3 1. Introduction The Institute of Therapeutic Massage of Western Colorado (hereinafter referred to as ITM) was founded by Marilyn S. Veselack, Grand Junction, Colorado. ITM is committed to teaching massage therapy training and offers the finest environment for student training with a experienced instructor. The curriculum includes the use of a study guide, in class training, hands-on practical training and examinations that cover the basic areas of information that are needed for Colorado Licensure in massage therapy. The flexibility of the teaching program allows the teacher to pass on valuable concepts and share learning experiences with students. ITM is owned by Marilyn S. Veselack, Grand Junction, Colorado. Veselack, MS Ed (School Administrator); Business Manager Priscilla Walker, President, Walker Innovations; Business and Marketing Eligibility: A student seeking admission to ITM s program must be at least 16 years of age, have earned a high school diploma or a GED, be in good health, and be able to pass the state of Colorado s required fingerprinting and background check required to become a licensed massage therapist in Colorado. Student Complaints: If a student has an issue/grievance with the school, the procedure for the student to follow should be to present the complaint, in writing, to the Director of the School to resolve the issue at the local level. Attempting to resolve any issue with the School first is strongly encouraged. 4 Curriculum 2. The 500 and 750 hour Massage Therapist Program curriculums consists of three main components: Anatomy and Physiology covers the study of basic organized systems of the human body: skin, skeletal, muscular, circulation, endocrine, respiratory, digestive, excretory, and reproductive systems. Massage Practical and Theory encompasses a practical understanding of the mechanics of the body, and basic massage strokes. This includes instruction about different massage strokes: including special techniques, joint movements and hydrotherapy. Business Practices are designed to share basic knowledge with the student about the practical information needed to operate a massage practice. This section contains information from the realms of history of massage and ethics, equipment, record-keeping and marketing. This component will prepare the student for starting a practice upon completion of ITM. Tuition and Payments Tuition for the 750 Hour Massage Therapy Course is $9,000. The tuition for the 500 Hour Massage Therapy Course is $6,000. ITM s payment plan is to pay per Unit at that Unit s testing. No monies are returned for tests taken and no tests are given on credit. Each student will be responsible for supplying their own textbooks, note taking equipment, massage table, sheets, towels, slacks (no jeans) and shoes (no sandals or open toe shoes allowed during clinical). 6 TOTAL HOURS: 750 for 750 Hour Program or 500 Hours for 500 Hour Program 4. ITM records graded exams and monitors class work and practicals (methods of performing massage strokes) to make certain that requirements are met. A Certificate of Completion is issued after a student successfully completes the training program. A student is expected to take and pass a CPR class and a Basic First Aid class. Proof of successful completion of these courses is required prior to a student receiving a certificate of completion from ITM. Program Cost by Unit The 750 hour massage therapy program consists of 36* (thirty six) units of study and the cost is $250 per each unit. The 500 hour program consists of 38 units and the cost is $158 per unit. Tests will be issued to the student as payment is credited to their account. No unit tests will be given to a student on credit. * The 750 hour curriculum combines some of the 39 chapters which results in 36 units. Class Schedules Class time may consist of weekly, evening and/or weekend sessions of various lengths a month. The dates and times will be discussed with prospective students and an attempt will be made to fit class time around individual work schedules. When an unexpected closure occurs due to extraordinary conditions such as inclement weather, students will be notified as soon as possible by phone and/or radio, TV, who provide closure information as a public service. Classes are not held on the following holidays: New Years Eve New Years Day Memorial Day Independence Day Labor Day Thanksgiving Day Christmas Eve Christmas Day The School does not discriminate based on race, sex, religion, ethnic origin, or disability. Prospective students must have a high school diploma or equivalency diploma to be accepted for enrollment. If applicants are beyond the age of compulsory school attendance in Colorado and do not possess a diploma or equivalency diploma, they may complete an ability to benefit test. The School does not administer the test, but will provide information on availability when requested.

7 Enrollment 5. Prospective students may enroll anytime. The competency designed curriculum allows flexibility for individual scheduling of course work and testing. ITM s massage therapy program is divided between time-based and lesson based programs Students are expected to complete the 750 hour program within nine to eighteen months of making their first payment (start date); while 500 hour program should be completed within six to twelve months from their start date. An extension of time may be applied for in writing to the Director of Education. Postponement of Starting Date Postponement of the starting date, whether at the request of the school or the student, requires a written agreement signed by the student and the school. The agreement must set forth: (a) whether the postponement is for the convenience of the school or student; and: (b) the deadline for the new start date, beyond which the start date will not be postponed. If the course is not commenced, or the student fails to attend by the new start date set forth in the agreement, the student will be entitled to an appropriate refund of prepaid tuition and fees within 30 days of the deadline in accordance with the school s refund policy and all applicable laws and Rules concerning the Private Occupational Education Act of Placement Assistance The Institute of Therapeutic Massage offers employment assistance to graduates, consisting of posting job information and job including skills development in the curriculum. While assisting students in the job search, ITM makes no guarantee, expressed or implied, of future employment. Current Colorado law prohibits any school from guaranteeing job placement as an inducement to enroll students. Attendance Requirements Students are expected to arrive for hands-on classes on time with proper materials. All missed classes must be made up at the convenience of the instructor. The instructor may request your withdrawal from a course or program if excessive absences or tardiness leads to unsatisfactory progress. Students who are unable to continue classes for medical reasons or severe personal problems will be required to take a leave of absence until they are able to return to class. Proper documentation will be required to substantiate a student s withdrawal.

8 Progress Policy 6. Students must maintain an 85% grade point average. Those who do not maintain the 85% grade point average will be placed on probation for 30 days. During the probation period students must raise their grade average to a passing percentage or higher. Grades are discussed upon the return of the graded tests. Grading System Excellent Above Average Average Below Average Under 66 Unsatisfactory Periodic progress reports, with grades (numerical averages), will be discussed with students upon return of their graded tests. Refund Policy The school program is time based and students are expected to complete it within a 9 to 18 month period of time for the 750 Hour Program and a 6 to 12 month period of time for the 500 Hour Program. These time periods may be expanded when a student submits a written petition to the School Director requesting deviation from ITM s policy. The School Director s decision must be signed and dated by both the School Director and the student and placed in the student s permanent file. Students not accepted to the School are entitled to all money paid. Students who cancel this contract by notifying the School within three (3) business days are entitled to a full refund of all tuition and fees paid. Students who withdraw after three (3) business days, but before their beginning their program, are entitled to a full refund of all tuition and/or fees paid except the maximum cancellation charge of $ or 25% of the contract price which ever is less. In the case of students withdrawing, after commencement of classes or starting the program, the School will retain a cancellation charge plus a percentage of tuition and fees, which is based on the percentage of time in the program as described in the table below. The refund is based on the span of time in the program after receiving their curriculum guide.

The student may cancel this contract at any time prior to midnight of the third business day after signing this contract. 2. All refunds will be made within 30 days from the date of termination. The official date of termination or withdrawal of a student shall be determined in the following manner: a. The date on which the School receives written notice of the student s intention to discontinue the training program; or b. The date on which the student violates published school policy, which provides for termination. c. Should a student fail to return from an excused leave of absence, the effective date of termination for a student on an extended leave of absence or a leave of absence is the earlier of the date the School determines the student is not returning or the day following the expected date of return. 3 The student will receive a full refund of tuition and fees paid if the school discontinues a Program/Stand Alone course within a period of time a student could have reasonably completed it, except that this provision shall not apply in the event the school ceases operation. 4. The policy for granting credit for previous training shall not impact the refund policy. The above regulations do not apply to students that pay by the unit. The school s flexible program allows the students to make payments for the unit test that they are taking and there are no monies returned. No tests are given on credit.

Veselack BM Ed; MS; DMA; LMT; BCTMB; ABWA Student Complaints/Grievance Procedure If a student has an issue/grievance with the school, the procedure for the student to follow should be to turn in a written complaint to the Director of the School to resolve the issue at the local level. Attempting to resolve any issue with the School first is strongly encouraged. Student Complaints may be brought to the attention of the Division of Private Occupational Schools online at There is a two-year statute of limitations for the Division to take action on a student complaint (from student s last day of attendance). Conduct Policy All students are expected to act ethically and maturely. ITM students are required to display respect to faculty members and students. Possession of weapons, illegal drugs, and alcohol of any kind are not allowed at any time on school property. An arrest by an officer of the law for unlawful possession of weapons, illegal drugs and/or alcohol is cause for dismissal from The Institute of Therapeutic Massage of Western Colorado. The arrest is a violation of school policies and will result in a student s permanent dismissal from School (ITMWC). Dismissal Any student may be dismissed for violation of rules and regulations of the School, as set forth in school publication. A student also may be withdrawn from classes if he or she does not prepare sufficiently, neglects assignments, or makes unsatisfactory progress. The School Director, after consultation with all parties involved, makes the final decision. The School Director of ITM may temporarily suspend a student whose conduct is disruptive or unacceptable to the academic setting. After appropriate counseling, students who demonstrate a genuine desire to learn and conform to School standards of conduct may be allowed to resume attendance. The School Director will review each case and decide upon re-admittance.

11 Facilities 9. The Institute of Therapeutic Massage of Western Colorado is located at 2352 North 7 th Street, Unit A, Grand Junction, Colorado, The educational programs are facilitated with massage equipment, computer programs, charts, diagrams, DVDs, and videos to enhance classroom experiences. Our library along with the library at local hospitals in Grand Junction assists students with developmental study and career information. Educational Services Courses and the overview of the components of the program are listed under curriculum on pages 2-3. ITM targets its education process towards preparing qualified massage therapists for working with muscular problems and muscular related problems of the human body. Mastering the unit modules aids the student in learning needed aspects of anatomy and physiology, various practical massage skills and basic business and marketing skills. The education process is a combination of in-class lectures and workshops along with instructor guided hands on practicals. Research insights as well as opportunities to acquire massage therapy experience. The fee for student make-up hours required for completion of the program is to be no less than $10 an hour or more than $35 an hour. The dollar amount of make-up fee will depend on the nature of and educational content of classes for which the student was absent. Previous Credits Credits from another institution may not be accepted. A student may test out of ITM Unit Tests in order to receive credit for previously learned information. The credits will count towards completion of ITMWC s massage therapy program. There is a $50 fee for the process of testing out of a Unit whether the test is failed or passed. Transfer of Credits ITM does not guarantee the transferability of its credits to any other institution unless there is a written agreement between the two institutions. May 7, 2015
