2 2 Laparoscopic cholecystectomy
3 Introduction This booklet has details about the care which you will receive from staff and the services which the hospital provides. During your stay with us, our primary concern will be your comfort and return to good health. If you have any questions or concerns, our staff will be pleased to assist you in any way they can. For this procedure you will be admitted as a day case to the day surgery unit on level 3 East. Please ensure you have an escort to take you home that evening and a responsible adult to stay with you overnight and for the first 24 hours. A small number of patients require to stay overnight. If the surgeon makes this decision, please be appropriately prepared for this. Visitors Although visiting is not permitted in the day unit, the patient will be taken to the ward straight from theatre, where relatives will be able to visit. Please check the visiting hours with the ward, most wards have an open visiting policy. If your visitors have an infection, cold, vomiting or diarrhoea ask them to get in touch with the ward staff before they arrive. Admissions The booking office staff will make admission plans with you and will make this as easy as possible. Laparoscopic cholecystectomy 3
4 You may have been asked some personal details before you arrive which we may need to ask you again at admission. These may include contact numbers, your GP s name, address and telephone number and other relevant information. If at any time during your pre-operative assessment or any other medical assessments you would like to be chaperoned, please tell a member of nursing staff. Personal items Please bring nightwear, dressing gown, slippers and toiletries. Towels are provided in the ward. Valuables We do not take responsibility for the loss or damage of your valuables or personal items during your stay. Please leave valuables at home or ask whoever is accompanying you to take responsibility for them. If you must bring money or valuables into the hospital, please ask the nurse if she can store them for you. Electrical appliances We allow small electrical appliances in patient rooms although engineering staff must check these before you use them. If you need a hairdryer please ask nursing staff. Be aware these may be in use by other patients. Medication Please bring your medication in with you, the nurse will store this until your discharge when it will be returned to you. 4 Laparoscopic cholecystectomy
5 Infection control Patients who come into hospital have a greater risk of picking up infection because any kind of treatment can make them less resistant for a short period of time. Please ask your visitors to use the hand hygiene stations before entering your room and avoid sitting on your bed. The following points should help reduce the risk to you: Avoid touching your wound and any dressings or drips that have been used as part of your care. Good basic hygiene is also important. Members of staff who need to touch your skin should wash their hands before and after examination (using soap and water or hand gel). All our rules demand this essential procedure, and we would welcome you to question members of staff if you would like them to confirm that they have washed their hands. It is not possible to avoid infection completely but the staff at the Golden Jubilee National Hospital work hard to reduce the risks with your help. Data protection We ask for information about you so that you can receive proper care and treatment. We keep this information, together with details of your care, because it may be needed if we see you again. We may use some of this information for other reasons: for example, to help us protect the health of the public generally and to see that the NHS runs efficiently, plans for the future, trains its staff, pays its bills and can account for its actions. 6 Your information is treated confidentially at all times and is only seen by those who need to know. Sometimes we will share your information with others involved in your care to ensure continuation of your treatment. This may include your local social work department who are responsible for providing any equipment you will need for returning home. Like everyone working for the NHS they have a legal duty to keep information about you confidential. You have a right to see what has been written in your health records; details of how to go about accessing your records and how we use your information are available from the hospital. Spiritual Care Chaplains from the main religious denominations can visit you on request. Please ask a member of staff if you would like this to be arranged. Shopping facilities You can buy newspapers, magazines, phone cards, toiletries and gifts from the shop on the ground floor next to the West Lifts. Open from: 8:00am 7:00pm (Monday Friday) 9:00am 2:00pm (Saturday and Sunday) Shop staff visit wards between 10.30am 11am and 2.15pm 2.45pm with a selection of items from the shop. Housekeeping Our hospital rooms are cleaned daily. This is checked by ward staff and the Housekeeping Supervisor. The Senior Infection Control manager runs the housekeeping service. 6 Laparoscopic cholecystectomy
7 Laparoscopic cholecystectomy Cholecystectomy is removal of your gallbladder. The gallbladder is a small pouch in the upper right part of your abdomen. It stores bile produced by the liver and this helps to break down fatty foods. Laparoscopic cholecystectomy is keyhole surgery to remove your gallbladder and this can be done as a day case. Open cholecystectomy is when a larger incision has to be made to remove the gallbladder and the surgeon will discuss both of these methods with you. It is not possible to predict which method is best until in theatre and for this reason you will be asked to consent to both. Pre assessment clinic You will be asked to attend for pre-assessment to ensure that you are fit for the surgery and anaesthetic. This will be at your own local hospital or here at the Golden Jubilee National Hospital. You will be asked about your past medical history. Please bring all the tablets you are taking. The nurse will take a blood sample and you will be asked to provide a sample of urine. If you are unable to attend this assessment please let us know. Preparation for surgery You will be introduced to your nurse who will discuss your care with you. You will be admitted by the doctor and will be seen by the anaesthetist prior to going to theatre. If a pre-med (tablets) is prescribed these will be given to you by the nurse. The surgeon may have to mark the area with ink. He/she will discuss the procedure with you before asking you to sign the relevant form. Laparoscopic cholecystectomy 7
8 Recovery If you have had laparoscopic surgery and the surgeon is happy for you to go home the nurse will let you know at what time your discharge is planned for after allowing sufficient time for recovery. If you are staying overnight, observations will continue overnight and the day staff will prepare you for discharge around 10.00am the next day. If you have had open removal of gallbladder you may have to stay for a few days to ensure full recovery. A discharge summary will be given to you to be handed into your GP. You will be advised if there are any sutures to be removed. These will be removed by your GP s practice nurse. A date for the removal of the sutures will be given to you before discharge. Discharge advice Rest this evening and avoid strenuous activities. In the next 24 hours, you must not: drive; drink alcohol; or use machinery. Take your usual medications and any that you may have been given by the nurse today. It is normal to feel some pain after surgery. You will be prescribed and given regular painkillers during your stay in hospital. On discharge from hospital you will be given painkillers to take home. 8 Laparoscopic cholecystectomy
9 Other advice If a follow up appointment is required, you will be advised of this on discharge. Contact the hospital immediately after discharge for any advice. Contact your GP if: You become breathless. You have severe pain not relieved by medication. You have excess bleeding from the wound site. You have extreme redness or swelling around the wound site or drainage of pus. You are unable to pass urine. You have continual vomiting. You have a fever. In the event of an emergency, you should go to your nearest Accident and Emergency Department. Specific advice for discharge Mobility You will feel tired for the first few days following discharge from hospital. Rest for some of the time, but gradually increase your mobilisation until you feel fit to resume normal activities. This will take a few weeks. You should not go to your bed as if you were ill. Avoid heavy lifting for six weeks following surgery. It is advisable to wear your support stockings during the day until your mobility has increased to near normal levels. 10 Wounds You will normally have four small wounds on your abdomen. Treat them carefully, as you would any cut. You can have a shower the day after your operation if you wish. Change the dressings when they get wet. You will be given some dressings to take home with you. The stitches that are used are normally dissolvable. You may have some paper stitches over your wounds. These should be left in place for seven days. If the stitches are not dissolvable and are not of the paper type, you will be given a date to attend your practice nurse to have them removed. You may experience some bruising around the wound sites. This will gradually settle. Driving You may return to driving a vehicle when you are comfortable to apply the brakes and perform emergency stops without pain. Check with your car insurance company. Pain relief Continue to take the painkillers given to you on discharge. Returning to work You will normally need six weeks off work depending on how well you recover from your surgery and on what job you do. You may require longer if you carry out heavy physical work. 10 Laparoscopic cholecystectomy
11 Day surgery patients You may feel drowsy or weak for several hours after your surgery. It may take 24 hours for the effects of a general anaesthetic to wear off completely. On discharge from hospital: A competent adult should stay with you overnight. You should eat and drink normally as able (unless otherwise advised). Common side effects are: dizziness; drowsiness; nausea/vomiting; sore throat; and general aches and pains.
