3 Conference Purpose & Objectives The purpose of the conference is to provide state-of-the-art advance care planning and end-of-life communication from a clinical, ethical, educational, research, legal, and policy perspective. Identify key elements in developing evidence-based advance care planning programs. Describe cutting-edge, innovative strategies that improve advance care planning and end-of-life communication. Discuss sound evaluation research methods to examine the impact of advance care planning.
