3 This Consensus service provides eight bright and contemporary styled en suite bedrooms. There are a number of communal areas including a large kitchen, dining room, two lounges and a computer room. Individuals using the service are provided with opportunities to embark in new interests should they wish to do so. Activities such as swimming, going to the cinema, bowling, farm visits, museum excursions and paid or voluntary employment are actively participated by those using the service. Each individual is also supported to plan and enjoy a minimum of one holiday per year of their choice. The main aim for staff at King Edwards Grove is to offer support and guidance for individuals with learning disabilities. People using the service are encouraged by staff to enjoy life and live it to the full in a safe, happy and fulfilling environment. Appropriate person centred support plans are developed in consultation with each individual to reflect their needs and aspirations. Our aim is to increase confidence and nurture independence for all individuals we support by keeping them informed of their choices, responsibilities and rights within both the local and wider community. A structured weekly programme is generated by agreement with each individual; a weekly activity plan is developed with an individual s key worker or team leader. Our aim is to provide individual care, promote personal development and foster a valued lifestyle within a comfortable homely environment. We believe by creating a supportive environment, all individuals with a learning disability have the ability to meet their full potential, live in the community and experience valued lives. Norwich Road Norwich Road provides accommodation and support for 6 adults with learning disabilities, autistic spectrum conditions, complex needs and physical disabilities in a supported living setting. Accommodation is provided in six separate apartments, each with one bedroom. Two of the apartments are wheelchair accessible. 5 Station Road offers two registered flats supporting three people and eight fully accessible bedrooms for individuals with complex physical needs. The service supports individuals nursing needs within a homely environment where individuals can develop their independence and control. The bedrooms all have large en-suite wet rooms. There is an assisted bathroom, a large communal living area, dining room, an accessible kitchen and a separate quiet/living room. This Consensus service, situated in the village of Burton Latimer is within easy walking access to shops, pubs, a gym, banks, cafes and parks. The bus route stops just outside the service providing convenient links to nearby towns. Phoenix House This new service provides seven, one bedroom flats within a single property offering a sheltered environment for individuals who prefer to live on their own but need 24 hour support. Each flat has its own bedroom, lounge, kitchen and bathroom. Links Day Care Centre The service provides day opportunities for adults with learning disabilities who may also have behaviours that challenge. Individuals can come and meet new friends, gain new skills and participate in a wide range of activities including horticultural sessions. The service also encourages individuals to become more independent and confident on public transport and being safe in the community. The two services combined provide domiciliary services for 8 adults with mild to moderate learning disabilities and can cater for those with physical disabilities. All individuals have a tenancy agreement and are supported with daily living activities, local and wider community engagement and the maintenance of their tenancy. Lime Grove Apartments Location: Carmarthen, Carmarthenshire Close to Carmarthen town centre, consisting of four, one bedroom flats offering a sheltered environment for adults with moderate to severe learning disability who prefer to live on their own but need regular support. Ty Hendy - The Laurels Location: Hendy, Carmarthenshire The Laurels at Ty Hendy provides five self contained flat for those who require greater independence and supports those with learning disability, challenging behaviour and mental health needs. Each flat has a large lounge/bedroom area and well equipped kitchen and the service supports individuals towards greater independence. New Horizons Location: Magor, Newport An intensive support centre for individuals with profound and complex learning disabilities, including sensory impairments and additional physical needs. Accommodates up to 12 people who receive a high level of support in accordance with their individual needs, through developmental activities within the service and local community. West End Day Opportunities Location: Magor, Monmouthshire Support for up to 15 adults with learning disabilities in a day setting. Situated in a quiet residential area of Magor, within walking distance of local shops and community facilities. Also suitable for those with physical disabilities. Support is tailored to suit specific needs and encourages the development of daily living skills. Ty Hendy - The Oaks Provides planned short breaks, holidays and emergency admission respite services in the beautiful setting of Carmethenshire, for those with learning disability, physical disabilities, autism and early on-set dementia.
