1 Congratulations on taking the first step in joining Weight Watchers. The following document will provide you directions on how to finalize your enrollment on the Weight Watchers website. Fee: The monthly fee that WW will charge your account is $ Reimbursement: If you log onto the WW website once a week for 16 consecutive weeks, the University will provide you an incentive of half of your enrollment which is $ Once you have completed 16 consecutive weeks, print the progress report (directions attached), and send this info to Well U at well-u-info@rochester.edu. Include Weight Watchers Online Reimbursement in the subject line. Once we review your information, we will load your incentive. Please let us know if you have additional questions. 2 Weight Watchers Online Subscription Now that you have enrolled in the Well-U Weight Watchers Online Subscription you will need to register through the Weight Watchers Registration Portal. Please refer to the steps below to assist you through this process: Getting started: 1. Access the Weight Watchers Health Solutions Website, link 2. Under the First Time Users Option you will enter your Company ID and Company Passcode, which are listed below U of R Company ID : U of R Company Passcode: WW Enter your personal information under Personalize your Account 4. Enter the following information under Workplace Info Employee Type: Employee Workplace Address: University of Rochester State: NY Zip code: Select the Follow the Plan Online 6. Review your Plan Information and Complete any incomplete information 7. Under the next page you will Create your account Login 8. Enter Payment and your Contact Information 9. Review and Accept Subscription Agreement 10. Complete Sign Up 11. Congratulations you are an official member of the Weight Watchers Online Subscription, let the journey begin! 12. Once you have completed your 16 weeks of consecutively logging onto the web site follow the PDF file directions on WW online progress reports how to. 13. 3 On Line Subscription FAQ s 1. How do I keep my current account history if I cancel and re-subscribe for Monthly Pass? If a member wishes to cancel their current subscription and sign up for a new subscription in order to receive the University of Rochester special pricing and reimbursement, they can transfer their existing account history to their new account so they will not lose previously recorded data. The first step is to purchase Monthly Pass, which employees can do directly from the Weight Watchers portal. Note that in order for employees to be eligible for the University of Rochester special pricing and reimbursement, their previous account must be cancelled. Please instruct all members who need to cancel and re-sign up through the Weight Watchers portal that they must cancel their membership before signing up in the Weight Watchers portal in order to be able to keep their account information. Please follow the steps below to restart Monthly Pass directly from the Weight Watchers portal: 1. Visit the Weight Watchers portal at: join.weightwatchers.com 2. Use the Company ID: Passcode: WW Register and express interest in Monthly Pass. 5. Enter your zip code into the box provided and click Submit. 6. Choose either an existing At Work meeting or a community location that interests you, and click to buy Monthly Pass. 7. If you do not see your At Work, please choose any location for Monthly Pass and you will be contacted once an At Work meeting is started. 8. On the next page, click the Returning to Weight Watchers or currently using our Message Boards link to enter your existing user name and password. If you do not log in first, your subscription will not convert properly. If this link is not there, this means you are already logged in and can proceed. 9. Fill out the health screener and any other information required to continue with sign up. 10. Review your payment information, confirm your shipping address and agree to the subscription agreement before proceeding. 11. Click Complete Sign Up. 12. Print your Temporary pass, which you will need to show at your meetings until your Monthly Pass card arrives in the mail. 13. After you have successfully signed up for Monthly Pass, instructions on what to do next will be available for you on the next page. 2. How do I access Weight Watchers Online? Go to: Company ID: 46928, Passcode: WW If you have any problems please call

4 3. How is payment for the Online subscription made? You pay for the Online subscription by inputting your credit card information when you are registering through the Weight Watchers portal. 4. After registering for and purchasing the Online subscription, how can I access it online? Once you have registered for and purchased a Weight Watchers Online subscription, you may access it via a link from your company s website or by going directly to WeightWatchers.com and logging in. In all cases, once you log on to WeightWatchers.com, you will be recognized as a returning registered Online subscriber. The easiest way to continue to access WeightWatchers.com is to save it as a favorite within your browser. 5. How do I get help for a Weight Watchers Online subscription? Help is available in the upper right-hand corner of every page on WeightWatchers.com. Using this help button, you can your specific question to one of seven areas of inquiry: subscription process, tools and features, technical support, login help, start weight changes, cancellations, and Weight Watchers Onthe-Go. The typical response time is within 24 hours. 6. How do I cancel my Online subscription? Log on to your account, scroll to the bottom of the page, and click the contact us link. Choose cancel my account and follow the instructions.

13 Step 9: Print your Account Status and submit with completed reimbursement form and Monthly Summary to obtain reimbursement
