4 Welcome to the Comprehensive Rehabilitation Center This guide is designed to provide you with information about our services, outcomes, mission, expectations and your patient rights during your stay with us. Through compassionate patient care and quality rehabilitation services, our goal is to assist our patients to restore function, regain independence, and return to the community. We provide a full complement of Rehabilitation Services through an integrated therapeutic approach and interdisciplinary team. Your physician orders therapy for you based on your individual needs. You will receive three hours of therapy at least five days a week. Length of stay at the CRC depends on your needs and progress; however, the average stay is around 11 days. In some cases, your length of stay is determined by your health plan. The team works with your health plan to obtain authorization for services necessary for your treatment. You and your family/caregiver are important members of our team. We encourage and welcome your full participation as we develop and achieve your rehabilitation goals and incorporate them into your overall plan of care. Thank you for joining our team and allowing us to serve you. Who We Serve The Comprehensive Rehabilitation Center provides services to adults over the age of 18 with, but not limited to the following conditions. Stroke Brain injury Neurological disorders Spinal cord injury Pain syndromes Amputation of limbs Cardiac disorders Multiple trauma Deconditioning Orthopedic conditions such as joint replacements or fractures 2

6 Satisfaction Survey Your satisfaction is of utmost importance for the CRC team. We want to ensure that you are provided EXCELLENT care and service. If at any time throughout your stay, you are unable to rate your experience as EXCELLENT, please let one of the team members know immediately. We are always appreciative of your feedback and the opportunity to improve. Our goal is to meet and exceed your expectations. You may also be contacted by phone to formally ask for your opinions and feedback regarding your experience at the CRC. With your help and input, we can continue to improve and provide an EXCELLENT level of service for our patients and their families. 4

8 Your CRC Experience Admission: The rehab team will orient you to the CRC so you can immediately become involved in planning your care. We will be asking you and your family members many questions on admission. This information will help us to develop an individualized plan that will meet your needs and your goals. Participation in Care Planning: You will have opportunities to be involved with your treatment program and to express your needs, expectations, preferences, and goals. Our team adjusts the services we provide to meet your needs. Family/Caregiver Involvement: Family and friends who will help you at home are also considered part of your team. These individuals are encouraged to attend your therapy session(s) scheduled throughout the day. Therapists will coordinate times with caregivers to observe and participate in the treatment process as appropriate. Their close involvement in your program is designed to assist you to transition from the inpatient rehabilitation program to community living. Items from Home: Please do not bring valuables (money, jewelry, purse, wallet, credit cards) or medications with you to the CRC. These items should be sent home with family members for safe keeping. The Hospital is not financially responsible for any lost or misplaced items. For your safety, personal medications are not permitted at your bedside unless ordered by your physician. A list of medications you were taking at home is requested upon admission. We will review this list and ensure that you receive the proper medication and dosage during your stay and upon discharge. 6

10 We recommend that you bring the following items with you on admission to the CRC: Comfortable, easy to put on clothing to include sweatpants, undergarments and shirts. You will be required to dress every day. Sturdy, rubber soled shoes with a good fit Nightclothes and robe Personal grooming items Personal medical equipment to include your dentures, hearing aids, eyeglasses, wheelchairs, walkers, braces and prostheses, c-pap, etc. Each should be identified with your name. Discharge Planning: Plans for discharge should begin immediately upon admission in order to allow for a smooth, timely and safe transition into the community. Our case manager will assist you to identify discharge options. Any follow-up services or equipment needs identified will be arranged prior to your discharge from the CRC. When you leave, you will be given detailed discharge instructions and follow-up information. Our case manager will continue to be available to answer any questions you may have once you have been discharged. Discharge from the CRC is a process that starts at 10 a.m. daily and can last several hours when: You have met your goals and it is safe for you to return to your prior setting. There has been limited progress toward goals for 2 or more days. You are unable to consistently participate in rehabilitation services. You consistently decline therapy services. You are a danger to the health and safety of others. You and/or your family choose not to participate in the plan of care that we developed together. 8

12 Your Interdisciplinary Team You and Your Caregivers: You and your caregivers are an important part of the team and your participation in the plan of care is vital. Early in your stay, the social worker or case manager will discuss your rehabilitation needs and how long you may be staying at the CRC. Our team includes you in your plan of care and discharge planning throughout your stay. Discussions about your care can occur in a variety of ways including; bedside discussions, telephone calls with caregivers, meetings and/or family conferences, as appropriate. Physiatrist: Physiatrists are physicians specializing in physical medicine and rehabilitation who coordinate and supervise your medical care and rehabilitation. The physicians team work with the interdisciplinary team to maximize your progress and lead the team to best tailor the program to meet your specific needs. Rehabilitation Nursing: Rehabilitation nurses take a holistic approach to meeting patients physical, emotional, educational, environmental and spiritual needs. They assist individuals affected by chronic illness or physical disability to achieve their greatest potential, adapt to their disabilities and work toward productive, independent lives. Physical Therapist: Physical therapists focus on improvement of strength, endurance, motor control and stability. They may help you improve your ability to stand, walk, climb steps or push yourself in a wheelchair. Occupational Therapist: Occupational therapists teach daily living activities, such as dressing, bathing, eating and tasks involving household chores. Your therapy also may include strengthening weak muscles, educating you about energy conservation and accomplishing daily activities using special equipment. 10

14 Speech and Language Pathologist: Speech and language therapists work with people who have problems speaking, thinking and swallowing. These therapists may teach you methods that will help you to communicate with the treatment team and your family. If you have swallowing problems, the therapist works with you to help you eat and drink safely. Psychology: Psychology services are available to help you adjust to changes in your medical condition. Psychological services may be included in your treatment plan if depression, anxiety or another similar condition affects your progress in rehabilitation. Neuropsychological testing and ongoing care may be recommended to assess your memory, reasoning, behavior and decision making skills. Case Manager: Our case managers are responsible for coordinating your care and discharge plan while you are at the CRC. Keeping you involved in your plan of care is an important component of their job. Upon your request, the case manager will include your family or others in your care. Resources in the community which include support groups and other advocacy information are available after discharge and provided by your case manager. Nutrition: A dietician will visit you early in your stay to complete a nutrition assessment. The assessment will include a review of your special nutritional and cultural needs. In conjunction with your medical practitioner, the dietician will recommend the best plan for you. 12

15 Daily Schedule All patients eat meals sitting up in a chair or are encouraged to eat meals in the community dining room. Meals are delivered to the unit at 7:00 a.m., 12:00 p.m., and 5:00 p.m. Orders are placed for breakfast and lunch the day before by 6:30 p.m. Dinner orders can be placed on the same day up until 3:00 p.m. Room Service is available between meals for snacks and other items. All orders can be placed by contacting Nutritional Services at ext Showers are scheduled every third day. More frequent shower requests can be accommodated depending on the patient s ability to participate. Therapies are scheduled on a daily basis, and you will typically be scheduled some time between 7:00 a.m. and 4:00 p.m. If it is part of your care plan and goals, occupational therapy may also see you at 7:30 a.m. for self-care training to include showering/dressing. Your normal schedule will include three hours of therapy at least five days per week. These three hours may include a combination of physical therapy, occupational therapy, and/or speech therapy. You are responsible for following the treatment plan that you assisted in creating. This may include following the instructions of nurses, therapists, and physicians as they carry out the plan of care. Participation in daily therapies is vital to reaching your established goals, and is required unless there is a significant medical issue. You will also be given assignments to work on with your family or caregivers after the formal therapy of the day ends. Our goal is to help you achieve the greatest independence possible for your given situation. 13

16 Safety in the Rehabilitation Environment Part of our program is to teach you and your family the importance of safety. We would like you to have a clear understanding of safety issues by the time you leave the CRC. We ask for your help creating a safe environment for you and other patients. We want to keep you safe in the least restrictive way possible. If you are a potential safety risk, we will look at your environment and start a safety plan. If you have any concerns or questions during your stay, please talk with one of our team members. Protecting you from falls Rehab programs encourage you to increase your independence. Research shows that most falls happen in the first two days of admission and the last two days before discharge. We want you to be safe. It is important for you to call for help when you are moving around in your room or in community areas. The rehab team will let you know when it is safe to move around on your own. Medication We want to give you the right medicine. To make sure we do, your nurse will look for three things before giving you your medicine: The nurse will ask your name. The nurse will check your ID band. The nurse will ask your birth date. 14

18 Important Information for You, Your Family & Visitors Patient Amenities Self pay hairdresser services are available by appointment on Saturdays Available washer/dryer and supplies on each floor Computer kiosk for web access and education opportunities Wireless internet access throughout the Hospital Patient and family lounge Community room containing leisure activities Visiting Hours Patient and family centered care hours are 24/7, however, you will be scheduled in patient therapy from 7a.m. - 4 p.m. and visitors should only be immediate family/caregivers needed for training during those hours. They should also be aware of your need for rest so that you achieve optimal participation and results from therapy. All others may visit from 4 p.m. - 8:30 p.m. Other arrangements may be made upon request. 22 Spiritual Care provided by our Pastoral Care Professionals The Pastoral Care Department provides spiritual and emotional care and support to patients and their families. We care deeply about your spiritual, emotional and social well-being. A chaplain is available 24 hours a day to assist any patient or visitor of any denomination. The Pastoral Care office may be reached from 8 a.m. - 4:30 p.m., Monday though Friday at (847) , ext After hours, or on weekends and holidays, dial the switchboard (0) and ask the operator to page the chaplain on call. If, at the time of admission, you registered your religious affiliation and church name, this information will be available for your priest, minister or rabbi when he/she comes to visit. Chaplains routinely visit all patients, but feel free to ask your nurse to contact the on-call chaplain at any time. Prayers and Sacraments (Eucharist, Reconciliation and Sacrament of the Sick) are offered. Monday through Friday the Rosary is recited at 11:30 a.m. and Mass is celebrated at 12:00 p.m. in the Hospital Chapel and is televised on Channel 3. Scheduled Mass times are announced daily on the Hospital s public address system. The Hospital Chapel is located on the main floor and is open 24 hours per day for meditation and prayers. A non-denominational prayer room is also located on the first floor of the hospital near the chapel. 20

Next Steps After discharging from the CRC, patients may require further care which may include short-term, sub-acute rehabilitation services, home care, medical equipment needs, private duty nursing, or outpatient rehabilitation services. You and your treatment team will decide your specific plan of care. It s nice to know that whatever steps you need to take to return to your highest quality of life, the Presence Saint Joseph Hospital Comprehensive Rehabilitation Center will be by your side every step of the way. We can care for you at every stage of your rehabilitative care journey.
