2 Mission, Vision and Financial Assistance Policy Mission University of Maryland Rehabilitation & Orthopaedic Institute delivers innovative, high-quality and cost-effective rehabilitation and surgical services to the community and region. We provide: An interdisciplinary continuum of care, including inpatient and outpatient surgery, rehabilitation and additional services as required. A proactive environment for patient safety, implementing improvements as patient safety risks are identified. A site for public and professional health care education and research. Vision UM Rehabilitation & Orthopaedic Institute s vision is to become widely recognized as an integral component of the University of Maryland Medical System in its role as: A regional hospital specializing in the provision of acute, chronic and outpatient rehabilitation services; A regional hospital specializing in the provision of a full array of orthopaedic services for adults and children; A high-quality provider of specialized medical/surgical programs. Financial Assistance Policy Hospital care is available to all patients regardless of their race, color, national origin, age, gender or ability to pay. We recognize the financial needs of our patients and families who are unable to afford charges associated with medical care. Our Financial Assistance Policy follows the federal poverty guidelines and is based on household size and income. We may consider other financial assets and liabilities of the patient and family when determining the ability to pay. The patient is responsible for providing information requested during the qualification process. Bills will continue to arrive until eligibility has been determined. 3 A Message to Our Community Hospitals in Maryland are moving into the field of population health management to improve the health of their patients and local communities. Maryland s new Population Health Reimbursement System signals a fundamental change in the delivery of health care services in our state. The University of Maryland Rehabilitation & Orthopaedic Institute (UM Rehab) is increasingly expanding its services to focus on population health management as a pathway to improve the health of our patients and surrounding community. The ultimate goal of population health management is to promote the overall health of a given population or community while also reducing health disparities, by integrating public health principles into health care delivery. Cynthia A. Kelleher, MBA, MPH With a focus on rehabilitation, UM Rehab meets the unique needs of the most vulnerable post-acute populations through specialized programming. As an example, the adapted sports program provides individuals with physical disabilities the opportunity to participate in adapted recreational and competitive sports, maximizing their independence, self-confidence, health and overall well-being. In 2015, the program provided a community benefit worth nearly $50,000. Other programs serve populations with limited resources and access to care. With a presence at several area high schools, the sports physical program was developed with the goals of reducing obesity and promoting physical activity. In fiscal year 2015, the program provided more than $44,000 in benefit to the community. Anne D. Williams, DNP, RN UM Rehab is also committed to the education and training of future health care professionals through internship programs for several disciplines. Students of physical, occupational and speech therapy, as well as nursing and medicine, participate in a number of hands-on educational programs each year. In fiscal year 2015, the internship program resulted in a community benefit of more than $7 million. In total, UM Rehab contributed more than $9 million to community benefit programs and services, including $877,000 in charity care in fiscal year Thank you for your support of the University of Maryland Rehabilitation & Orthopaedic Institute. 4 Adapted Sports Program The University of Maryland Rehabilitation & Orthopaedic Institute s (UM Rehab) Adapted Sports Program promotes involvement in adapted recreational and competitive sports for people with physical disabilities in order to improve independence, self-confidence, health and overall well-being. People in UM Rehab s Adapted Sports Program boost their physical strength, range of motion, flexibility and endurance as well as emotional well-being and overall quality of life. Just ask 28 year-old Randy Johnson. In 2013, Randy suffered a traumatic spine injury in a horrific car accident. He spent more than a month in therapy at UM Rehab. A seasoned athlete of many sports, all Randy could think about was not competing again, or doing much of anything else for that matter. After a visit from recreational therapist Mike Henley, a trauma survivor and inspiration for many others, Randy began to embrace his new life. After leaving UM Rehab, Randy spent the next two years in a variety of settings, having to rely on others for his daily care. He lived with several family members, and ended up living in a nursing home for an entire year. In September 2014, Randy attended the annual Adapted Sports Festival at UM Rehab, where he was introduced to wheelchair (quad) rugby, and the Maryland Mayhem quad rugby team (UM Rehab s home team). He had found his new sport. Randy began working out in his chair and managed to find rides to rugby practice whenever he could. Through hard work and desire, he eventually earned a starting position on the Maryland Mayhem team. Wheelchair rugby has given Randy a new lease on life. Through UM Rehab s Adapted Sports Program, and his desire to compete, Randy has achieved tremendous strength and independence. In fact, he recently moved into his own apartment, where his Mayhem teammates surprised him with a house-warming party. Now Randy is living independently and making the most of every day; his next goal is to make the USA Wheelchair (Quad) Rugby team. 5 Dental Service at UM Rehab The mission of the UM Rehabilitation & Orthopaedic Institute (UM Rehab) Dental Service is to serve children and adults who have limited access to oral health care in the community. The UM Rehab Dental Service population includes special health care needs (SHCN) patients (individuals who are mentally and/or physically disabled), as well as many children in the Maryland Medicaid Assistance Program. The Dental Service at UM Rehab is one of the few providers in the state for both pediatric and adult SHCN populations. In particular, mentally disabled adults experience a range of oral health problems greater than that seen in the general population. Their disabilities can make even routine care difficult, sometimes requiring the use of general anesthesia; but the general dentists at UM Rehab have taken up the challenge of treating these patients. More than 40% of our current patient population, or nearly 3,800 SHCN patients per year from across Maryland, come to the dental program for specialized care. Referrals to the dental program come from Maryland County Health Departments (including Allegany, Anne Arundel, Carroll, Frederick, Garrett, Howard, Somerset, Wicomico and Worchester counties), over 30 Maryland group home programs, the University of Maryland School of Dentistry and community dentists across the entire state. In fiscal year 2015, visits to the Dental Service reached nearly 9,400; and more than 1,100 patients received comprehensive dental surgery and restorative services under general anesthesia in UM Rehab s operating rooms. With a mission to serve the low income, underinsured, uninsured, medically underserved and SHCN populations across the state of Maryland, the UM Rehab Dental Service provides a critically needed benefit to the surrounding community. 6 Community Health Needs Assessment To effectively achieve our community health mission, UM Rehabilitation & Orthopedic Institute (UM Rehab) conducted a formal Community Health Needs Assessment (CHNA) during fiscal year For purposes of community benefits programming and this report, the Community Benefit Service Area (CBSA) of UM Rehab is a diverse community geographically. As the largest provider of acute spinal cord injury rehabilitation in Maryland as well as traumatic brain injury, the majority of the community outreach strategic priorities and initiatives focus on serving the needs of the disabled adult community over four large areas of Baltimore City, Baltimore County, Howard County and Anne Arundel County. The comprehensive assessment of community needs was conducted utilizing the Association for Community Health Improvement s 6-step Community Health Assessment Process as an organizing methodology. The UM Rehab Leadership Team conducted the CHNA with input from other University of Maryland Medical System Baltimore City-based hospitals, community leaders, the public, community partners, health experts and many former patients and families. CHNA Implementation Plan The top priorities represent the intersection of documented unmet community health needs and the hospital s key strengths and mission. These priorities were determined after analyzing all of the data collected from the CHNA. Health literacy is a priority throughout all UMMS hospitals and will be adapted to meet the unique literacy issues facing the respective communities served by UMMS hospitals. In addition to the shared priority of health literacy, UM Rehab identified strategic community priorities that will drive community outreach programming and partnerships over the next three years. These needs fell into two categories: clinical needs and quality of life and social needs. In accordance with the federal requirements outlined in the Patient Protection and Affordable Care Act (ACA), a formal CHNA will be conducted again in fiscal year 2018 and every three years thereafter to identify changing community needs and strategic priorities.
