2 ADDENDUM 2 PROPOSAL CLOSING DATE EXTENSION: The Proposal Closing Date has been extended to October 28, 2005, 2PM CST. RFP QUESTION & ANSWERS: Michael Aultman - IBM Global Services 1. Page 5 under in the Deliverables section, 3rd bullet - Will the vendor who wins the bid be expected to perform a "disaster impact analysis and application impact plan" or will we be using the results from a study performed by others. UTMDACC will be using the results from a study performed by Business Continuity Planning Department. 2. Page 5 under in the Deliverables section, 9th bullet - When the vendor identifies a tool, When will UTMDACC make a purchase decision? - A decision on the tool will have a material affect on the vendor's ability to deliver the plan The vendor will be expected to gather requirements, develop an RFP for the tool selection. There will be no time frame for making a purchase decision. 3. Page 5 under in the Deliverables section, 11th bullet - How many tests? How many systems? Out of the estimated 50 critical applications/systems, we expect to complete approximately 20 system recovery plans during this period and anticipate conducting 2-5 applications/systems tests. 4. Page 6 under in the Deliverables section, 16th bullet - Should we assume that since we are supposed to 'deliver recovery plans' that we are developing plans? If so, how many? One for the data center? One for each platform type? One for each RTO? The vendor will be expected to develop one overall institutional Disaster Recovery Program plan, approximately 20 mission critical system plans, and a tiered recovery approach/guideline set for 5. Page 5 under in the Deliverables section, 17th bullet - Does UTMDACC have a feel for how many processes they want developed or we make an assumption of number of processes needed. Please see #4 for the number of plans to be developed. 6. Is it possible to get a list of equipment in place today? Mainframe(s)? How much storage? How many DEC Systems? Open Systems? IS UTMDACC looking to have plans developed for desktop computers? If so, how many are there? Please refer to the Scope of Work, Section 3 for Information Technology Infrastructure regarding software, hardware and operating systems, network, servers, and desktop. More detailed information will be made available upon onset of this project.

Section 3, Information Technology Infrastructure, Software, Page 2: Other (non-sms) mainframe and/or client-server based applications support critical functions in Diagnostic Imaging, Laboratory Medicine & Pathology, Surgery, Anesthesiology & Critical Care, Research, Human Resources, Payroll, Financial Services, Facilities Management, and many other areas of the Institution. There is an estimated 50 critical applications that have been identified across the institution. M. D. Anderson has an established co-location facility located in the northwest side of Houston. There are several departments listed. At the end of the paragraph, many other areas of the Institution, what are the other areas? Are there other user departments under the departments listed? For example, in Financial Services, are there other departments under this area? Please list the approximate number of additional users. The business process plans are currently being developed by the Business Continuity Planning Department. Deliverables for this RFP should include one overall institutional Disaster Recovery Program plan, 20 mission critical system plans, and a tiered recovery approach/guideline set for 2. Section 6, Page 5, third paragraph: States Vendor acknowledges that responses not received by October 20, 2005, 2PM CST at the destination listed on the cover page of this RFP will not be considered. Section 7, RFP Timeline, shows the closing date to be October 21, Which of these dates is correct? Since the answers to the questions have been postponed to October 14, 2005, we will accept the responses received by October 28, 2005, 2:00 PM. 3. Section 8, Page 1: Rider 104 states If subcontracting opportunities are probable, each state agency s invitation for bids or other purchase solicitation documents for construction, professional services, other services, and commodities with an expected value of $100,000 or more shall state that probability and require a HUB Subcontracting Plan (HSP). Does this information apply if the price quoted for this RFP is under $100,000.00? The above information does not apply if the price quoted for this RFP is under $100,
4 Jill Almaguer - SAC 1. Have all the key stakeholders been identified? If so, how many are there and what departments do they represent? Currently all the key stakeholders have been identified, which include, but not limited to, Information Security, Risk Management-Disaster Recovery, Central Information Systems, Business Continuity Program Office, Institutional Compliance Office, and Data Center Operations. 2. When is the maintenance window when testing can be done? For example, another institution we work with in the Texas Medical Center has a maintenance window every other Wednesday for 4 hours overnight. The maintenance window for testing is Saturday night & Sunday morning (i.e. 6 PM 6 AM given conditions) 3. Is training delivery in the scope of this project? If so, how many people will need to be trained? Although there is not a formalized classroom training required, the walk-through of the completed plans will serve as an integral part of the awareness process.. 4. How many system administrators will be involved in this project? Approx. 20 system administrators will be involved in this project. 5. How many working groups and management committees will be involved in this project? There are 5 working groups and estimated 2 management committees will be involved in this project.

MD Anderson is eligible to receive GSA pricing on selected items. 2. In section 6 of the RFP, last paragraph, the RFP states the closing date as Oct 20th and the table below says October 21st. Which is the correct date? The correct date is now October 28, 2005, 2PM CST 3. Please provide a listing of departments, which are candidates for interviews/surveys as part of the disaster recovery application impact analysis. The vendor will be expected to develop one overall institutional Disaster Recovery Program plan, approximately 20 mission critical system plans, and a tiered recovery approach/guideline set for 4. Please provide a more detailed listing of the IT infrastructure for hardware and enterprise servers. Please provide a network diagram. Please refer to the Scope of Work, Section 3 for Information Technology Infrastructure regarding software, hardware and operating systems, network, servers, and desktop.
