2 Effective immediately, the following will be accomplished by all contractors before starting any work or excavation around or on any sewer line owned by the North Davis Sewer District: Hours of Operation a) b) c) Sewer Lateral Connections: a) b) c) d) e) The District's working hours are 7:30 a.m. to 4:00 p.m., Monday through Friday. No sewer inspection will be done on weekends, holidays or after 3:00 p.m. All inspections lasting after 4:00 p.m. will result in overtime being billed to the contractor. Multiple inspection visits required for work that is incomplete, not to District standards, or not ready for inspection will result in additional time being billed to the contractor. No work shall begin until the proper fees of $ for connection and inspection, plus impact fees as calculated by the City or District (whichever may apply) are certified to the District as having been paid. The District only accepts cash or check. No work shall begin until the proper building permits and licenses have been obtained from the City, County, or Sewer District. Minimum notice of 48 hours shall be given to the District prior to any work commencing on or digging being done on District-owned lines. No digging or work shall be done on District-owned lines without the presence of a District Inspector. All sewer taps must be Core Drilled, INSERTA TEE, GPK 4, or equivalent. Setting of Manholes on District owned-line a) Manholes are required to be set for connecting lines 6 inches or larger. b) No manholes shall be set on District-owned lines without prior approval by the District. Contractors shall submit design drawings and manhole details of proposed manholes on District line for approval 14 days prior to work commencing. Design drawings shall be submitted in PDF electronic format unless otherwise requested by the District. c) After approval of drawings, a minimum of 48 hours notice shall be given to the District inspectors before any manhole is set on a District-owned line. The manhole must be set in the presence of a District inspector. Flow Control and Bypass Pumping Operations a) Where required to complete the Work, the Contractor shall provide bypass pumping and flow control. Contractor shall submit a bypass pumping plan prior to commencement of the work for review and acceptance by the District. The plan shall at a minimum provide materials, equipment and flow calculations and be of sufficient detail to demonstrate the system's capacity to control flows. The Contractor shall coordinate with the District to determine the required capacity. Concrete Batching a) Ready mix concrete shall be provided in accordance with ASTM C94, Standard Specification for Ready-Mixed Concrete. b) The design water content of the mix (w/c ratio) shall not be exceeded. Addition of water, air entrainment or superplasticizer shall only be performed by the Supplier. c) Non-air entrained concrete shall be discharged ninety (90) minutes after batching. Air entrained concrete or concrete that includes silica fume shall be discharged within sixty (60) minutes of batching. d) Requests for longer batching times shall be submitted and accepted by the District prior to procurement of concrete by the Contractor. e) The District shall not be responsible for the cost of rejected batches exceeding time limitations, loss of air-entrainment or slump. Warranty: a) Contractor shall guarantee work for a period of one year from the date of installation. Failure of contractors to comply with these guidelines could result in the contractor being barred from working on District lines. If there are any questions on a line being a District line, please contact the District ND-01 Notice to Contractors

3 STANDARD MANHOLE ON EXISTING SEWER LINE 1. Manhole shall be constructed so as to prevent load to the existing pipe. The base of the manhole shall provide a minimum of 9 inches of concrete beneath the existing pipe unless otherwise shown, placed on 12 inches of one inch gravel bedding compacted to 95% density. 2. Manhole floor elevation shall be a minimum of 2 inches above top of pipe, have a 12:1 slope to the pipe, and a smooth finish. 3. Connection to manhole shall be made using a flexible, watertight connection per ASTM C923, Kor-n-seal or equal. Flexible couplings shall be correctly sized, flexible water tight couplings per ASTM C1173, Fernco Inc, Mission Rubber Company or approved equal. All hardware shall be 316 series stainless steel, unless otherwise approved. 4. Manhole shall be constructed using a pre-cast eccentric type sections and cast iron traffic type frame and cover marked "NORTH DAVIS SEWER". Section joints shall be sealed with rubber gaskets and shall be watertight. 5. Wrap exterior joints. Wrap shall conform to ASTM C877 Type II or Type III. Press-seal Ez-Wrap, Cretex Wrap, Conseal 2/2 or approved equal. 6. Invert of branch line at manhole connection shall be adjusted to be at or near crown of existing main line. 7. Branch line channel to be formed with same slope, size, depth, and shape as pipe with smooth finish; it shall be curved in direction of flow with uniform radius and minimize dropping and splashing of flow. 8. Cutting into existing line shall be made by smooth saw cuts. 9. Installing contractor shall be responsible for proper slope and installation of connecting line. 10. Installing contractor shall be responsible for proper backfilling, compacting, and pavement restoration. 11. Contractor shall notify District 48 hours before installation begins and a District inspector must be present before installation takes place. 12. Contractor shall meet all restoration requirements of the agency, city or UDOT where the manhole is installed and shall coordinate any additional installation or inspection requirements with the agency. CONNECTION TO AN EXISTING MANHOLE 1. Item numbers 3, 5, 8, 9, 10 and 11 above shall apply. 2. In addition, the manhole junction channel shall be constructed to the same size, shape, and slope of the incoming line. Channel shall curve in the direction of flow with uniform radius and minimize dropping and splashing of flow. 3. No manhole connection shall be made until complete design drawings of the manhole and line installations are submitted to the District for approval - 14 days prior to work commencing.
