2 INTRODUCTION The decision about which nursing home or elder care facility would be the best fit for a loved one can be very difficult and should be made carefully. We advise our clients to take a tour of facilities and become informed about the relative benefits of each nursing home in the area before making a decision. Some things to consider when researching include the type of care that the facility provides, the quality of services, the location of the facility, and the cost of care. If there is a need for Medicaid benefits to pay the costs of care, then it is important to investigate whether the facility accepts Medicaid and if a Medicaid bed will be available. Regardless of the facility chosen, the most effective way to ensure quality of care is to be diligent with checking in on the loved one, asking questions of the staff, and participating in care conferences. The following information has been compiled for the benefit of our clients who must decide on which facility best suits their needs. We have also included information about residential and assisted living facilities. Please note that all of the following information is current as of October, You should contact the facility for the most up-to-date information. This information is subject to change quickly and the following link can also provide you with additional information: CHRISTINA HOUSE Christina House is a private-pay assisted living facility located just off of Highway 31 in Franklin. Christina House is owned and operated by Assisted Living Concepts, Inc. Assisted Living Concepts also operates twenty-three other facilities in Indiana including Shields House in Seymour, Jewel House in Madison, and Walker House in Shelbyville. Christina House can do an assessment to determine if they can meet a potential resident s needs, and 24/7 admission is available. Most residents are free to come and go as they please, but Christina House is a secure facility and the staff can monitor doors as they open and close. Alzheimer s and dementia patients are welcome. In addition to room and board and coordinated activities, the daily room rate at Christina House includes assistance with medication, bathing, and dressing. There are now fifty-nine individual apartment units at Christina House since the addition of their newest wing. Studio, one and two bedroom units are offered. Each unit has a kitchenette that includes a small refrigerator, microwave, and sink. Other than using the microwave, cooking is not permitted in the apartments. There is a full bath in each unit with a walk-in-shower. Residents may also use the community spa tub. Each unit is equipped with two pull cords for emergencies, one in the bathroom and one in the bedroom. Some of the room services provided as part of the daily rate include cleaning, maintenance, laundry services, and standard utilities.

3 Three meals each day and snacks are included in the daily room rate. The meals are coordinated by a dietician and served restaurant-style to the residents. At each meal, the menu features two main choices as well as several alternates. Special dietary needs can often be accommodated also. Activities at Christina House are scheduled three times each day and once every evening. Activities include games, exercise, church services, and outings to area attractions. A beauty shop is available for residents, as well as a computer lab, two courtyards, an exercise room, a laundry room for those who choose to do their own laundry, and several common sitting areas. Two full kitchens are also available for activities or for residents to reserve for private events. Christina House also has a massage room, with a massage table and a massage therapist so that residents can relax and be pampered. Christina House offers 24 hour, on-site care for all residents. Licensed nurses are on site or on call at all times. Staff members coordinate with each resident and their physician to develop an individual care plan. The facility also has a doctor s office available so that local physicians can visit and see patients without the residents ever having to leave the building. For those who choose to see a physician outside of Christina House, transportation is available. Physical, occupational, and speech therapies are available in-house at an additional cost for residents who are in need of those services. Christina House prides itself on being able to care for residents, even as their care needs start to increase. Hospice services are available to residents should it become necessary. Respite care is provided at Christina House for residents who need short-term care or for caregivers who need a break from taking care of a loved one. Residents who are receiving respite care are provided a furnished room, all meals, and access to all activities. Expenses: A one-time, non-refundable, New Resident Fee will be collected at the time of move in. Potential residents should contact Christina House for rates specific to their needs based on the following scales: Basic Service Fee: The basic service fee is a daily rate based on the style of apartment that a resident chooses. The basic service fee ranges from $50.00 to $70.00 per day. Service Level Rate: The service level rate is a daily rate based on the supportive personal care that is required and desired for each individual resident. The service level rate ranges from $14.00 to $78.00 per day.

4 Contact Information: Location: 1435 Christian Blvd. Franklin, IN Contact: Kathryn Pierce- Residence Sales Manager (317) Website: COUNTRY CHARM INDEPENDENT & ASSISTED LIVING COMMUNITY Country Charm is a licensed assisted living community on the west side of Greenwood. Country Charm has two sister facilities, River Crossing in Charlestown, and River Bend in Jeffersonville. Country Charm is a provider in the Indiana Medicaid Waiver Program and is currently full with a waitlist. Country Charm is not a secured facility and can only accept potential residents with early stages of Alzheimer s or dementia if they are not at risk of wandering. There are eighty-nine individual apartment units at Country Charm. Seventy units are large, one-bedroom units, and the other nineteen units are studio or small onebedroom units. All of the units are certified for both independent living or assisted living. Every unit at Country Charm has a door to the interior of the building as well as a door to the outdoors. Each unit also has a small covered porch, a private bathroom with a walk-in shower, and a full kitchen. An emergency pull cord is located in every bathroom and every bedroom for safety. Residents all have mailboxes on their doors and mail is delivered directly to their units each day, rather than being delivered to a central mail location. The apartment units do not come furnished, and residents are welcome to furnish and decorate as they please. Many activities are available to residents at Country Charm. Daily activities are scheduled and posted on an activities calendar. Some of the activities include crafts, bowling, bible studies, exercise, and outings to many local attractions. In addition to the scheduled activities, Country Charm has a lake with a walking path, a beauty shop, an activity room with exercise equipment, a library, and a general store. Residents also enjoy the large foyer with a brick hearth and flat screen television. Country Charm has a large chapel, where both catholic and non-denominational services are offered each week. Meals are served at three scheduled times each day in the dining rooms. There are three dining rooms in all, one in each building, so that residents do not have to leave their own building if they so choose. The meals are selected from a selective menu one day in advance, but residents are always free to make an alternate choice instead.

5 Residents may also visit the ice cream parlor, where drinks and snacks are available between meals. A nurse is on staff or on call 24 hours per day at Country Charm. A nurse is always in the building from 8:00 a.m. until 10:00 p.m. Both qualified medical assistants (QMAs), and certified nursing assistants (CNAs) are on staff at all other times. There is a physician s office on-site, so that residents may be seen by doctors without having to travel to a doctor s office. Best Choice Home Health provides physical therapy for those residents who are in need of therapy services. Respite care and adult daycare are not available at Country Charm. Expenses: There is a $ non-refundable application fee to get on the waiting list or hold an apartment. The $ will only be refunded in situations where the potential resident passes away or is determined by staff to be not suited to living at Country Charm. 6 Website: FRANKLIN MEADOWS Franklin Meadows is a skilled nursing facility owned by American Senior Communities. American Senior Communities operates a sister facility, Arbor Grove Village, in Greensburg. Franklin Meadows has 114 beds, all dually certified for Medicare and Medicaid. The staff at Franklin Meadows can do an assessment to determine if the facility is able to meet a potential resident s needs. There is currently no waitlist at Franklin Meadows. Franklin Meadows has a rehab-to-home unit with six Medicare rooms. All six Medicare rooms come equipped with flat-screen televisions. The skilled nursing unit has forty-three rooms, and the secured Alzheimer s unit has nine rooms. All rooms are companion rooms with the exception of two private rooms in the Alzheimer s unit. All rooms come furnished with a bed, closet, nightstand, and chair. Restrooms are shared between rooms. Cable and telephone services are not included in the daily room rate, but those services may be provided at an additional cost. Dining at Franklin Meadows is served in the main dining room, or residents may be served in their rooms. An assisted dining area is also available for those residents who may need assistance with eating. Residents meals are served three times each day from a set menu. Alternate meal choices are also available. Franklin Meadows has a full-time activities coordinator and an assistant activities coordinator. A variety of activities are available each day for residents who choose to participate. Many of the activities take place in the facility s large activity room. For those residents who choose to smoke, Franklin Meadows is the only facility in Johnson County that offers an indoor smoking room. The smoking room has a door that is closed at all times to separate it from the rest of the building. All residents who choose to smoke must be supervised while doing so. The medical staff at Franklin Meadows includes Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and Certified Nursing Assistants (CNAs). Physical, occupational, and speech therapies are also available on-site. Franklin Meadows offers options for short-term, respite care for those who need a break from taking care of a loved one. The respite stay is for a twenty-four hour period and is paid based on a daily rate. Expenses: There is no buy-in fee, and the room and board are payable month-to-month based on current daily rates. 7 Contact Information: Location: Contact: Website: 1285 West Jefferson Street Franklin, IN Cherry Smith (317) FRANKLIN UNITED METHODIST COMMUNITY Franklin United Methodist Community, (Methodist), is a not-for-profit continuing care retirement community. Methodist offers independent living options, assisted living options, long term nursing care options, Alzheimer and dementia care options, as well as temporary rehabilitation options. Methodist has some nursing beds that are dually certified for Medicare and Medicaid, but Medicaid waivers are not accepted. Staff members can do an independent assessment to determine where a potential resident would best fit within the community. Methodist operates at full capacity in most areas, and there is currently a waitlist for the final stage Alzheimer s unit. Methodist has 110 beds in the skilled nursing area known as the Health Care Center. Approximately two-thirds of those beds are dually certified. The Health Care Center has a newly renovated dining room and several new resident rooms. The newer rooms offer private bathrooms with showers. Those rooms are only available for private-pay residents. Thirty-seven additional beds are located in the rehabilitation area, and fifteen are dually certified. The remaining twenty-two beds in the rehabilitation area accept Medicare. Rehabilitation rooms are all semi-private with a partial wall divider. Each rehabilitation room includes television and telephone. Methodist has three units dedicated to Alzheimer s and dementia care. All three units are secured for the safety of the residents. Rooms in all three units have a bed, nightstand, table and a bathroom. Shower facilities are centrally located. The first unit, Special Care East, has twenty-four beds and caters to the needs of those residents in the beginning stages of Alzheimer s or dementia. Special Care East is a private pay only unit and cannot accept Medicaid. The second unit, Special Care West, also has twenty-four beds and focuses on the needs of those residents in the middle stages of the disease. Special Care West offers a larger nursing staff to properly meet the growing needs of those residents. The final unit, Advanced Special Care, has twenty-nine beds and is designed to care for those residents who are in the final stages of the disease and

8 require additional care. All rooms in the final unit are semi-private, with a partial wall dividing the living space in each room. There is no outside access in the final unit. The three dementia units have their own chapel, courtyard, activities, and dining areas. Methodist offers a variety of apartment options for residents. Apartments are available in studio, one-bedroom, or two-bedroom, unfurnished units. Each apartment unit has a refrigerator, microwave, freezer, and sink. Independent apartments are available for residents who are still able to get themselves to nursing care on their own. Assisted living apartments are available for those residents who can no longer get themselves to nursing care and now require a nursing staff member to come to them. Library apartments are additional assisted living apartments for those residents who require a higher level of care. One unique apartment option at Methodist is the swing apartment units. Swing apartments are designed for married couples, where one spouse requires assistance and the other spouse does not. The swing apartment plan allows the couple to only be charged for assistance to the spouse that needs it, not to both. The assisted living apartment area has its own dining room, and residents are also welcome to dine in the main dining room. All assisted living residents enjoy a whirlpool bath once each week, at no additional cost. For those residents who are completely independent, cottage homes are also available. 197 cottage homes are available. The cottages are ranch-style or duplex units, with two or three bedrooms and attached garages. Meals at Methodist are served restaurant style. A rotated seating schedule invites residents to dine anytime during set dining hours. Waiters/waitresses seat residents as they arrive. A variety of meal options are available at each meal. In addition, every Sunday residents enjoy a buffet. Residents in the nursing center can be served in their rooms as needed. One unique feature at Methodist is the Life Care Agreement. Residents may choose to pay for a Life Care Agreement as an alternative to a monthly agreement when they move into a cottage home, independent apartment, or assisted living apartment. The Life Care Agreement guarantees that the resident will continue to be cared for throughout life, regardless of what their future care needs might be. The Life Care Agreement also guarantees care regardless of a resident s inability to pay in the future. Many amenities are available to residents at Methodist. A beautiful chapel offers non-denominational services each week. Multiple sitting areas and living spaces are available throughout the community as well as an exercise facility. The ground floor of Methodist however, is what sets it apart from other facilities in Franklin. The ground floor offers a computer lab with computer classes for residents, a ceramics room, an art studio, a woodshop, general store, used goods store, a bank branch, indoor greenhouse, and much more. A fellowship hall and full kitchen is also located on the ground floor for residents who would like to host special events. If residents care to enjoy activities outside of the community, three wheelchair vans and three cars are

9 available to transport them. A concierge is available to help residents plan the activities that they would like to participate in. Respite or adult daycare is not available at Methodist to anyone outside of the community. Only current residents may enjoy those services. Expenses: United Methodist offers two ways to pay for care. Jefferson Street Franklin, IN Diane Amari- Sales Director (317) Toll free: (877) GREENWOOD HEALTH AND LIVING COMMUNITY Greenwood Health and Living Community is located just off of Highway 31, on Fry Road in Greenwood. The facility is owned and operated by an Indiana family business known as CarDon & Associates. Other area communities that are operated by CarDon & Associates include Bell Trace Senior Living Campus in Bloomington and Brown County Health and Living Community in Nashville. There are a total of 18 communities owned by CarDon & Associates throughout the state of Indiana. Greenwood Health and Living is generally able to accept residents with all medical needs. While there is no separate Alzheimer s or dementia unit, potential residents with Alzheimer s or dementia are welcome. There is currently a waiting list for long-term Medicaid beds. However, a new resident can be accepted into a short-term room until a Medicaid bed becomes available. Those residents who are in short-term rooms waiting for a Medicaid bed receive priority on the waiting list. There are 121 beds at Greenwood Health and Living, and all but four are dually certified for Medicare and Medicaid. The remaining four beds are located in private rehabilitation rooms. Most of the rooms are semi-private, but a few private rooms are also available. All of the residents at Greenwood Health and Living are provided with standard furnishings, and residents are welcome to bring additional furniture as desired. Cable service is included in the daily rate. Telephone service is only included in the daily rate for those residents in rehabilitation rooms. Each room has a private bathroom and shower facilities are centrally located. One unique feature at Greenwood Health and Living is that each resident room has a set of French doors. The French doors do not open, but they allow a maximum amount of natural light to enter each room. One additional room at Greenwood Health and Living is a Hospice suite. The Hospice suite is dually certified for Medicare and Medicaid. The Hospice suite

12 includes a bathroom with a shower, a refrigerator, and sitting area. Family members are welcome to stay in the Hospice suite as well. Meals at Greenwood Health and Living are served in the main dining room. Meals are planned on a set menu. Menus are posted three days in advance and substitutions are always available. In 2011, Greenwood Health and Living will implement a new dining program that will allow residents to choose from a selective menu at each meal. The medical staff at Greenwood Health and Living includes a medical director, Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs) and a full therapy staff. A registered nurse is on staff during all shifts. Physical, occupational, and speech therapies are available in multiple therapy gyms located within the facility. A back to home apartment is also available so that residents can practice life skills in a safe environment before returning home. Before a resident goes home, a therapist will also make a home visit to help the resident prepare their home for their return. Activities at Greenwood Health and Living are coordinated by a full-time activities coordinator. In addition to the daily scheduled activities, residents also enjoy visiting the beauty salon and the secured courtyard. Residents are also free to have visitors at any time due to the facility s 24-hour visitation policy. Greenwood Health and Living also offers respite care for those who need a break from taking care of a loved one. The respite stay is paid daily, and there is no limit to how much or how often one is able to use those services. 13 Contact Information: Location: Contact: Website: 937 Fry Road Greenwood, IN Mahima Regmi- Admissions Coordinator (317) GREENWOOD VILLAGE SOUTH Greenwood Village South is a continuing care retirement community located just off of Highway 31 in Greenwood. The community is managed by Life Care Services. Greenwood Village South offers independent retirement living, assisted living, dementia care, skilled nursing care, and short-term rehabilitation. Staff members can do an assessment to determine where the best place for a potential resident would be within the community. The health care center at Greenwood Village South, The Village Pavilion, has a total of 137 beds. 77 beds are dedicated to long-term, skilled nursing care. Some of the beds in skilled care are Medicaid certified, however those beds stay full internally and are not available to non-residents. Both Medicare and private pay rooms are available. 30 additional beds in long-term care are located in a dedicated, dementia care unit. The remaining 30 beds are for short-term rehabilitation stays. The rehabilitation rooms include a flat screen television, cable, and telephone services. All of the rooms in the Village Pavilion include wall-to-wall carpeting and are furnished with a bed, wardrobe, table, and nightstand. Residents are welcome to bring their own furnishing as well. Each room has a private bathroom. Shower facilities are centrally located. Meals at the Village Pavilion are served three times each day. Meals are selected from a weekly menu. Each resident chooses meals ahead of time and may eat in the dining room or in his/her own room. Special dietary needs can be accommodated as needed. A variety of activities are available to residents each day at the Village Pavilion. In addition to daily scheduled activities, the Village Pavilion has a beauty salon, multiple courtyards and patios, a chapel, and a large activity room to keep residents busy and active. Activities in the dementia care unit are specifically designed for dementia patients. The dementia unit has its own secured courtyard as well. The Village Pavilion offers 24-hour nursing care, restorative nursing, multidisciplinary licensed health care, and social services staff professionals. An on-site dentist, podiatrist, audiologist, and optometrist are also available. A variety of therapy

14 services including physical, occupational, and speech therapies are available to residents. Residents in the rehabilitation unit are provided with a home evaluation before returning home. A staff member evaluates the home environment of each resident before they return home to ensure that they are returning to a home that can accommodate their needs. The assisted living apartments at Greenwood Village South are located in the Village Manor. Studio and one-bedroom assisted living apartments are available. Each apartment is carpeted and includes emergency call buttons in the bathroom and bedroom. Apartment units at the Village Manor are unfurnished, and residents are free to decorate and furnish as they please. 24-hour nursing care, housekeeping, medication management, assistance with bathing, and all utilities except telephone services are included. Assisted living apartments do not have kitchenettes, but Village Manor has its own dining room for residents to enjoy. Meals are served restaurant style, three times each day. Activities are also scheduled each day for the residents of Village Manor. Residents in the Village Manor assisted living apartments pay a one-time, Lifetime Occupancy Fee, or LOF, in addition to monthly service fees. The LOF gives the resident priority in the Village Pavilion, should skilled nursing care ever become necessary. Skilled nursing care is guaranteed with the LOF, regardless of a resident s ability to pay in the future. Residents who are not interested in the LOF option may choose to lease at the Village Manor instead. Residents who choose to rent sign a one year lease and pay a deposit equivalent to one month s rent. The final part of the Greenwood Village South community is the independent senior living apartments and cottages. To qualify for independent living at Greenwood Village South, potential residents must be at least 62 years old, pass a memory health evaluation, and meet the established financial requirements. Cottage homes are 2-3 bedroom ranch style homes. Independent apartments in the Arms building are customized studio, one-bedroom, or two-bedroom units. Each apartment unit is remodeled completely before a new resident moves in. Each apartment unit includes a full kitchen. Every independent living resident enjoys one meal each day included in the monthly fee. Residents may choose to enjoy a meal in the dining room from a full menu, enjoy a breakfast or lunch in the café-bistro, or order carry-out to be enjoyed in their apartments or cottages. Amenities for independent living residents are limitless. A full-service bank, library, beauty shop, barber shop, fitness center with personal trainer, woodworking shop, billiard room, card room, private meeting and dining rooms, and a convenience store are all located in the Arms building. A professional activities coordinator is also on staff to schedule in-house activities and outings for residents.

15 In the independent living apartments or cottages, an LOF, or Lifetime Occupancy Fee is available. The LOF not only guarantees care for life, but it also includes a 5-year pro-rated refund on 90% of the LOF. The refundable portion is reduced to zero after the 5-year period. Potential residents can also choose to pay a Return of Capital Occupancy Fee, or ROC. The ROC functions like the LOF with one additional perk. When a resident pays the ROC, in addition to lifetime care, the resident receives a 90% refund of the total lifetime occupancy fee paid, regardless of the time period in which the apartment or cottage is occupied. The refund is issued when the unit is reoccupied by the next resident, or after one year if the residence has not been reoccupied. A resident may be able to simply pay a monthly rental fee for an independent living apartment if he/she is not interested in the LOF or ROC options. Expenses: Health Care Center- The Village Pavilion Resident rates reflect a 10% discount over the non-resident rates for those residents who were already living at Greenwood Village South in either assisted living or independent living apartments/cottages. Escrow deposits for non-residents may also apply. The Village Pavilion requires an advance deposit of thirty times the daily rate for private pay individuals. Deposit to be applied to invoices at the end of a resident s stay. Unused portions of a deposit will be refunded. The Hearth is a senior living community that offers independent living and assisted living apartments, and secured memory care. The community is owned by Hearth Management Communities. Hearth Management Communities operates a sister facility, Sycamore Village, in Ft. Wayne. The Hearth is a private pay facility and cannot accept memory care residents on Medicaid. There is currently a waitlist at The Hearth. The apartments at The Hearth are available in a studio, one-bedroom, onebedroom deluxe, and two-bedroom size. The one-bedroom deluxe model features more square footage than a traditional one-bedroom unit, and the one-bedroom deluxe includes a walk-in closet. All units include wall-to-wall carpeting, individually controlled heating and air conditioning, and scheduled housekeeping. All one and twobedroom units are equipped with a full kitchen including a stove, refrigerator, and microwave. Studio units have a kitchenette, but do have a stove. In addition, all units

17 have emergency pull cords in the bathroom and bedroom. Residents can also request emergency pendants to wear for an additional cost. Bathrooms in each unit are equipped with oversized medicine cabinets, walk-in showers, and handrails. All of the apartment units at The Hearth can be occupied as an independent living unit or assisted living unit. Residents who require the extra care of assisted living simply pay for those extra services. Residents in the apartments at The Hearth enjoy sunrooms on every floor, a billiard s room, a library, numerous sitting areas, and a country kitchen that features tea and cookies for residents at all times. One unique feature at The Hearth is the movie theater room. Residents can enjoy movies in a theater-like setting, complete with theater seating and popcorn machine. A beauty shop and barber shop are also on-site for those residents who choose to utilize them. Activities, such as exercise classes, are scheduled daily for residents, as well as club meetings and special events. Outings are scheduled every Monday, Wednesday, and Friday. Some of the outings offered include trips to local churches and restaurants. Meals at the Hearth are served in the beautiful dining room. Residents are served restaurant-style. Each meal features two main choices as well as numerous alternatives. A private dining room is available for residents who may be hosting family meals or special events. Nursing care in the assisted living and independent living apartments is available 24 hours a day. Two nurses are available each day, as well as two or three Certified Nursing Assistants (CNAs). The residential care program at The Hearth is licensed by the State of Indiana. A physical therapy room is also available for those residents who would benefit from therapy services. The memory care unit at The Hearth, Keepsake Village, is located in an adjoining building. Keepsake Village is a thirty-bed unit that runs full most of the time. The Keepsake Village building is protected by a Wander Guard safety system. All of the rooms at Keepsake Village are private and include a private bathroom with shower. Residents at Keepsake Village enjoy their own secured courtyard with walking paths, dining room, and activities coordinator. Meals are selected from a set menu that includes two options at each meal. Homeview is owned by TLC Management, a company based on Christian faith. Homeview prides itself on carrying those Christian beliefs into its facility. Homeview has been in the community for over 80 years, with the center originating in the founder s own home. Homeview has grown over the years to include a secured memory care unit and a rehabilitation unit. All beds at Homeview are dual certified to accept both Medicaid and/or Medicare. Staff members can do an assessment to determine if they will be able to meet a potential resident s needs. Homeview of Franklin has 114 beds. All rooms in the skilled nursing unit are semi-private, with two beds in each room. All of the rooms come furnished with a bed, dresser, and nightstand. Residents are welcome to bring their own additional furnishings and decorate as they please. Private bathrooms are located in each room. Common shower facilities are centrally located. The rehabilitation unit, Pathways, is only four years old. All forty-three beds in the rehabilitation unit are located in semi-private rooms, with the exception of one private room. The rehabilitation unit includes a large therapy gym. Physical, occupational, and speech therapies are all available. Each resident may receive up to three hours of therapy each day. The Alzheimer s unit at Homeview, Touchstone Terrace, specializes in providing a warm, caring environment that is safe and secure for residents with Alzheimer s or forms of dementia. All of the rooms in Touchstone Terrace are semiprivate, with the exception of two private rooms. Touchstone Terrace has its own dining room, courtyard, and garden so that residents do not have to leave their secure environment. Touchstone Terrace also schedules its own activities that are specially designed to benefit residents with memory loss. Daily routines and consistent staff presence are utilized to minimize resident confusion. Meals at Homeview are served in the dining room for residents in the skilled nursing and rehabilitation units. Residents are also able to eat in their rooms if they so choose. Meals are chosen from a selective menu. The Pathways rehabilitation unit now has a steam table, which allows meals to be served to individual rooms without getting cold. The memory care and skilled nursing units will be getting steam tables soon.

19 Activities at Homeview are scheduled daily. Activities include games, live music, craft projects, cooking, religious services, and cookouts. A beautiful courtyard and raised gardens are available for resident enjoyment. Outings are also scheduled to local attractions such as shopping centers and the local community senior center. A resident council meets once every month to communicate to staff what activities the residents would prefer. One unique feature at Homeview, is the resident dog. Residents enjoy spoiling the resident dog at their leisure. The medical staff at Homeview includes Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and Certified Nursing Assistants (CNAs). Homeview features 24-hour nursing care. Nursing services are available for complicated medical conditions that require IVs, wound care, feeding tubes, and respiratory treatments. Hospice care is also available. Homeview offers Respite Care for those family members or caregivers who may need a short break from caring for a loved one. Respite stays are generally less than thirty days. Respite stays include three meals each day, overnight nursing care, medication administration, and the opportunity to participate in activities. The Healthcare Foundation also operates Hickory Creek facilities in Columbus, Scottsburg, Madison, and Greensburg. Hickory Creek is a small facility that has capacity for thirty-six residents. Each bed is dual certified to accept residents who receive Medicare and/or Medicaid. The staff can do an individual assessment to determine if they will be able to meet a potential resident s needs. The rooms at Hickory are semi-private rooms with two beds in each. Each room comes furnished with a bed, bedside table, and chair. Residents often bring their own additional furnishings as well. Residents may bring in their own phone and television. Cable and phone services can be installed by local providers. The rooms at Hickory Creek do not have private bathrooms. Residents use common bathrooms and shower facilities. Meals at Hickory Creek are served to the residents in the dining room. The dining room has been decorated to make the atmosphere more home-like. Staff members assist residents in choosing their meals from a weekly selective menu, which includes several meal choices for each day. In addition to three meals, snacks are available throughout the day and supplemental nutrition is provided as needed. Some of the activities at Hickory Creek include bingo, exercise, and visits from local churches. Activities are coordinated by a full-time activities coordinator. Hickory Creek also has a beauty shop on site, and a beautician comes in once each week. The medical staff at Hickory Creek consists of a Medical Director, Registered Nurses (RNs), Licenses Practical Nurses (LPNs), and Certified Nursing Assistants (CNAs). Hickory Creek also has physical, occupational, and speech therapies available for the residents. One unique feature at Hickory Creek is the Guardian Angel program. Through the Guardian Angel program, each department head is assigned two rooms where they will be responsible for giving the residents in those rooms assistance with special tasks such as shaving, or reading the weekly menus. Hickory Creek offers options for temporary or respite care for those who need a break from taking care of a loved one. Adult daycare is payable by the hour and is available throughout the day and evening. The adult daycare rate includes meals and a bed for the resident to lie down. The respite stay is for a twenty-four hour period and is paid daily for up to thirty days. Expenses:

The Masonic Home has been a fixture in Franklin since The Masonic Home has a population of 400 residents and offers skilled nursing, short-term rehabilitation, secured memory care, assisted living, and independent senior living. The community is made up of six main buildings, all devoted to specific types of care. Assisted living and independent living units are only available to Masonic members and their families. All other areas are open to the public. The Masonic Home has been managed by Life Care Services since August of The Masonic Home has 172 beds in the areas of skilled nursing/rehabilitation, and memory care. Semi-private and private rooms are available. All of the beds are dual certified to accept Medicaid and/or Medicare. Each room is furnished with standard furnishings and residents are welcome to bring a small number of personal furnishing with them. All of the rooms in the skilled nursing area have a view of the outdoors. Every room also has a private bathroom. Meals in the skilled nursing building are served in resident rooms on trays. In addition to a full nursing staff, the residents in skilled nursing have access to specialists such as dentists, podiatrists, and therapists, who are all on-site to meet residents needs.

22 One unique feature of the Masonic Home is that it has a chapel located in the skilled nursing building, specifically for those residents. Volunteers and staff members help residents make their way to the chapel each week so that everyone has the opportunity to attend church services if they so choose. In addition to the chapel, the skilled nursing residents enjoy a large courtyard, a shelter house for cookouts, and several daily activities. Masonic Home offers assisted living apartments in the Scottish Rite Memorial Building for Masonic members and their families only. Assisted living apartments offer independent living with additional assistance with daily activities such as medication administration and bathing. Apartments are studio-style units with private bathrooms, nurse-call systems, walk-in closets, individual heating and air conditioning, small refrigerators, and carpeting. Assisted living apartments are unfurnished and residents are free to decorate and furnish individual units as they wish. The second floor of the assisted living building is secured, so that residents with early stages of Alzheimer s and dementia can continue to live in their own apartments in a safe manner. The assisted living building has its own small dining room so that residents do not have to leave their building for meals if they choose not to. In May of 2011, a new assisted living building will open at Masonic Home. The new building will offer forty-eight units. All of the new units will be complete with a refrigerator and microwave. Masonic Home is hopeful that they will be able to accept Medicaid waivers when the new building opens. The Masonic Home offers one, two, and three room apartments for independent living. Ranch style homes are also available. All independent living units are only available for Masonic members and their families. Independent living apartments all have refrigerators, and some two and three room units have kitchenettes. All units are carpeted and have private bathrooms. Dining for independent and assisted living residents is served in both the main dining room and the smaller dining room. Meals are served restaurant style with a full menu of options to choose from. The main dining hall is also open to the public for dining purposes. 24 hour nursing care is available for residents in assisted living and independent living apartments. Acacia Home Health Services are available to residents so that care can be administered in a home setting, preventing the need for a lengthy hospital or nursing home stay. Masonic residents enjoy a variety of amenities and activities. The main building houses a chapel, a bistro, an ice cream shop, library, and a gift shop. A concierge is available to shop for those residents who do not wish to travel to local stores themselves. Buses are available to transport residents to outings and appointments. Residents can visit the on-site beauty salons, barber shop, and dental clinic as they wish. In addition, many residents enjoy participating in classes at Franklin College.

The Morning Pointe facility is owned by Independent Health Care Properties. Morning Pointe is a private pay facility and cannot accept potential residents on Medicaid. Morning Pointe does accept residents who are eligible for the American Wartime Veterans Pension. Move in specials are often available for potential residents. Morning Pointe has fifty assisted living apartment units. Units range in size from mini-studio to one bedroom deluxe. Each unit is unfurnished and residents are welcome to furnish individual units as they wish. All assisted living units include a microwave, refrigerator, sink, kitchen cabinets, curtains, and bed linens. Bed linens are changed weekly. Residents are welcome to use laundry facilities to do laundry, or laundry services can be provided. Each unit has a private bathroom with a walk-in shower. All units are private, however couples are welcome to share. Meals at Morning Pointe are served in the spacious dining room. Residents enjoy three meals each day from a set menu that is posted daily. Several alternative choices are also available at each meal. For late risers, a continental breakfast is available daily. Snacks, coffee, and tea are available between meals. A full-time activities coordinator plans activities such as daily devotionals for residents. Residents often enjoy outings to local restaurants and attractions. A beauty shop and barber shop are available on-site every day, as well as a television room, activity room, and multiple sitting areas. Two courtyards and a large front porch are also available for resident enjoyment. One unique feature at Morning Pointe is the hallway décor. Each month the hallways are decorated to reflect upcoming holidays or seasons. Morning Pointe provides 24 hour nursing care for its residents. A Licensed Practical Nurse (LPN) is on staff daily. Home Health Services provides therapy for those residents who may benefit from therapy services. A doctor also visits Morning Pointe once each week for residents who do not wish to travel off site to see a physician. The Lantern is the dedicated Alzheimer s/dementia unit at Morning Pointe. The Lantern has fifteen private rooms; however, couples are welcome to share. Apartment units at the Lantern are similar to the assisted living units at Morning Pointe. One difference is that apartment units at the Lantern do not have a microwave or refrigerator. The Lantern is only two years old and is designed to specially care for those residents who are suffering from Alzheimer s or similar dementia disorders.
