3 Annual 2014 Conference: Multidisciplinary Approaches Across the Palliative Care Continuum Saturday March 22, 2014 Historic Academy of Medicine Atlanta, GA Keynote Speaker: Linda Emanuel, MD, PHD The Quiet Revolution of Palliative Care Dr. Linda Emanuel currently serves as the Buehler Professor of Geriatric Medicine and director of the Buehler Center on Aging, Health & Society at Northwestern University s Feinberg School of Medicine and as an adjunct professor at the Kellogg School of Management. She is the founder and principal of the Education in Palliative and End-of-life Care (EPEC) Project ( its adaptations (e.g. EPEC-Emergency Medicine (EPEC-EM)), and the Patient Safety Education Project (PSEP) ( Dr. Emanuel practiced medicine at the Massachusetts General Hospital for twelve years before moving to Northwestern University, where she passed the written boards in palliative medicine in Dr. Emanuel serves as the principal investigator of multiple NIH and foundation grants, including one funded by the American Cancer Society that assessed the impact of a comprehensive palliative care assessment and care implementation guide. She has published and lectured extensively on clinical ethics, end-of-life care, the patient-physician relationship, academic integrity, accountability, organizational ethics, and professionalism. Among her many accomplishments, Dr. Emanuel has served on the National Commission on Scientific Integrity, has chaired the National Institutes of Health s Ethical, Legal, and Social Implications Study Section, and was the Founder of the American Medical Association Ethical Force Program. Dr. Emanuel has served on numerous advisory and editorial boards for national and international agencies and journals. Dr. Emanuel received her PhD from the University College in London, England and her MB from the University of Oxford, also in England. Dr. Emanuel received her MD from the Harvard Medical School in Boston, Massachusetts. Highlighted Speaker: James Mittelberger, MD, MPH, CMD, FACP The ROI of Palliative Care (Better Care, No Increasing Cost) Dr. James Mittelberger is the Chief Medical Officer for Evercare Hospice and Palliative Care and Senior National Medical Director of Optum (UnitedHealth Group). Dr. Mittelberger is the current President of the California Association of Long Term Care Medicine and has served in several leadership capacities in Long Term Care, Palliative Care, and Geriatric Medicine, to include Chief of the Division of Geriatrics and Palliative Care, Alameda County Medical Center in Oakland, CA for nearly two decades. Dr. Mittelberger has presented extensively on the topics of palliative medicine,

4 POLST, and interdisciplinary teams. Dr. Mittelberger is board certified in Internal Medicine, Geriatric Medicine, and Palliative and Hospice Medicine. He is a Certified Medical Director in long term care. Dr. Mittelberger obtained his BA in biology at Brown University and received his MD from the University of California, San Diego School of Medicine. He completed his residency in Internal Medicine at the University Of Washington School Of Public Health, where he also completed his Master s in Public Health. Breakout Session 1 Speakers: Debbie A. Gunter RN, MN, FNP, ACHPN: Managing Distressing Symptoms Near the End of Life Debbie Gunter is a certified Family Nurse Practitioner and a certified Hospice and Palliative Care Nurse Practitioner with over thirty years of nursing experience. She received her BSN from Florida State University, her Masters Degree in Adult Health-Critical Care from Emory University, and her Family Nurse Practitioner certification from the University of Missouri. Debbie is on the faculty at Emory University in the School of Medicine and the Nell Hodgson Woodruff School of Nursing and was honored with the Nurse Practitioner State Award for Clinical Excellence from the American Academy of Nurse Practitioners in Ms. Gunter is well known for her dynamic and innovative teaching style and her ability to present challenging concepts in a form that is interesting and easy to understand. She has presented over 700 nursing seminars in the critical care, medical surgical,geriatric, and palliative care nursing fields across the country. Mary Margaret Oliver, JD: Guardianship in Georgia A native Georgian from DeKalb County, Mary Margaret Oliver currently represents the 82nd District in the Georgia House of Representatives. Rep. Oliver maintains an active leadership role in the GA House and sits on the House Appropriations, Judiciary, Governmental Affairs, Juvenile Justice, and Science and Technology Committees. She has been appointed by the Georgia Supreme Court to the Committee on Justice for Children and by Governor Nathan Deal to the Special Council on Criminal Justice Reform. She earned a BA from Vanderbilt University and a law degree from Emory University. From 2001 to 2009, she served as visiting professor at the Barton Child Law and Policy Center at the Emory University School of Law and was one of 30 women named as an Emory History Maker during Emory s 175th anniversary year. Breakout Session 2 Speakers: David Vance, PhD, MS, MGS: HIV and Successful Aging? They are More Compatible Than you Think! Dr. David Vance is an Associate Professor in the University of Alabama-Birmingham School of Nursing and is currently engaged in cutting edge research on the cognitive effects of aging with HIV. Vance says that while aging with HIV is most definitely a welcome trend, cognitive problems may emerge which interfere with successfully aging with this disease and that it is essential that researchers work to ameliorate negative cognitive aging effects in this growing population. He is committed to research and education which may increase the quality of life for persons aging with HIV and to supporting the spiritual and mental health of those individuals.dr. Vance received a Master s in Gerontological Studies from Miami University of Ohio, a Master s in Applied Developmental Psychology from the University of

5 New Orleans, and completed his Ph.D. in Developmental Psychology at the University of Alabama- Birmingham. Dr. Vance completed 3 years of an NIA postdoctoral fellowship studying cognitive aging and cognitive remediation therapy. He is the Principal Investigator on two NIH grants investigating aging; has over 120 peer-reviewed publications, book chapters, and editorials; and has presented his research in local, national, and international venues. Richard Cohen, MD: Next Steps for POLST in Georgia Dr. Cohen is the current chair of the Georgia Physician Orders for Life Sustaining Treatment (POLST) collaborative and board chair for Georgia Health Decisions. He is a board certified orthopedic surgeon and has served as medical director for Wellstar Kennestone Hospital. He is the former chair of the ethics committee and was the founder of the WellSar Health System total Joint Center of Excellence. Dr. Cohen was also vice chair of the board at the United States anti-doping agency and is currently a member of the National POLST Paradigm Task Force. Dr. Cohen Received completed his undergraduate education at Washington and Lee University and attained his MD from the Jefferson Medical College in Philadelphia, PA. Dr. Cohen completed his orthopaedic residency at the University of Pennsylvania and is currently boarded by the American Board of Orthopaedic Surgery. Closing Speaker: Dr. Tammie E. Quest, MD Ensuring Communication of Healthcare Wishes: Dr. Tammie Quest is the Director of the Emory Palliative Care Center for Emory s Woodruff Health Sciences Center. She is Associate Professor in the Department of Emergency Medicine and in the Division of General Medicine and Geriatrics within the Emory University School of Medicine. Dr. Quest is board certified in Emergency Medicine and Hospice and Palliative Medicine and is a Project on Death in America Faculty Scholar. Currently, Dr. Quest serves as the Chief of the Section of Palliative Medicine at the Atlanta VA Medical Center and is the former director of the Grady Cancer Center for Excellence Palliative Care Oncology Program. Dr. Quest is the Fellowship Program Director for the Emory University School of Medicine program in Hospice and Palliative Medicine and holds a strong interest in novel palliative care curriculum design and teaching methodologies. She is a nationally recognized expert in palliative care in the emergency setting and is the Director of the NCI sponsored Education in Palliative and End of Life Care Emergency Medicine (EPEC-EM) Project. She serves as a member of the National Priorities Partnership s Palliative and End-of-life Care work group. With a strong interest in ethics and end of life care, she holds a core faculty appointment at the Emory University Center for Ethics and she is the immediate past-chair of the Ethics Committee for the Society of Academic Emergency Medicine and a member of the ethics committee for the American College of Emergency Physicians. Dr. Quest received her MD from the University of California, San Francisco and completed her residency at the Alameda County Medical Center in Oakland, CA.
