6 What Do I Need to Get Started? Enterprise Carriers Services will help you get your business moving in the right direction by applying for Federal Motor Carrier Safety Administration (FMCSA), Operating Authority and US DOT Number on your behalf. The US DOT Number is usually issued upon completion of the application. Your Operating Authority will be granted after your insurance and process agents are filed with the FMCSA. Enterprise has the connections in place to help save you time and money getting your insurance requirements in place. Remember: Your company cannot legally operate until all Insurance, Operating Authority and Permits are in place. Once you have been issued your MC number: Enterprise will contact your insurance agent with the MC number and request your insurance requirements to be filed with the governing agencies. Enterprise will then file a BOC-3 form, known, as a Designation of Agents form, to be filed with the FMCSA. This information is required of all companies requesting Operating Authority. If you will be traveling in New York, Kentucky, Oregon, or New Mexico, you will have special needs that Enterprise is ready to help you meet. NOTE: If your company will not be regularly traveling thru New York, Kentucky, New Mexico or Oregon, Enterprise Carrier Services will help you save time and money by applying for temporary permits only WHEN and IF you need them. New York requires a New York HUT sticker if (gross weight is over 18,000) and New York will also require separate Quarterly Tax Reporting. Kentucky will require a KYU number if (gross weight is over 60,000) and Kentucky will also require separate Quarterly Tax Reporting. New Mexico will require a New Mexico Cab Card if (gross weight is over 26,000) and New Mexico will also requires separate Quarterly Tax Reporting.
7 Oregon requires a carrier to obtain an Oregon Highway Use Tax Permit. This permit requires a bond and will also require separate Quarterly Tax Reporting. If your gross vehicle weight is over 26,000 and are traveling interstate, an International Registration Plan (IRP) or pro-rated tag is also required. Next, Enterprise Carrier Services will help apply for your International Fuel Tax Agreement (IFTA) (Requires Quarterly Tax reporting). Finally, Enterprise will provide you with information and helpful tips on keeping your paperwork in proper order to meet your Quarterly reporting and DOT regulations. Enterprise Carriers Services department has made this document only as a guide to assist you. Remember to review your Local, State, & Federal Regulations for complete compliance I information. DOT Compliance Requirements After obtaining your authority, you will be subject to a 90 day review. Listed below are six tips to assist you in meeting the DOT Compliance Requirements. 1. DRIVER QUALIFICATION Employment Application should be completed and signed. Current MVR Check should be in file. (This needs to be rerun annually) Current DOT Physical should be in file. These are just a few of the required forms needed for a DQ File. Enterprise can setup and maintain these files for your company. 1. RANDOM CONTROLLED SUBSTANCE TESTING All drivers must have a DOT certified drug test before driving any commercial motor vehicle.
8 You need to join drug consortium. (Remember to provide consortium with a current employee list every time it changes, i.e. someone quits or you have a new hire). Files must be secure. Enterprise has connections for quickly getting your company started in a DOT approved drug consortium. 1. LOG AUDITING All logs and work related documents must match. (Basically your receipts and bill of ladings must match your log) Log falsification is a serious violation! We also provide this service, allowing you to keep freight moving without the added hassle of more paperwork. Remember that if you lease owner-operators under your authority, your company is required to keep all the required paperwork for that owner-operator, as well as the company owned equipment and drivers. All receipts must be turned in by owner-operators to company. Don t forget those maintenance receipts for your Vehicle Maintenance File! 2. ACCIDENTS A Register must be kept of all DOT recordable accidents. All documents pertaining to each accident must be in a file. This means copies of Logs, receipts for wrecker service, medical attendance and documentation of phone calls. 3. COMPLIANCE FILES Each truck and trailer must have its own file, with the VIN number and tire size on the outside of each folder. Copies of every receipt for repairs made on that particular piece of equipment must be in the file. Most important is the copy of the vehicles current annual DOT inspection.
9 4. INSURANCE You MUST have a copy of your MCS90. This is part of your insurance policy. Call your Insurance Agent, if you do not have a copy on file. Your DOT auditor will ask for this form during your review.

23 Safety Certifications Please answer all questions truthfully and with the understanding that the Department of Transportation will be reviewing this information during your audit. All are Yes/No responses. Applicant maintains current copies of all U.S. DOT Federal Motor Carrier Safety Regulations, Federal Motor Vehicle Safety Standards, and the Hazardous Materials Regulations (if a property carrier transporting hazardous materials), understands and will comply with such regulations, and has ensured that all company personnel are aware of the current requirements. Applicant certifies that the following tasks and measures will be fully accomplished and procedures fully implemented before it commences operations in the United States. DRIVER QUALIFICATIONS: The carrier has in place a system and procedures for ensuring the continued qualification of drivers to operate safely, including a safety record for each driver, procedures for verification of proper licensing of each driver and procedures for identifying drivers who are not complying with the safety regulations. The carrier has procedures in place to review drivers' employment and driving histories for at least the last 3 years to determine whether or not the individual is qualified and competent to drive safely. The carrier has established a program to review the records of each driver at least once every 12 months and will maintain a record of the review.
24 The carrier will ensure that all of its drivers are at least 21 years of age and, if applicable, possess a valid Commercial Drivers License (CDL) The carrier has in place a record keeping system and procedures to monitor the hours-of-service performed by drivers, including procedures for continuing review of drivers' log books, and for ensuring compliance with all operations requirements. The carrier has ensured that all drivers are knowledgeable of the hours-ofservice requirements, and has clearly and specifically instructed the drivers concerning their responsibility to comply with applicable 10/11, 14/15, and 60/70-hour rules as well as the requirement for preparing daily log entries in their own handwriting for each 24-hour period. DRUG AND ALCHOHOL TESTING: The carrier is familiar with the alcohol and controlled substance testing requirements of 49 CFR part 382 and 49 CFR part 40 and has in place a program for systematic testing of drivers.
25 VEHICLE CONDITION : The carrier has established a system and procedures for inspections, repair and maintenance of its vehicles in a safe condition, and for preparation and maintenance of records of inspections, repair and maintenance in accordance with the Federal Motor Carrier Safety Regulations and the Hazardous Materials Regulations. Yes /No The carrier will ensure that all violations and defects noted on inspection reports are corrected before vehicles and drivers are permitted to continue operation. ACCIDENT MONITORING PROGRAM : The carrier has in place a program for monitoring vehicles accidents and maintains an accident register in accordance with 49 CFR The carrier has established an accident countermeasures program and driver training program to reduce accidents. PRODUCTION OF RECORDS: The carrier can and will produce records demonstrating compliance with the safety requirements within 48 hours of receipt of a request from a representative of the US DOT/FMCSA or other authorized Federal of State official.
26 HAZARDOUS MATERIALS (to be completed by carriers of hazardous materials only): The HM carrier has full knowledge of the U.S. DOT Hazardous Materials Regulations and has established programs for the thorough training of its personnel as required under 49 CFR part 172, Subpart H, and 49 CFR The carrier has established a system and procedures for inspection, repair and maintenance of its reusable hazardous materials packages (cargo tanks, portable tanks, cylinders, intermediate bulk containers, etc.) in a safe condition, and for preparation and maintenance of records of inspection, repair and maintenance in accordance with the U.S. DOT Hazardous Materials Regulations. The HM carrier has established a system and procedures for filing and maintaining HM shipping documents. The HM carrier has a system in place to ensure that all HM trucks are marked and placarded as required by 49 CFR part 172, Subparts D and F. The carrier will register under 49 CFR part 107, Subpart G, if transporting any quantity of hazardous materials requiring the vehicle to be placarded.
27 FOR CARGO TANK (CT) CARRIERS OF HM : The carrier has a system in place to ensure that its cargo tanks are inspected and tested as required by 49 CFR 180 by a facility registered with the U.S. DOT under part 107, Subpart F. By agreeing to these certifications, the carrier official is on notice that the representations made herein are subject to verification through inspections in the United States and through the request for and examination of records and documents. Failure to support the representations contained in this application could form the basis of a proceeding to assess civil penalties and/or lead to the revocation of the authority granted. All Applicants Must Certify as follows: Applicant is willing and able to provide the proposed operations or service and to comply with all pertinent statutory and regulatory requirements and regulations issued or administered by the U.S. Department of Transportation, including operational regulations, safety fitness requirements, motor vehicle safety standards, and minimum financial responsibility requirements.
