1 Internal Medicine Residency Faculty Allan L Markus, MD, MS, MBA, FACP Program Director Dr. Markus came to HonorHealth with the goal of building a state-of-the-art Internal Medicine Residency Program from the ground floor. He previously worked as a chief resident at Baystate Medical Center, Springfield, Mass.; associate and interim program director at Berkshire Medical Center, Pittsfield, Mass.; and associate program director at St. Joseph s Hospital and Medical Center. He also served as director of Arizona State University Health Services and has teaching appointments at University of Arizona College of Medicine-Phoenix, Mayo College of Medicine, and Kirksville College of Medicine, Kirksville, Mo. He has served as the editor of the Chief Resident Manual of the Association of Program Directors in Internal Medicine and has been active in the Arizona Chapter of the American College of Physicians. Along with his two children, he enjoys working with the Flying Samaritans Phoenix Chapter, providing care for patients in the Baja de California. CORE FACULTY Jared Berkowitz, MD, FAAP Dr. Berkowitz completed his undergraduate and medical school training at Tulane University in New Orleans, La. He completed a residency in internal medicine and pediatrics at Banner Good Samaritan Medical Center and is board certified in both internal medicine and pediatrics. He s had a passion for teaching and staying involved in academic medicine since completing his residency program. Dr. Berkowitz is involved with teaching physical exam skills to first- and second-year medical students at the University of Arizona School of Medicine-Phoenix and is mentoring several residents in his continuity clinic. His interests include cooking, hiking, going to the movies as often as possible and spending as much time as he can with his wife, son and daughter.

2 Patrick Gaylor, DO Dr. Gaylor hails from Abilene Texas. He did his medical school training at The University of North Texas Health Sciences Center/Texas College of Osteopathic Medicine, Denton, Texas, and his residency training at Banner Good Samaritan Medical Center in Phoenix where he also served as a chief resident. He now serves as a preceptor for the University of Arizona College of Medicine-Phoenix thirdyear students rotating at HonorHealth. Dr. Gaylor s professional goals are maintaining the relevance of the hospitalist model within the healthcare delivery system as it evolves. He enjoys spending time with his wife, Allison, a certified nurse practitioner, and his son and daughter. He loves outdoor adventures like backpacking and fly fishing. Mark Lewis, MD Dr. Lewis is passionate about providing high-quality medical care for his patients and excellence in medical education for our future physicians. His philosophy on medical education for students and residents starts with the basics from evidence-based medicine to best practice and includes learning how to maintain the love and excitement for the profession. Because he also believes that health has a strong basis in our foods and nutrition, he battles the summer heat while maintaining a flock of backyard chickens and micro-farm of grapes, squash and tomatoes. 3 SUBSPECIALTY FACULTY Cardiology Daniel Storch, MD Dr. Daniel Storch has been practicing cardiology in Scottsdale for over 20 years. He received his undergraduate degree from Union College, Schenectady, N.Y., and his medical degree from the New York University School of Medicine in After completing his internal medicine internship and residency at the Bronx Municipal Hospital Center/Albert Einstein College of Medicine in New York, he stayed on for fellowships in adult cardiology and then interventional cardiology. Dr. Storch is triple board certified in internal medicine, cardiovascular diseases and nuclear cardiology. He is a designated Fellow of the American College of Cardiology. He serves as Az Cardiology Group s Nuclear Stress Lab Director and is chair of internal medicine at HonorHealth Scottsdale Thompson Peak Medical Center. His areas of clinical interest include coronary artery disease, angioplasty and stenting, congestive heart failure, lipid management and preventive cardiology. He has been recognized as one of Phoenix Magazine's TOP DOCs. David Rizik, MD Wazhma Aslamy, MD Greg Sanders, MD Adam Sabbath, MD Christina S. Reuss, MD Gastroenterology Gavin Levinthal, MD Dr. Levinthal is a staff gastroenterologist at the HonorHealth Scottsdale Thompson Peak Medical Center campus. He is board certified in gastroenterology, hepatology and internal medicine. A native of South Africa, Dr. Levinthal received his medical degree from the University of The Witwatersrand in Johannesburg. He completed his internship and residency in South Africa before coming to the U.S. as an internal medicine resident at Mt. Sinai Medical Center in Cleveland, Ohio. Later he became chief resident in internal medicine and was a gastroenterology fellow at both Mt. Sinai Medical Center and Metrohealth Medical Center in Cleveland, Ohio.

His interests are in pediatrics, ultrasound and toxicology. He is an attending physician at HonorHealth Scottsdale Thompson Peak Medical Center in the department of emergency medicine and serves as director of education there. He concurrently is Clinical Associate Professor of Emergency Medicine at The University of Arizona. He also teaches Evidenced Based Medicine and Directs Journal Club for the Maricopa Medical Center Emergency Medicine Residency.

His interests are in pediatrics, ultrasound and toxicology.

5 Neurology Charles Crinnian, MD Dr. Crinnian is a board-certified neurologist and lead neuro-hospitalist at HonorHealth. He graduated from the University of Arizona Medical School, Tucson, with honors and completed his neurology residency at Barrow Neurologic Institute in Phoenix in After residency, he founded Frontier Neurology and established several rural neurology clinics around the state. As a professional pilot, he flew to these clinics to provide neurological care. He s still an active pilot and is involved in aerospace medicine and aviation safety as a consultant to the Federal Aviation Administration and community pilots. After residency, he founded Frontier Neurology and established several rural neurology clinics around the state.
