1 4102 N. Mall Ave. Fayetteville Arkansas, May 2013 Program hours This is a 12 week course that includes an in office internship during the course of study. There will be two eve classes per week. Times will be 5:30pm-8:30 pm for 12 weeks which will be 72 hrs class room and lab work combined in a working dental office setting. You will have an opportunity to do an internship in the dental office during working hours on a rotation basis of 48 hrs starting in the 5th week of study. We will also have a saturday session for extra or makeup hours. Sessions will be held during the summer,fall,spring,winter depending on the number of applicants. We will accept between 3 and 12 students per session. You can call the school for the next available class. All federal holidays will be observed. Enrollment Requirements Students must have a High School Diploma or GED and be at least 18 years of age. Due to the limited number of students per class, and the quality of the program,the classes fill fast and we may not be able to accept all applications the first time. There is a non refundable registration fee of 50 dollars. Program cost Cost of the program is 2550 which will include tuition, lab cost, material fees, study materials and the cost of preparing you for the state radiology and polishing certificate tests. This is an extremely competitive price considering the amount of materials, equipment and supplies available to you in the actual dental office you will be training in. We will help you arrange for CPR training which is a separate fee.
2 Attendance Students must complete all hours stated in the program hours section of this handbook. If more then two classes are missed the student will be placed on probation. If during the probation period the student doesnʼt make up the missed work and or misses more time, the student will be dropped. Tardiness is not acceptable and will be subject to probation after 3 tardies. After 6 tardies the student will be terminated. Tardiness is more then 10 minutes late. Due to the shortness of the program leave of absence will not be granted. Any student requiring a leave of absence will be offered the opportunity to enroll for the next scheduled class. Academic Students will receive a pass/ fail grade for the course. Credit will not be given for previous education,training or work experience. All students will be required to make an 80% average on chapter quizzes, midterm, and final test with a satisfactory clinical participation to be able to receive a certificate of completion from the school. If you fall below these guidelines you will be notified and put on probation until your progress improves to satisfactory levels or dismissal. Conduct Any student under the influence of alcohol or drugs will not be permitted on school premises. Unbecoming conduct,acceptable to professional standards, will be subject to dismissal. Stealing, profanity and disruptive behavior will not be tolerated. Students will wear scrubs and tennis shoes for classes starting the second week of class. Long hair will be tied back and no large earrings or perfume allowed. You will work as a team and treat all students the way you would want to be treated. An incomplete grade will be recorded upon with-drawl or termination. Any one who feels the institution has not complied with their rights under the law may file a complaint with the state board of career education at if the matter is not settled with in the school.
3 Refund policy The request for refund or termination refund is computed on the the last date of actual attendance by the student. The school must refund all money paid if the applicant is not accepted. This includes instances where starting class is canceled by the school. The refund shall be based on the cost and length of the program All tuition beyond the current enrollment period shall be refunded when a student terminates. Registration fee of 50 dollars will be retained by the school, plus tuition and fees as outlined in the following schedule. Refunds will be returned within a 30 day period. At completion of less then 25% of the program the refunds shall be made on a pro rata basis." At completion of 25% but less then 50% of the program the student shall be refunded not less than 50% of the tuition. At completion of 50% but less than 75% of the program the student shall be refunded not less than 25% of the tuition. At completion of 75% or more of the program no refund is due the student.

Pain and anxiety control. 3hr lecture 3 hr lab ch 12,13,14 week 5 Dental Radiology, radiation safety. 2hr lecture 4hr lab ch 15,16. week 6 MID TERM EXAM preventive care, instruments and handpieces. 2hr lecture 4hr lab. ch 17,19. week 7 Impressions and diagnostic casts, restorative material and procedures, lab procedures. 2hr lecture 4 hr lab. ch 20,21,22. week 8 Prosthodontics, oral and maxillofacial surgery, making provisional crowns. 2hr lecture 4 hr lab ch 23,26. week 9 Endodontics, pediatrics, periodontics,orthodontics. 4 hr lecture 2hr lab. ch 24,25,26,27. week 10 Radiology and polishing exam. Job search 3 lecture 3 lab ch 29. week 11 Review for final ch lecture 3 lab. week 12 Final written and Practical exam, Grad Certificate 3 lab 3 lecture
