Our centers give nurses the opportunity to move to the forefront of healthcare, while educating ASU's undergraduate and graduate nursing students and providing access to healthcare in the process. The health centers have an interdisciplinary staff that provides both physical and mental health services. The support that the NMCH's receive from healthcare officials, third party health insurance companies, policy makers, donor agencies, the news media, religious and community leaders, is critical to the sustainability of the clinics. ASU Health Center, located on the first floor of the Nursing & Healthcare Innovation (NHI) building at 500 N. 3rd Street. Students, faculty and staff from any ASU campus, as well as the public, are able to access quality, affordable healthcare while studying or working in the Downtown Phoenix area. Breaking the Cycle Community Health Care, based in central Phoenix, a few blocks north of the Downtown Phoenix campus, provides free or low cost reproductive health services for adolescents and adults who do not have health insurance. Community Health Services Clinic, in Scottsdale, is nationally recognized as one of the first primary healthcare centers in the country to be directed and staffed by nurses. Escalante Health Center, in Tempe located at the Escalante Community Center, serves low income children, their families, and elderly adults who are residents of Tempe. North Tempe Health Center, located at the North Tempe Multi Generational Center aims to promote healthy lifestyles and behaviors, improve quality of life, and provide access to healthcare for uninsured or underinsured individuals, children and families in Tempe. 2 Each year, the centers' staff mentors more than 200 undergraduate and graduate ASU nursing students, providing educational experiences from the exam room to the Board room. Providing healthcare in community settings is cost effective and reduces the use of emergency rooms for non urgent needs. Combined, the centers reach more than 1,000 people each month, creating an environment that encourages community involvement among tomorrow's professional nurses. The centers bring healthcare to the clients in the neighborhood where they live. Centers are located on a university campus, in a church, community centers, and in a retail mall. The centers serve as sites for faculty practice, research and clinical trials. The centers also serve a diverse population that crosses all socio economic levels. The staff are members of insurance networks such as Blue Cross/Blue Shield, Aetna, United Health, RAN & AMN, Schaller Anderson, Arizona Foundation for Medical Care, Medicare, TriCare, APIPA and Cigna. Community Health Mission provides basic medical coverage, for non emergency condition. This program while is basic the details are what counts. Some of these services are provided thanks to partners such as Memorial University Medical Center, St. Joseph's Candler Health Systems, Coastal Health District Chatham County Health Department, Quest Diagnostics, Trident, and local healthcare providers. Heartland Health Outreach (HHO) Heartland Health Outreach, the health care arm of Heartland Alliance, is an established leader in providing affordable, highquality health care to men, women, and children who would otherwise struggle to address basic health needs. Founded as a national Health Care for the Homeless demonstration project in 1985, Heartland Health Outreach's services have since grown significantly. Our services include primary care, mental health, oral health, and substance use treatment and services for low income, homeless, immigrant, and refugee populations throughout the Chicago region. Through services delivered at our health center, outpatient and residential locations, and outreach programs at more than 120 area homeless shelters, Heartland Health Outreach annually serves the health needs of thousands of medically underserved people, filling major gaps in the region's health care system. Heartland International Health Center (HIHC) The mission of Heartland International Health Center (HIHC) is to improve the well being of the communities we serve by providing accessible, high quality healthcare. Last year, our four locations on the north side of Chicago provided over 13,000 patients with a medical home where they could receive comprehensive services including primary care, prenatal care, pediatric care, immunizations, women s health care, chronic disease management, medication discounts, oral health, mental health, and psychiatric services. It is one of the largest providers of community based primary care in Chicago. We provide health care, social services and nutrition education to the medically indigent and uninsured residents of the Near North side (Cabrini Green), West Town, Humboldt Park, West Garfield Park, Austin, Kenwood/Oakland, Douglas and Grand Boulevard, and Uptown communities. Over the years, Near North Health Service Corporation has been called upon to expand in the face of increasingly diverse community needs. Our first clinic was housed within the Olivet Community Center, more than forty years ago. However, the unique approach to health care that we began back then has proven itself to be a strong foundation that has withstood the test of time. Our philosophy reflects an approach that anticipates community needs and plans for expansions that are developed in accordance to these needs. Throughout our continuous evolution, we are happy to report that not only have our methods held up under the intense new demands of ever changing communities, but also we have come through with shining results. 8 Near North Health Service Corporation (NNHSC) targets a primary service area that is spread across many of Chicago's 77 community areas, encompassing a population of more than 1,500,000. A full array of primary care clinical services and comprehensive social support programs are provided through six (6) Near North community based health centers. While disparate, NNHSC communities are characterized by large concentrations of CHA high rise apartments, abandoned residential buildings, blighted commercial properties, and an utter absence of basic services and health care resources. In fiscal year 2008, NNHSC served 47, 506 clinical and non clinical patients through more than 98,508 visits. Almost half of NNHSC users, 51%, are uninsured; 42% have Medicaid coverage, and 7% are covered through Medicare. Females account for 67% of NNHSC patients and males represent 33%. Seventy percent (70%) of NNHSC users live at or below 100% of the Federal Poverty Index (FPI) and another 25% is between 100% to 200% of FPI. By providing a variety of services on a group basis, the Network maximizes cost efficiency for its members and allows them to focus more on direct patient care. The Network provides shared services in five primary areas: clinical quality and disease management, information technology, managed care and joint contracting, community outreach, and joint fundraising. The 14 clinic members of the Network operate 46 clinic locations in the Twin Cities, serving more than 145,000 people annually. These clinics primarily serve low income people more than half without any health insurance who often get left behind by the traditional health care system. To effectively serve such a diverse population, member clinics go beyond traditional medical services to provide transportation, translation, social services and culturally appropriate health care and education. For over 12 years, the Network and its member clinics have played a critical role in reducing health disparities and meeting the evolving needs of increasingly diverse communities in the Twin Cities. We accept people where they are regardless of race, ethnicity, religion, sexual orientation, or social status. We offer services in a welcoming and affirming environment. Our ministry is one of serving others, being present with them. Every month, The Night Ministry serves hundreds of individuals all ages, races, religions, sexual orientations, economic and educational backgrounds. We meet people where they are, on physical, spiritual, emotional, and levels. Our ministry is one of serving. We serve youth and adults who face multiple complex challenges on a daily basis, including homelessness, poverty, abuse, loneliness, and neglect. Many of us who are not on the streets take for granted a lifetime of "helping hands" from family and friends. The people we serve often have not had this help. Our Outreach and Health program provides services at night to youth and adults in neighborhoods where they live. Staff and volunteers work to build non judgmental relationships aimed at empowering individuals to meet their own needs. We try to help those we serve find and use their own strengths and resources. Relationship building requires dependability. Showing up consistently is one of the most important things we do. Our customized Health Outreach Bus travels to six Chicago neighborhoods from 7 11:30 pm, Monday Saturday, on a regular schedule all year. The Bus does not transport visitors to or from any location. With thousands of uninsured individuals in Chicago, The Night Ministry s outreach and health services are important to those who have difficulty accessing traditional health services. Many of our bus visitors find it challenging to seek services elsewhere. The health and well being of patients and community alike are promoted through direct services, training/education, outreach, and advocacy. North Country HealthCare provides extensive primary care services to low income, uninsured, or underinsured people in northern Arizona. As the only publicly supported community health center in the area, our charges are based on a sliding fee scale determined by income and household size. North Country offers full scale outpatient primary healthcare in Flagstaff, Lake Havasu, Ash Fork, Seligman, Grand Canyon, Kingman, Winslow, Holbrook, St. Johns, Round Valley and Show Low. Approximately 90% of Clinic patients earn less than $10,000 a year and 25% are homeless. With Tenderloin hospitalization rates for diabetes and complications associated with the disease five times the city average, the Clinic has responded by becoming a leader in chronic disease care including Diabetes care management and COPD/Asthma care. Through individualized care plans, support groups and education, patients learn to effectively manage their chronic conditions. The Clinic also provides the only free pediatrics care in the Tenderloin, serving 600 children every year. Nearly one in four of our young patients are overweight, and many struggle with high blood pressure. Infant immunizations, developmental evaluations and diet and nutrition counseling put families on track with the preventative and routine check ups needed to ensure healthy futures for their children. The Health Center provides primary care for uninsured, financially eligible adults years of age who live or work in Chatham County. The Nursing Practice Corporation (NPC) is a not for profit corporation whose members hold faculty positions in Wayne State University s College of Nursing. NPC opened a pilot phase of the current Campus Health Center within South Hall, and operated in a first floor student lounge area for 3 semesters, providing solely acute illness care services to students residing in the residence halls. In August 2006 the Campus Health Center moved into two renovated apartments in the Helen DeRoy Apartment Building. This new space allowed expansion of health care services being offered, including travel health, routine health exams, contraception care, and management of chronic illnesses. As the number of students seeking services at the Campus Health Center increased, the clinic expanded again in 2008 and now has a total of seven examination rooms, two counseling rooms, professional offices, and has increased services to include health promotion counseling and mental health counseling services on site.
