2 Welcome Welcome to Holy Family Hospital Rehabilitation at Providence Health Care (PHC). We have a culture of caring hearts guided by our principle of How you want to be treated. We are committed to support and respect you as we provide quality, compassionate care that meets your physical, emotional and spiritual needs. Providence Health Care has been serving the community and province for more than 115 years. We have a rich history founded on the enduring values of the sisters who built our organization. While you are here, you will work to improve your strength and ability to do normal, everyday activities. Please ask questions if you don t understand something. Let us know what is important for you to work on. Our focus is to promote your independence in a safe environment. We ask and encourage you to take part in your own care and therapy. 3 How Long Will You Stay? The length of time you stay depends on why you are here and what rehabilitation you need. The chart below gives you an idea of what the average length of time might be. What You Need to Bring Patients get dressed every day for active therapy and exercises. Please bring the following: If you have had A stroke A general decline in your strength or physical ability An amputation A fractured hip A knee or hip replacement Ask your therapist 5-6 weeks Ask your therapist Up to 4 weeks 2-3 weeks Comfortable and loose fitting clothes such as jogging pants, shorts and T-shirts Socks and supportive flat shoes with closed toes and backs Pyjamas Brush or comb, toothbrush, toothpaste, deodorant, facial tissue, hand wipes and shaving supplies Watch and/or clock Your stay may be shorter or longer, depending on your needs. This should be discussed with your rehabilitation team. Eyeglasses and hearing aid (if you use them) A cell phone if you wish, (there are no phones in the patient rooms), long distance phone cards Money (Keep only $5 at your bedside) Please leave valuables and large amounts of money at home. More money can be put in safekeeping at the Admitting Front Desk. 2
4 What You Need to Know Bed Assignments You are assigned a bed when you are admitted to the Holy Family Rehabilitation Unit. Sometimes male patients and female patients will need to share the same room. There are times when we may need to move patients from one room to another (such as a need for special equipment or a change in health). If you need to be moved, we will explain the reason for the move. Our private rooms are only used for special care situations. Thank you for your understanding. Therapy We design your therapy to meet your specific needs. Your therapist will help you make a plan to meet your goals for rehabilitation. Every week you will decide on goals of therapy with your rehab team. These will be posted on the whiteboard at your bedside. Your Therapy Schedule Your therapy schedule is posted in the hallway. Please check daily because therapy times can change. If you need assistance to read your schedule or are unable to attend therapy, please let a staff member know. Please arrive promptly and turn cell phones off. Therapy sessions run Monday to Friday. There is no therapy on weekends or holidays. Outside Therapy and Extra Care Holy Family Hospital Rehabilitation has a variety of health care professionals. You may want to have extra care beyond what we provide, or you may want to have treatments we do not offer. We call the people who give this extra care Non-staff Practitioners. It may be possible to have these types of care while here. Let us know what type of extra care or treatment you want, and we will look at the proposed care or treatment. If it does not interfere with the care you are getting from us, we will give you instructions on how to arrange for extra care. Please note: These are private services. They are not covered under your medical coverage for your hospital stay. You will need to pay for these services directly to the service provider. Weekend and Day Passes A pass may be given for you to go home for a day or overnight. Sometimes, we may suggest to your doctor that you be able to go out on passes. Your doctor must agree that you are ready to go on passes. If you would like to request a day/weekend pass, you must inform your nurse and therapist a minimum of 3 days prior to the day of the pass. This allows us time to organize any equipment or medications you may need. You or your family must arrange the ride to your home. When you leave, always sign out in the Leave of Absence Book at the Nurses desk, and tell your nurse. 3
5 Daily Living Meals Everyone eats meals in the dining room. Your medications will be given to you by your nurse during this time. Please remain in the dining room until you have received your medications. Please come to the dining room for meals at these times: 9:00 am 12:30 pm 5:30 pm Refreshments In the Main Dining Room (Rehab 2 Dining Room) Ice water, juice, tea, and milk are available in the patient kitchen. Coffee is offered from: 7:00 to 9:00 am Cookies and juice are offered from: 6:30 to 7:00 pm If your visitors would like refreshments, please direct them to the Cafeteria. Visiting Hours Visitors are welcome until 8:30pm Visiting at mealtime: Due to limited space and patient safety, we ask visitors not to visit in the dining room during mealtime. If you need to visit at this time please talk with your nurse. You may be able to take your meal tray and visit in the cafeteria. Your family and friends can stay in touch with you while you are in the hospital with our wellwishes service at: wellwisheshf@providencehealth.bc.ca Ask them to include your full name, room number and their message, and our dedicated volunteers will hand deliver it. Mail will be delivered several times a week. Communicating We need to understand each other. If you do not speak or understand English well enough to have conversations about your health or to make medical decisions, we can book an interpreter to help us communicate. You will not need to pay for this. This also applies if you need a Sign Language interpreter. Showers We assist you with a shower once per week. All bed linen is changed at this time. Your shower day is posted at the bedside. 4
6 On Site Services Cafeteria Our cafeteria is located downstairs (Level 1) for patients, visitors and staff to enjoy. - meal service is available from 9:00 am to 1:00 pm. Vending machines or counter service are available 7:00 am to 7:00 pm. Church Service Pastoral Care offers different religious services throughout the week. Times and locations are posted on the door of the Pastoral Care office and on the blue bulletin board outside the Multipurpose Room (also known as the Rehab Unit 1 Dining Room). The chapel is located on the 3rd floor (ECU). Laundry Your family needs to take your personal laundry home for washing. If you prefer, you can arrange for laundry service at the Admitting Front Desk. There is a cost for this service. Gift Shop The Gift Shop is located in the front lobby. Hours are usually: Monday to Friday - 10:00 am - 4:00 pm Saturday, Sunday, and Holidays - 2:00 4:00 pm 5
7 Pain Pain is common for many patients who come to rehab. You may have pain because of your increased activity while here. It is important to reduce pain for your health, your ability to function and your quality of life. Medication Schedule If you get pain during therapy, please take your pain medications at least 30 to 60 minutes before your therapy. Everybody s pain medication schedule is different. Please ask your nurse to explain your schedule. What Medication? To help you manage your pain, we give pain medication that usually has opioids in it (also called narcotics). Opioids are a strong pain medication. In Rehabilitation, we give opioids in such a way that it is not likely that you would become addicted. As your pain reduces, your need for pain medication also reduces. Safety and Falls Prevention Preventing falls is important to maintain health and independence. Ways to help prevent falls: Slow down and take your time. Do one task at a time. Look where you are going and don t get distracted. Sit for a moment before getting out of bed. Get out of chairs slowly. Get your balance before you walk. Wear good, supportive shoes with slipresistant soles. Wear non-skid socks at night. (We provide these.) Your physiotherapist checks to see how well you can move, stand, and walk. If we feel you would be safer with one of us helping you move, please wait for us. Do not try to move on your own. You may need to use a cane, a walker, or a wheelchair to move around safely. While on this pain medication it is common to experience constipation (difficulty having a bowel movement). To help prevent constipation: Drink plenty of fluids. Get as much exercise as your therapist advises. Take the medication the nurse gives you for constipation. 6 It is better to treat the side effects than to stop taking pain medicine!
8 Your Rehab Team You are assigned a care team while here. Teams are identified by different colors. You and your family are the most important members of the team! Your Therapy Team Occupational Therapist Your Occupational Therapist (OT) helps you re-learn how to do daily activities and become as independent as possible. Your OT makes sure you have the equipment you need to be safe in the hospital and when you go home. Physiotherapist Your Physiotherapist (PT) helps you to move and function as normally as possible. To do this, you will work on improving movement, strength, and balance. Rehabilitation Assistant Your Rehabilitation Assistant may help you do your therapy as planned by your PT or OT. Speech Language Pathologist You may see a Speech-Language Pathologist (SLP) if you have problems with swallowing, communication (including speech and cognition) or changes in your hearing after a stroke. Transition Services Team The Transition Services Team (TST) Coordinator plans the services you will need when you go home. The Coordinator works with our Rehab Team and the Health Care Team in your community to make the plan. Your Care Team Nurses Our nurses help manage your pain and health problems, and maintain your medication schedule. Doctors Our doctors (hospitalists) look after your medical needs while you are here. A Physiatrist is also involved in your care. This is a doctor with specialized training in physical medicine and rehabilitation. Your family doctor and surgeon generally will not visit here. Unit Coordinator The Unit Coordinator looks after the clerical activities on the Nursing Unit and is located at the Nurse s Desk. Social Worker The social worker can offer support to help you deal with the changes you are going through as a result of your medical condition. They are also able to refer you to community resources. Please talk to the social worker if you have any concerns about returning home at discharge. Pastoral Care Worker Pastoral care workers can give you and your family support for your spiritual well being. Our Pastoral Care Service is for persons of all faiths and beliefs, and is offered in the spirit of our Mission - recognizing the dignity and worth of every person. They offer one-on-one support, as well as opportunities for worship and prayer. They can arrange for your own faith leader to visit you. 7
9 Dietitian The dietitian can help you with problems such as poor appetite, weight loss, food allergies, special diets, food preferences at meals, and nutrition education needs. Pharmacists The pharmacy prepares and supplies your medications while you are here. Working with your doctor and nurse, the pharmacist will review your medications and recommend changes if needed. It is important to tell the pharmacist (or your nurse) if you are taking any other types of medications e.g. natural medicines, herbal products etc. Please have family take home any unneeded personal medications. The pharmacist can teach you about your medications. To prepare you for going home, you could be asked to practice taking your medications on your own while you are here. Recreation Coordinator The Recreation Coordinator plans recreation and social activities on the Rehab Unit, as well as seasonal evening bus outings. Getting Ready to Go Home You are ready to leave Holy Family Hospital when: You have worked on your identified goals and can manage safely at your discharge destination with appropriate supports or: You have stopped progressing with your goals and would benefit from other options. Planning to Go Home The day a person leaves can be a stressful time. You can reduce much of the stress by planning ahead. Start by talking to your therapists about what you will need to get ready for home. Your Social Worker can arrange a meeting with you and your family and the rehab team to discuss your progress and recommendations for returning home. The coordinator also has various supplies for you to borrow during your stay such as Books on CD/tape, DVDs, games, books, magazines, painting/drawing/knitting supplies etc. The coordinator can also refer you to programs or let you know about any leisure resources in your community. Please visit the Recreation Therapy board located by the elevator in the Main Dining Room for more information. 8
