1 Clinical Nursing I, NUR 1234L Syllabus Addendum Fall, 2015 Psychiatric Mental Health Nursing Clinical Experiences The clinical experiences for Psychiatric Mental Health Nursing will include the following: 1) Two (2) six-hour clinical days at life Stream Behavioral Health (7:50 am 2:00 pm). 2) One (1) 6-hour clinical day at a long term care facility: Osprey Lodge for Leesburg students and Clermont Health for Clermont students. (7:50 2:00 pm). 3) Attendance at two support groups: one should be Alcoholics Anonymous and the other may be a support group of your choice; i.e. other 12 step groups or hospital sponsored support groups, etc. If you have questions regarding the suitability of a support group, please see your classroom instructor. 4) Attendance at one session of Teen Court. Tuesday evenings from 5:30 7:30. Go to search for teen court and read about the Teen Court program and check the schedule as a session may occasionally be canceled. Arrive by 5:15 so that you are seated in the courtroom when the process begins. NOTE: Students will complete written assignments each week based on their scheduled clinical location and submit them through Blackboard by 11:00 pm on the Monday following the experience. You may begin to attend the support groups and teen court at any time during the semester. DO NOT however, plan for a large group to attend any single meeting as your presence would overwhelm the participants. Small groups of 2-4 for support group meetings and for Teen Court should be the maximum number of student attendees. Please submit your teen court and support group assignments as you complete them. Deadline for all support group and teen court papers to be submitted is Friday, November 20, 2015 by 11:00 pm. ASSIGNMENTS Life Stream Behavioral Health: 1) Completed interaction report (also known as Process Recording) each day based on your interaction with a client. On page 125 of Essentials of psychiatric Mental Health Nursing (6 th ed), you will find an example of an interaction report or process recording. You are to pattern your report after that format. Refer to the Communication Techniques and Techniques of Therapeutic Communication that are available to you on the nursing webpage under Alterations in Health I forms/other and your text. These resources will assist you identifying therapeutic/non-therapeutic communication techniques. This report is NU: Fall /17/15 1
2 to be submitted through Blackboard by 11:00 pm on the Monday night following the clinical experience. 2) Reaction paper After your first day at Life Stream, complete your one page reaction paper in response to the following: Discuss how you feel about your beginning therapeutic communication skills. What did you do well and why? What do you need to work on? What would you like to do differently in your next clinical experience? After your second day at Life Stream, complete your one page reaction paper in response to the following: Discuss your observations and thoughts about your experience with the hospitalized client who has a mental health diagnosis. Reflect on how your experience has altered any preconceived ideas you may have had about the inpatient mental health setting and the clients found there. How do you believe that this experience will impact the rest of your nursing school experience and your nursing career? Papers are to be submitted via through Blackboard by 11:00 pm on the Monday night following the clinical experience. ASSIGNMENTS Long term care facilities: Osprey Lodge, Leesburg and Clermont Health and Rehab, South Lake 1) Folstein Mini-Mental Status exam You will find this assessment tool on the nursing web page under Clinical Nursing I forms/other. You will need to read the assessment tool, and bring a copy of it with you as well as the tools required to complete it: wrist watch, pen, and 4 blank pieces of paper. Evaluation: Score of = normal; = mild/moderate cognitive impairment; 0-17 = severe cognitive impairment. This assessment should be completed and turned in to instructor prior to leaving the facility. 2) Comprehensive Nursing Mental Health Assessment A comprehensive nursing mental health assessment will be completed on a client during your day at your long term care facility. You will need to print a copy of the assessment form from the nursing web page under Clinical Nursing I forms/other to make notes for the interview. The form is interactive (open with Adobe Reader) so that you may type your assignment directly onto the form, save, and submit through Blackboard by 11:00 pm on the Monday night following the experience. 3 ASSIGNMENT Support Group/AA Meeting Reaction Paper Students will complete a Reaction Paper on each support group attended. The paper should include a cover sheet with the student s name, group attended, and the date. The body of the paper should include the following details: 1) Describe the seating arrangement and the number of members. 2) What commonalities do you observe among the group members such as occupation, dress, age? 3) Describe how a group member was able to express positive feelings during the meeting. 4) Describe how a group member was able to express negative feelings. 5) What did you witness that would lead you to believe that a member found hope in overcoming a problem? 6) Did the group members support each other leading to positive self-esteem? If so, what did you observe that led you to that conclusion? 7) Did one member serve as a role model for another group member or the group as a whole? 8) Describe your feelings about this group. Papers should be 1-2 pages long, 12 font, double-spaced with detail given to grammar, punctuation and spelling. Be sure to protect the group members' privacy. When attending the groups, go to the group leader prior to the session and identify yourself and your purpose (to learn about support groups that are available for your clients). Do not wear name tags. Dress in business casual (see instructions for dress in the mental health facilities). DO NOT TAKE NOTES DURING THE MEETINGS. Papers should be submitted through Blackboard by 11:00 pm the Monday following the support group experience. ASSIGNMENT Teen Court Reaction Paper Each student will attend one session of Teen Court/ Teen Court is from 5:30-7:30 pm on Tuesday evenings at the Lake County Court House in Tavares. When you go into the courthouse you will have to go through a security check-point (metal detectors) so be aware prior to going there of any contraband that you might have on your person. Ask at that point which courtrooms are being used for Teen Court. Dress is business casual. You do not wear your name tag. You are expected to behave in a professional manner. If you live in an area that has a Teen Court experience; i.e. Sumter or Marion Counties, and wish to attend at that location it is permissible. NU: Fall /17/15 3
4 The assignment for this experience is a Reaction Paper in which you will address the following 6 questions: 1) What were your expectations for this experience? 2) Were your expectations met, disappointed, or exceeded? How? Why? 3) How did you feel about the way the court process was handled? 4) What was most interesting to you? Why? 5) Did the experience change your mind about anything? If so, what? 6) What are your feelings about Teen Court being an effective way to handle problems with juveniles? You may begin to attend the support groups and teen court at any time during the semester. Submit your papers by through Blackboard by 11:00 pm on the Monday evening following your experience. Remember that all support group and teen court papers must be submitted by Friday, November 20, 2015 by 11:00 pm Clinical Agency Locations Use MapQuest if unsure of location Life Stream Behavioral Health 2020 Tally Road Leesburg, FL Park in spaces further away from the front door. Meet your clinical group and instructors at the front door at 7:50 am. Teen Court/Lake County Courthouse 550 W. Main Street Tavares, FL Parking is available in the parking garage across the street from the courthouse. Osprey Lodge 1761 Nightingale Ln Tavares, FL Clermont Health and Rehab 151 E. Minnehaha Ave. Clermont, FL CARPOOLING IS ALWAYS A GOOD IDEA FOR CLINICAL SITES DUE TO PARKING LIMITATIONS NU: Fall /17/15 4
5 Psychiatric Mental Health Clinical Contract While at Life Stream Behavioral Health, clinical hours will be from 7:50 am 2:00 pm. Lunch break to be determined by the instructors. Do not bring anything into the facility other than your car keys which will be held for you by the instructors. You may want to have a notebook and pen in your car if you wish to make some notes at the end of the day. While at the long term care facilities, (Osprey Lodge for Leesburg, Clermont Health for Clermont), clinical hours will be 7:50 am 2:00 pm. Lunch break to be determined by instructor. Bring a pocket notebook, pen, copy of the comprehensive nursing assessment, and Folstein Mini-Mental Status exam. Leave valuables at home or locked in car. Dress for ALL Mental Health clinical experiences shall be: casual business attire NO JEANS, NO OPEN-TOED SHOES, NO HIGH HEELS. Clean athletic shoes with clean laces are acceptable footwear at Life Stream Behavioral Health. You must wear the name badges provided to you by the clinical instructors at all times while at Life Stream (not your LSSC name tags). LSSC name tags are to be work at the long term care facilities. Lab coats may be worn at the long term care facilities. NO LAB COATS ALLOWED AT LIFE STREAM. You may wish to have a sweater or jacket while at Life Stream as the temperature is usually cool in the units NO SWEATSHIRTS, FLANNEL SHIRTS, etc. Long hair should be pulled back or worn up, earrings must be no more than ONE PAIR of small studs (no dangling, hoop, etc.) for female students. No necklaces should be worn. Suggestive clothing is absolutely prohibited and will result in the student being sent home and having a clinical absence as well as an unsatisfactory item on your clinical evaluation. Suggestive clothing includes but is not limited to: low-cut or see-through tops, tight tops or pants, lowriding pants with underwear or skin showing, tops and pants that do not overlap and display visible skin when walking or reaching. USE GOOD JUDGMENT AND DECISION-MAKING SKILLS. Should illness or emergency prevent you from attending clinical, call your clinical instructor as soon as the situation is known. Your clinical instructors will advise you how they wish to be contacted. DO NOT SEND . Confidentiality of patient information is critical during your mental health experience as in all of your clinical experiences. Breach of confidentiality will result in a failing grade for the clinical component of this course. My signature indicates that I have read and understand the above requirements.
