 D. FETAL INFANT CHILD MATERNAL MORTALITY REVIEW 1. To ensure that all fetal, infant, child, and maternal deaths are reviewed by the FICMMR team within one year of receipt of death certificate. Completes necessary forms and submits to DPHHS MCH department, FICMMR coordinator. 2. To schedules FICMMR meetings, ensures confidentiality of information, maintains team member roster, and obtains all necessary case information from hospital, provider, etc. to present case. E. TOBACCO PREVENTION SPECIALIST 1. To work collaboratively with diverse local organizations and community members to promote tobacco free lifestyles and policies to achieve the goals of decreasing tobacco use, nicotine addiction, and secondhand smoke exposure in Park County. 2. To fulfill all the grant obligations and adheres to the fiscal, evaluation, and record keeping requirements of the Montana Tobacco Use Prevention Program (MTUPP) grant. 3. To develop and implement the annual Park County Tobacco Use Prevention Program Work Plan objectives and activities. 4. To communicate regularly with the MTUPP Contract Liaison, provides up-to-date reporting, attends required meetings, and submits invoices as directed. 5. To maintain high level of knowledge about tobacco issues and acts as a resource for healthcare providers and other community members and organizations. 6. To develop and maintain collaborative relationships with diverse community organizations, businesses, and individuals to accomplish program objectives. Organizes and facilitates regular meetings with community partners. 7. To increase public awareness and support of tobacco use prevention and cessation through the creation and distribution of newsletters; submittal of press releases and ads to local media outlets; and, conducting presentations to professional groups, decision makers, and the public. 8. To educate local businesses and organizations about secondhand smoke and the Montana Clean Indoor Air Act to enhance compliance. 9. To assist local organizations, agencies, and businesses with the development and implementation of policies that support tobacco use prevention. 10. To work with local schools and/or youth groups to engage students in youth advocacy activities in the schools and community. 11. To promote tobacco cessation and the services of the Montana Tobacco Quit Line. 12. To serve as role model for tobacco free lifestyle. F. OTHER DUTIES AS ASSIGNED Performs other duties as assigned by Nursing Director. This includes special projects and a variety of other functions as needed. III. KNOWLEDGE This work requires the knowledge of educational techniques, motivational interviewing, the ability to communicate effectively with people from all educational, cultural and economic backgrounds, knowledge of health and family assessment, knowledge of local resources, and coordination of services. Defensive driving in all road conditions. Must be familiar with basic medical terminology, and fundamental medical and legal guidelines. Knowledge of addiction and recovery as it relates to tobacco and other addictive substances. Proficient ability to facilitate meetings and conduct presentations to professional groups and the public. Must be able to operate a computer, have skills in word processing, data entry and specialized programs as necessary. Must be able to 3

4 operate fax, copier and cell phone. Ability to work a flexible schedule (infrequent evening and weekend work and the ability to travel overnight on occasion). Education and Experience Minimum of a Bachelor of Science or Arts Degree in Social Work, Human Resources, Education or Nursing, preferably with a Public Health emphasis. Preference for Master of Social Work. One year experience in public health. Experience working with youth and high need population is preferred. Certificates, Licenses, Registrations Current Montana Driver s License IV. ACCOUNTABILITY This position is accountable for providing public health education and outreach services through public health home visiting to high risk individuals, pregnant women, non-pregnant women of child bearing age, infants younger than one year of age, children and adolescents under age 22yrs, or children with special health care needs. Comply with the requirements of Title V: Maternal and Child Health Services Block Grant, Section 501 to 510 [42 U.S.C.701 to 701]. Comply with the requirements of ARM governing the Maternal and Child Block Grant. Immunization activities are directed per DPHHS Task Order to Park County Unified Government Master Contract that covers the period of JULY 1, 2012 through JUNE 30, 2019 IMMUNIZATION PROGRAM. Comply with requirements of Montana Tobacco Use Prevention Program (MUTPP) grant. V. CONFIDENTIALLY The position requires handling non-public confidential information. The person in the position acknowledges the confidential nature of non-public information regarding PHHV and immunization clients and FICMMR information. Consistent with applicable policy and guidelines, this position will respect and safeguard the privacy of PHHV, immunization clients and the confidential nature of information in accordance with Montana state law, without limiting the general nature of this commitment. The person in the position hereby acknowledges that the person understands that in this context, confidential information is considered all non-public information that can be personally associated with an individual. If in the course of executing job responsibilities, the person in the position accidentally access information that others might consider inappropriate for this position to access, the person in the position will notify the person s supervisors of the date and time of the access. INDEPENDENCE OF ACTION This position plans and carries out most assignments independently, and uses judgment and initiative to solve problems. Work objectives and priorities are defined by PHHV, MCH, Immunization contract services, MUTPP and Montana Department Public Health and Human Services. 4

PERSONAL CONTACTS This position has contacts with the public, county personnel, elected officials, department heads, community services representatives and clients in order to be responsive to questions and inquiries regarding the Public Health Home Visiting service, Immunization Services, Tobacco Prevention Program and the Fetal, Infant, Child, Maternal Mortality Review Team. VIII. WORK ENVIRONMENT/PHYSICAL DEMANDS The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Home visiting in the client home is the preferred venue of service provision. If the employee deems the home to be unsafe, the client visits will be scheduled at an alternate location; in the office of a provider, the Health Department or the hospital. 5
