3 WE PROVIDE A SAFE AND SUPPORTIVE LIVING ENVIRONMENT WITH SUPPORTIVE HEALTHCARE SERVICES. Our Assisted Living Program is available to residents of New Broadview Manor Adult Care Facility. Methods of payment for Assisted Living Services include: Private Payment, Insurance and NYS Medicaid. We are a NYS Medicaid provider to residents of New Broadview Manor Adult Care Facility. All applicants of the Assisted Living Program receive a comprehensive assessment by our professional staff along with a nurse from a Certified Home Care Service Agency to determine if the resident meets the eligibility standards set by the New York State Department of Health for Admission to the Assisted Living Program. All participants of the Assisted Living Program receive ongoing assessments to verify continued eligibility regarding health and safety needs at least every 6 months. WE ARE AFFILIATED WITH THE FOLLOWING
4 OUR MISSION New Broadview Manor Assisted Living Program is committed to providing safe and adequate, quality, compassionate and supportive services in an ethical manner to residents living New Broadview Manor Adult Care Facility. Services are performed with dignity and respect for residents of all ages regardless of race, creed, disability or national origin. New Broadview Manor Assisted Living Program supports caregiver staff by providing continuing education opportunities for continued professional growth. FRIENDS AND FAMILY ARE ENCOURAGED TO VISIT AND IT IS THE JOB OF EVERYONE IN OUR COMMUNITY TO BE FRIENDLY, CHEERFUL AND WELCOMING TO EACH INDIVIDUAL THEY MEET AT NEW BROADVIEW MANOR

6 Our Assisted Living Program is committed to the provision of comprehensive quality services that are planned and tailored to meet your needs. All applicants to the Assisted Living Program receive a comprehensive assessment by our professional staff along with a nurse from a Certified Home Care Service Agency to determine if the resident meets the eligibility standards set by the New York State Department of Health for Admission to the Assisted Living Program. All participants of the Assisted Living Program receive ongoing assessments to verify continued eligibility regarding health and safety needs at least every 6 months. Our Assisted Living Program staff is highly qualified licensed and certified health care personnel. All health related services are provided by our Licensed Home Care Service Agency under the direction of a Registered Professional Nurse. Our RN collaborates closely with your physician in the development of your plan of care.
