2 THE APPLICATION PROCESS USER INFORMATION The information provided below is intended as a step by step guide through the process that an application for an entertainments licence is subject to before being approved or refused. 1. The application must be completed in duplicate. 2. The applicant for a private business may sign the application form. In the case of a Limited Company application form must be signed by the Company Secretary. In the case of a Club the application form must be signed by the Club Secretary. 3. When received, the application form with accompanying fee (see schedule of fees below) is recorded, receipted and checked. 4. The following bodies are provided with a copy of the application and asked for comment:- - The Area Commander, Southern Area Command, Northern Ireland Fire Authority, Thomas Street, Portadown, Craigavon BT62 3AH 5. A recommendation for approval of the application is subject to the following:- - no objection being received within the statutory 28 days allowed for objections - the receipt of satisfactory replies from the above mentioned body - the receipt of satisfactory certificates supplied, where appropriate, in support of the application - the receipt of satisfactory reports from inspecting Council officers - the applicant foregoing his right to appear before and be heard by Council 6. Premises inspected by Council officer to ensure compliance with requirements 7. Licence issued following satisfactory confirmation of all above criteria. 2

5 CONDITIONS OF APPLICATION The following Certifications are required in support of your application for a Cinema Licence: - 1. Electrical Installations Certificates in respect of the following:- (i) Periodic Inspection Report for an Electrical Installation together with the Schedule of Test Results. (Supplied by Electrical Engineer) Have you enclosed this certificate/report? NOTE: Yes No All Periodic Inspection and Testing shall be carried out in accordance with the current edition of the IEE Regulations for Electrical Installations, Inspection and Testing as recommended in BS 7671, with the maximum period between inspections and testing being 3 years, this does not exclude the requirement to carry out an annual routine check. (ii) Emergency Lighting Periodic Inspection and Testing Certificate (Supplied by Electrical Engineer) Have you enclosed this certificate? NOTE: Yes No All emergency lighting systems shall be inspected and tested annually and carried out in accordance with the current edition of BS 5266 Part 1, 1999, Annual Test. Additional Information Certificates from an approved competent electrical engineer, stating that the electrical installations on site have been examined and tested and found to be in a satisfactory condition. Current membership of the following bodies is considered as an acceptable qualification: Institute of Electrical Engineers (IEE) National Inspection Council for Electrical Installation Contracting (NICEIC) Electrical Contractors Association (ECA) or Electrical Contractors Association of Scotland 2. Fire Alarm and/or Fire Detection Test Certificate Have you enclosed this certificate? Refer to page 7 Yes No NOTE: All Fire Alarm systems shall be inspected and tested annually and carried out in accordance with the recommendations of Clause 45 of BS 5839, Part 1, 2002, incorporating Amendments 1,2,3, with particular reference to Section 4, User Responsibilities Annual Inspection and Test. 5

6 3. Fire Fighting Equipment a certificate confirming that all fire fighting equipment within the premises is in good working condition and suitable for the purpose for which it is intended. The Council will notify you if such certificates are necessary and they must be supplied by competent persons. For the initial grant of a licence or following structural alterations, three copies of a plan drawn to scale of not less than 1:50 are required showing the premises concerned. 6
