Thank you for inquiring about Hopedale Commons, downstate Illinois best value and best choice in Assisted and Independent Living. The Commons is nestled just off Interstate 155 in the serene and scenic little town of Hopedale. We are 30 minutes drive to Peoria and Bloomington, and less than an hour from Springfield, Illinois. Hopedale Commons is an integral part of The Hopedale Medical Complex, which includes a 25-bed hospital (with a 24 hour ER), Medical Arts Physicians Offices, phanriacy, dentist, optometrist, nursing home, retirement apartments, Midwest Vascular Institute, and a beautiful, modem 32,000 sq. ft. Weliness Center situated a few blocks from the Commons. You may be interested to know that our late founder, Dr. Lawrence Rossi, pioneered assisted living in 1964, when he opened Hopedale House. We offer an impressive array of services to our residents for a very reasonable price. There are no entry fees, no long-term contracts or leases, (everyone pays by the month), and an apartment may be reserved for just a $ deposit (which will be applied to your first month s rent). financial assistance is also available for those who qualify. Please feel free to request an application. A special feature in the Commons West Assisted Living is that we do NOT add fees for any of our 40 services. Tt s all included in the monthly rent. We also offer respite services which allow a resident to come and stay for a day, a week, or several weeks depending on the needs of the resident. Finally, and most importantly, T think you will be very impressed with our caring and competent staff who truly enjoy working with our residents each day. Hopedale Commons Below are the pre-admission requirements for entry into Hopedale Commons. Residents of Hopedale Commons must meet the following requirements: 2. Completion of Physical Therapy Screening prior to admission. 4. Approval of Pre-admission Physical and Physical Therapy Evaluation. 1. Completion of Hopedale Commons Admission Booklet. Note: FinanciaL assistance (reduced or free rent ) is available for those residents who qualify. 6. Set date for moving into apartment. 5. Reserve apartment and deposit if applicable 7. Sign Contract. Must be signed before moving into apartment. Please inquire of the Director for an application. 8. Pay first month s rent and security deposit 2. Able to bathe with some assistance. 4. Able to ambulate on their own with use of wheelchair, walker, or cane. 3. Able to take medications with medication reminders. 5. Must be continent of bowel and bladder.
