Assess community s current readiness for CTC and document milestones and benchmarks achieved. Ensure coordination of the CTC process with other prevention initiatives. Coach the CTC coalition s to ensure fidelity to the model, evidenced based programs are selected, and effective prevention strategies are utilized. Assists in the coordination of national and/or state campaign promotions for Weber County residents. Coordinate and monitor fidelity of prevention programs facilitated by Prevention Class Facilitators. Provide or coordinate program trainings for the Prevention Class Facilitators. Disseminate

DRIVER Nutrition Job # Part-Time; $7.31/hour (20-25 hours/week; week days) POSITION DESCRIPTION: Load and deliver Meals-on-Wheels, bulk foods, and supplies to congregate meal sites. Make sure needed food items and supplies are loaded into vehicle and that food is packaged in proper containers to keep food at safe temperatures and delivered safely and in a timely manner. Check on well-being of clients. Keep track of and report required vehicle maintenance. Keep vehicle clean, in good running order, and filled with gas. Clean and sanitize food transport containers daily. MINIMUM REQUIREMENTS: 21 years or older. Valid Utah driver license with a clean driving record since the job will require driving for the Agency; prefer one (1) year driving experience. Knowledge of van operation, food safety, and sanitation. Ability to: lift pounds, follow written and verbal instructions, establish and maintain good relationships with senior citizens, work in a timely and efficient manner, read route book and learn procedures. Will work in outside environment (driving on various road conditions in extreme weather conditions), but will spend some time in kitchen. Must have/obtain valid food handler s permit. 11 UA LAB SUPPORT UA Collection Job # Part-Time; $9.01/hour (up to 29 hours/week; week days/some weekend hours) POSITION DESCRIPTION: Greet therapists and clients politely, verify personal information, and maintain client confidentiality. Use lab procedures to collect urine specimen from clients for drug tests; label and seal for delivery. Enter client information into computer and complete forms. Answer phones and direct questions to supervisor. Stock and maintain supply levels. MINIMUM REQUIREMENTS: High school graduate/equivalent. Training or work experience in health, customer service, or criminal justice related fields. Ability to work with a variety of individuals in a continuously reoccurring circumstance. RESIDENTIAL SUPPORT Adult Mental Health Job # Part-Time; $9.01/hour (up to 29 hours/week; swing (4:00 pm 12:00 am), grave (12:00 am 8:00 am), weekend (all shifts), and Holiday (all shift hours) POSITION DESCRIPTION: Monitor residential clients and building to ensure safety and security. RESPITE SUPPORT Youth Mental Health Job # Part-Time; $9.01/hour; (average about 10 hours/week; flexible hours based on the needs of youth and family) POSITION DESCRIPTION: Assist professional staff in Direct Service Provider Team in providing respite services to families of seriously emotionally disabled youth ages The majority of time on the job will be spent with children doing activities in the community. This position entails being part of a multi-disciplinary team working directly with youth, parents, legal guardians and other community agencies working with youth.

Support maintenance of the therapeutic environment by facilitating activities of daily living consistently and according to policy and standards. Enforce approved residential rules. Clearly document any concerns. MINIMUM REQUIREMENTS: High school graduate/equivalent. This residential facility has a population of females only (therefore, this is a Bona Fide Occupational Qualify) allowing only female applicants for the position. Must be comfortable interacting with clients and children, coordinating activities, assisting with meals, etc. Able to demonstrate maturity and sensitivity to individual differences in children and families with the ability and judgment to work independently. There are likely no set shifts as schedules may change. This position could also involve your original shift to change depending on staffing needs. AUTISM SUPPORT Youth Mental Health Job # Part-Time; $9.01/hour; (up to 29 hours/week) POSITION DESCRIPTION: Assist classroom instructors with running discreet trial sessions and groups, accurately record data, assist with teaching skills-based curricula, actively engage with children between work sessions, transport children to and from school using program van, assist in providing a clean and healthy learning environment. MINIMUM REQUIREMENTS: High school graduate/equivalent; 21 years or older. Experience working with children with disabilities preferred. Required to complete mandatory trainings offered through the Agency: CPR, 1 st Aid, MANDT, and Defensive Driving. Valid Utah driver license with a good driving record (no more than 1 violation)
