Breakthrough Urban Ministries Chicago Jesuit Academy Children's Home & Aid Society of Illinois Communities In Schools of Chicago CommunityHealth Connections for the Homeless Corporation for Supportive Housing To support its Chief Clinical Officer position which addresses two major goals: improving the existing HIV care system in Chicago and helping the system prepare for implementation of the Affordable Care Act. Toward the Healthy Students, Healthy Schools program that provides students from communities that are medically underserved with behavioral health services at four public high schools in Chicago. In support of the Diabetes Prevention and Treatment Project that provides comprehensive diabetes prevention and treatment, as well as targeted health promotion services for medically underserved men, women and children in low-income communities. To support the Health Care for Homeless Adults program, which offers a comprehensive, integrated approach to behavioral health, and health promotion to medically underserved adults. Toward the services of a school nurse to assist with medical needs and health care screenings, to teach health classes and serve as a point person in health emergencies for students from modest economic backgrounds and their families. To support vital research-based trauma training to home visitors and other organizations partnering in the MIECHV (Maternal, Infant and Early Childhood Home Visiting) Home Visiting Network. Connecting Chicago Public School Students to a range of essential healthcare services and strengthening the role of nurses in the agency s school network. In support of salaries and benefits for key nursing program staff at this free clinic. Toward the creation of a system wherein all partnering agency clients are linked to a primary care physician at Erie Family Health Center under a coordinated care model. To support work that helps local supportive housing providers capitalize on opportunities to partner with the health sector, and by doing so increase access to primary and preventive healthcare, behavioral health $53, $10, $75, $60,000.00

To support nursing at the Northside Network of adolescent health centers, including Erie Amundsen School-Based Health Center, Erie Lake View School- Based Health Center, and Erie Teen Health Center. To improve access to behavioral health counseling and group programming for underserved residents through implementation of an integrated care model across three sites. Toward a nurse training project that increases access to care for the uninsured and provides clinical hours for student Nurse Practitioners. To support a nurse manager to administer clinic operations, and provide case management, patient education and primary care at this free clinic for at-risk, low income, Hispanic patients. In continued support of the Dental Care Program that provides free services by volunteer dentists and UIC College of Dentistry students for homeless adults in Chicago. Toward a nurse practitioner and expanded primary care access as the agency transitions to a FQHC Patient Centered Medical Home for critically underserved individuals on Chicago s North Side. In support of the Veterans programs, specifically the Illinois against MST (Military Sexual Trauma) project; and outreach to working poor populations about access to benefits through the Affordable Care Act. Toward the Schweitzer Fellows Program that recruits and mentors graduate students who design and implement health improvement projects for underserved Chicago communities, and the Fellows for Life Program that provides networking activities, project seed grants, and ongoing leadership development opportunities for Schweitzer alumni. In renewed support of the NICU Breastfeeding Peer Counselor Program at Stroger Hospital, which provides $45, $60, $60, $49,000.00

Toward the prenatal case management program that serves low- income women and their babies, providing holistic and comprehensive care through outreach, education and care coordination. In continued support of the dental hygienist/manager who provides preventive oral health services and oversees the day-to-day operations of the dental clinic. To partially support a nurse who provides services to formerly homeless adults with mental illnesses. To support the salary of a community nurse who oversees the Family Health Promotion Program, and toward the subscription fees for a patient database. To help fund nursing services at the Broadway Youth Center which serves LGBTQ and other medically underserved youth. To assist with the implementation of initiatives that advance free and charitable clinics as an integrated sector of the current safety net healthcare system, and to continue building the infrastructure of the Association. In renewed support of a school nurse who ensures that all students, staff, and families can access appropriate health care services. Toward the operations of this respite care center dedicated to serving homeless men and women. To fund a community nurse who manages the Out of the House Plus program, designed to provide opportunities for seniors to socialize, understand their own health and psychological needs, and empower them to take steps to positively impact their physical and mental health. Toward renewed support of the Building Bridges to North Lawndale project, designed to help traumatized Chicago children at six elementary schools in North Lawndale build social-emotional learning skills and improve classroom behavior. $32, $75,000.00

To support dedicated Doula services through the Project Hope Program which supports at-risk pregnant young women, ages 20-25, who reside in an impoverished area on Chicago s West Side. Toward the salary of a nurse who provides health services to vulnerable, medically underserved pregnant and postpartum women in residential substance abuse treatment and their children. To support the Breast and Cervical Cancer Program, relocated to MetroSouth Medical Center, where uninsured women seeking early detection services for breast and cervical cancer receive medical case management and treatment. Toward nursing salaries and the Transition Care Program that provides in-home transitional care for patients after inpatient discharge, and support and education for families that are not immediately able to cope with caring for their family member at home. Toward the provision of culturally proficient domestic violence and sexual assault services targeting Latina women in the western suburbs To support a nurse practitioner for the Integrative Health Care Program that provides physical and mental health care for clients who are homeless or formerly homeless. To support the salary of a clinic manager at this free clinic on the North Side of Chicago. $48, $45, $25, Pilsen Homeless Services Primo Center for Women and Children To provide free health care to homeless men, women, and children in the Chicago land area. In support of an additional therapist to provide traumainformed mental health services for homeless women

In support of a suburban Cook County coalition to improve the healthcare response to intimate partner violence, and increase the public s awareness of domestic violence prevention and intervention strategies and resources. Toward the operating costs of this comprehensive rape crisis center that provides crisis intervention, medical and legal advocacy, counseling, public education and professional training. To support the Prescription Medicine Program that serves men, women, and children who do not have insurance or the wherewithal to purchase urgently needed medications. To support prenatal, postnatal, and contraception services for underserved teens and pre-teens at the three Rush school-based health centers on the West Side of Chicago. In support of the Care Transitions Program that delivers home-based disease management education, daily telemonitoring and intervention, and care coordination to underserved patients with chronic conditions who are at high risk of hospital readmission. Toward the Whole Health program that provides homeless youth with primary and mental health care, health education, referrals, crisis intervention, safety, and nutritional support. To help support the salary of the nurse who oversees the ARK's Medical Clinic, which provides free medical treatment for low-income, uninsured patients. Toward medical, dental and behavioral health services for low-income children from near west Cook County and the City of Chicago whose families cannot afford private health insurance. To support the Outreach and Health Ministry Program that offers free health care, referrals, and human connection through the Health Outreach Bus which serves homeless and vulnerable individuals in six Chicago neighborhoods. Funding was awarded for operating support and the delivery of free medical care to uninsured/underinsured low-income individuals residing in central Kane County. $15, $25, $55,000.00

6 University of Chicago To support the Extension for Community Outcomes program that uses telehealth technologies which allows University of Chicago specialists to provide training and consultation to community providers, increasing their capacity to provide care of complex common chronic diseases in underserved communities.
