INTRODUCTION 1.1 Context The Light Vehicle, Mobile Equipment and Traffic Management Standard has been developed to: Eliminate the potential for fatalities and serious injuries associated with the operation and interaction of traffic and pedestrians. Eliminate damage to plant and equipment. Outline the methods of traffic management. Manage roads and associated road infrastructure. 1.2 Scope This standard applies to all employees and contractors who will be driving or operating light vehicles and/or mobile equipment and to the management of traffic, roads and infrastructure on site. 2. ACCOUNTABILITIES Mobile Equipment Superintendent o Ensuring inspections of light vehicles and mobile equipment that access and operate on site are completed. o Ensuring roads, road markings and signs comply with Transport Operations (Road Use Management) Act 1995 and are kept in good condition. o Communicate with the relevant government agencies on behalf of Queensland Nickel and apply for the relevant permit/s for any vehicle movements or vehicular associated with Queensland Nickel on Greenvale Street that may contravene the Transport Operations (Road Use Management) Act Area Superintendent o Ensuring prestart inspections of light vehicles and mobile equipment that operate on site are completed. o Authorising the use of an area vehicle. Area Supervisors o Arrange for dust suppression to control dust if it is required for a specific activity in an area on an unsealed road attached to that area. o Where there is no SWI, must ensure a JSA is reviewed and approved for all mobile equipment activities within their area of control. o Must ensure operators of mobile equipment are competent to do so. Drivers/Operators of Light Vehicle/Mobile Equipment o Complying with site rules for driving/operating light vehicles and mobile equipment. o Where there is no SWI, a JSA must be completed, ensuring it is reviewed and approved for all mobile equipment activities within their area of control. o Report any road condition/infrastructure concerns to their supervisor or the Mobile Equipment Superintendent. o Completing a prestart inspection o Must ensure they are competent to operate the mobile equipment that they are operating. Must ensure operators of mobile equipment are competent to do so. 3. DEFINITIONS TERM Light Vehicle Mobile Equipment DEFINITION A Light Vehicle is a vehicle which: Can be registered for use on a public road, has four or more wheels, seats a maximum of 12 adults (including driver), requires the driver to hold a C class license, does not exceed 4.5 tonnes gross vehicle (GVM) as specified by the vehicles manufacturer, an approved and accredited automotive engineer, if the vehicle has been modified to the extent that the manufacturers specification is no longer appropriate Mobile Equipment is a vehicle that generally requires a specific licence issued from the Department of Motor Transport to drive or requires a nationally recognised certificate of competency issued by Work Health & Safety to operate. Examples of mobile equipment include but are not limited to: rear dump, water trucks, graders, dozers, loaders, excavators, forklifts, telehandlers, mobile cranes, buses, backhoes, bobcats, and other trucks larger than light vehicles. Some requirements of this standard may not be applicable in some cases and a risk-based approach will be used to determine the level of compliance required with each of the specific requirements of this standard. NOTE: Mobile cranes are considered to be mobile equipment when they are travelling to and from job sites. Traffic Management The methods used to eliminate or minimise the risk of harm to people, damage to the environment or roads and surrounding infrastructure. PROCESS 4.1 Personal Requirements All personnel driving or operating light vehicles or mobile equipment must hold a current and appropriate licence or nationally recognised certificate of competency for the type of vehicle they are driving or operating. 4.2 Light Vehicle and Mobile Equipment Requirements All light vehicles/mobile equipment must be fit-for-purpose for the activity for which they are being used and be authorised to access the site as per the requirements listed on the Appendix 1 a Application for Light Vehicles & Mobile Equipment Gate Access. A Light vehicle/mobile equipment which has been authorised to access the site and complies with the requirements specified in the Appendix 1 a Application for Light Vehicles & Mobile Equipment Gate Access, will be issued with a Site Access sticker valid from 12 months from date of issue. 4.3 Pre-start inspections Prior to driving/operating any a visual inspection must be completed. A documented weekly inspection must be recorded in the light vehicle logbook, dated and signed. The logbook should be kept in the vehicle so that it can be easily referred to. A documented daily inspection must be recorded in the mobile equipment logbook, dated and signed. The logbook should be kept in the vehicle, where, possible, so that it can be easily referred to. Mobile Equipment must be isolated prior to performing a pre-start inspection. The light vehicle/mobile equipment must be isolated, tagged out-of-service and the key removed if the inspection finds a major defect that could make the light vehicle/mobile equipment unsafe to drive/operate. All defects are to be reported to relevant Supervisor or Contractor Supervisor. 4.4 Light Vehicle and Mobile Equipment Operation - General Requirements The light vehicle/mobile equipment driver/operator is responsible for compliance with all aspects of light vehicle/mobile equipment operation including: Keeping all body parts within the light vehicle/mobile equipment while it is in motion. Driver and passengers are to leave the light vehicle/mobile equipment while it is being refuelled. All light vehicle/mobile equipment must have the park brake applied, all raised equipment lowered and engine switched off if unattended. All items in the cab are secured or separated from driver where possible. All items carried in the tray of a truck or utility are secured. All occupants wear their seat belt at all times while the light vehicle/mobile equipment is being driven/operated. Adhering to sign-posted speed limits. Not using a mobile phone whilst driving a light vehicle/mobile equipment (including hands free). Giving way to pedestrians on designated zebra crossings. 5 Driving the light vehicle/mobile equipment with the head lights turned on low beam at all times. All vehicles while operating within the Queensland Nickel site are required to have an amber rotating warning light visible to any personnel from 360 ( and turned off when leaving site). All vehicles while operating within the Queensland Nickel site are required to have an operational reverse alarm fitted (and turned off when leaving site). Seeking authorisation from responsible manager before using a company vehicle for private business. Complying with Queensland Transport Operations (Road Use Management) Act 1995 and Regulations. Take into consideration the prevailing road surfaces & conditions and drive to suit the conditions. All Telehandler and Mobile Crane operations require a JSA to be completed if an SWI is not available. All light vehicle/mobile equipment are required to carry, as a minimum, a serviceable fire extinguisher Parking Key points include the requirements to: Park in designated parking areas where possible, or off the road if designated parking is not available. Leave keys in the ignition of the light vehicle/mobile equipment where ever possible. o NOTE: The management of leaving keys in area operated light vehicle/mobile equipment is at the area s discretion to manage. Not park in drains or directly under pipe racks or process equipment Driving Offsite or to Remote Locations When driving to locations offsite or in remote areas on site the driver shall inform his supervisor where they are going, their expected return time and have a suitable communication method such as a mobile phone or two way radio. 4.5 Company Vehicles Personnel from other areas are not permitted to use an area vehicle unless authorisation is sought from the relevant Superintendent Pool Vehicles The process and requirements to use a pool vehicle are explained in Appendix 2 b Pool Vehicles 4.6 Breakdowns Any light vehicle or mobile equipment that breaks down whilst in operation on the Queensland Nickel road network should, if possible be parked using any area that minimises traffic disruption. Once the light vehicle/mobile equipment has come to a complete stop the vehicle is to be immediately secured by: switching off the engine, placing the vehicle transmission in first gear if a manual, or in park if an automatic, applying hand brake, fitting wheel chocks/blocks (if required), Published Date: 12 June 2012 Rev: 3.0 Next Review Date: 31 May 2014 Page 5 of 12

6 activating hazard lights (if fitted) and, using break down triangles (if required and carried). Any roadside repairs that are to be performed to remobilise the vehicle from the break down area, or for recovery onto another vehicle, are to be carried out in accordance with YHSS052 Job Safety Analysis Standard & YHSS047 Permit to Work and Isolation Standard. 4.7 Queensland Nickel Road Infrastructure Management Damage or Deterioration to Road Surfaces The internal road network condition is monitored within Queensland Nickel and it is the responsibility of the Mobile Equipment Superintendent to ensure all roads and surfaces are safe and reasonably trafficable (depending on type of road and area). Whilst traversing the Queensland Nickel road network, all road users are encouraged to report any road damage, surface damage, deformation and wear that could possibly result in an incident or vehicle damage. If any significant deformation or subsidence is detected and involves the entire road width, or even a section of the road that is subject to normal traffic movements, and the integrity of the road can t be guaranteed, the road is required to be closed and notified immediately to the Mobile Equipment Superintendent. It is the responsibility of the Mobile Equipment Superintendent to arrange for all on-site road repairs Dust Suppression & Water Management for Roads Queensland Nickel under Environmental Protection Agency regulation & licensing requirements are obliged to keep dust emissions from site to as low as possible. All unsealed roads that mobile equipment or light vehicles are operating on are to be watered down at appropriate intervals, based upon the activity, with consideration given to current weather conditions, to prevent dust emissions. If dust suppression is required on an unsealed through road that light vehicle/mobile equipment normally use, it is to be referred to the Mobile Equipment Superintendent Lighting The Queensland Nickel site is a facility that operates continuously and as such has personnel present at night. There is a basic system of road lighting in operation in the hours of darkness between dusk to dawn. Safe walkways adjacent to sealed road areas are lit, however pedestrian crossings do not have dedicated overhead lighting. Pedestrian crossings and the safe entry routes into the various areas of the plant are lit by sufficient ambient light. 4.8 Public Roads Greenvale Street All road users operating on Greenvale Street are to obey ALL road rules as defined in the Transport Operations (Road Use Management) Act 1995 and its subsequent regulations. For any Queensland Nickel vehicle movements or vehicular activity associated with the Queensland Nickel on Greenvale Street that may contravene the Transport Operations (Road Use Management) Act 1995, communication is to be made prior to the movement/activity to the Mobile Equipment Superintendent. 7 Townsville City Council 4.9 Installation & Maintenance of Existing Traffic Control Devices Installation of Traffic Control Devices All Queensland Nickel road markings relating to installation of intersections, pedestrian crossings and speed and other signage is in accordance with, or equivalent to, the relevant sections of AS 1742:2010 Manual of Uniform Traffic Control Devices Control of On-Site Traffic All traffic operations upon the Queensland Nickel site must comply with the relevant operational aspects of: Transport Operations (Road Use Management) Act 1995 Transport Operations (Road Use Management - Road Rules) Regulation 2009 Transport Operations (Road Use Management - Driver Licencing) Regulation 2010 All on-site roads within the Queensland Nickel are signed and marked generally in accordance with AS 1742:2010 Manual of Uniform Traffic Control Devices to assist in immediate driver recognition and comprehension Alterations to Road Network & Road Closures Sometimes certain activities at the Queensland Nickel may cause temporary partial or full road closures. These may be in relation to: Lifting Operations. Refer YHSS046 Lifting Operations & YHSS055 Barricades and Barriers Maintenance or other operations activities within the process plant areas Construction or demolition activities Other considerations in relation to temporary, partial or full road closure situations are: Maintenance of the road asset; or, Safety considerations due to: o Flooding o Road damage o Movement of OVERSIZE vehicle combinations For the formal Queensland Nickel process regarding temporary, partial or full road closure refer to section Upon approval by the Mobile Equipment Superintendent of any temporary, partial or full road closures, all subsequent traffic control equipment (bollards, barriers, signs & traffic control etc) is required to be used in accordance with the locations and specifications as outlined within the following Australian Standards, including, but not limited to: AS 3845:1999 Road Safety Barrier Systems AS 1743:2001 Road Signs - Specifications AS :2007 Retro-reflective Materials and Devices for Road Traffic Control Purposes. AS 1742:2010 Manual of Uniform Traffic Control Devices Ensure any temporary, partial or full road closures, once approved, is communicated to site via the Yabnet and notification Formal Process for Temporary, Partial or Full Road Closure Whenever a partial or full road closure is required it is the responsibility of the person making the request to complete a risk assessment. 8 Time of delivery (scheduled for weekends, or off peak times); and, Survey of proposed route and set down area for its suitability including the following: o Identification and recording of any road infrastructure that requires temporary removal; and, o Analysis of clearance heights to pipe work and width clearances to buildings and other fixed structures etc; and, o Hazardous areas; and, o Restricted areas Any OVERSIZE vehicle combination/s entering or leaving the Queensland Nickel site are required to be operating under, and compliant with, the provisions of: Transport Operations (Road Use Management- Mass, Dimensions & Loading) Regulation; and, Performance Guideline for Indivisible Articles (up to 25m long & 3.5m wide); and/or, o Performance Guideline for Pilot and Escort Vehicles (over 25m long & 3.5m wide); and/or, o Performance Guideline for Excess Mass (where applicable) o Load Restraint Guideline Queensland Nickel Speed Limits Speed limits on site are in accordance with the specific area and signposted as follows: 8 kph on roads in process areas 25 kph on roads in non-process areas 40 kph on specified roads Overtaking Protocols With the exception of light vehicles overtaking slow moving vehicles and equipment i.e. vehicles or equipment travelling at or less than walking speed, overtaking of moving vehicles travelling in the same direction is not permitted on site. It is a light vehicle driver s responsibility to ensure that overtaking of a slow moving vehicle is only undertaken when it is safe to do so. An overtaking manoeuvre should not be undertaken unless: Positive communication has been established between all parties (refer Communication between Vehicles). The driver of the vehicle being overtaken signals their acknowledgement for the overtaking manoeuvre to proceed to the driver of the vehicle wishing to overtake. Visibility is clear and unrestricted. The vehicles are clear of any road intersection or junction. The area speed limit is not exceeded by the overtaking vehicle Pedestrian Traffic All pedestrians shall, where possible, traverse the Queensland Nickel site using the established safe walkways and should: Only cross roads at marked pedestrian crossings. Avoid having mid roadway discussions/meetings Use a footpath where one is provided, in preference to walking along a roadway During the hours of darkness, comply with YHSS062 Personal Protective Equipment Standard- in relation to high visibility clothing and reflective panels. 9 If required to enter zones of mobile equipment operations, ensure positive communication with mobile equipment operators is established before entering. Maintain positive communication with mobile equipment operators whilst in the operations zone Communication between Vehicles Communication between light vehicles and mobile equipment on-site is primarily by visual means only. A combination of visual confirmation, gestures, hand signals, operation of vehicle indicators and sounding of warning devices (horns, etc) are some of the communication methods utilised. Where required, spotters must be deployed to ensure clear and unambiguous communication. Mobile equipment shall follow all site road rules when tramming. Permission must be sought from the people within the barricaded area or the contact person on the barricade tag to travel through a barricaded area. Within the 311 area there are specific work instructions that outline communication methods between types of vehicles Queensland Nickel Site Based UHF- 16 Channels Queensland Nickel has its own licensed and dedicated UHF frequencies for internal communications. These specific frequencies are assigned numbers from 1-16 inclusive and are dedicated to voice communication for particular areas. Refer to Appendix 3 c Site-Wide UHF & VHF Channels and Frequencies. A spare UHF channel is available for special purposes such as commissioning activities, but is not normally programmed into 16-channel radios. The remaining channels are point-to-point (simplex) and do not rely on any infrastructure to transmit and receive. Note: Channels 3-16 are for operational purposes within assigned areas Emergency Channel Channel 1 is the emergency channel. This channel is for the emergency response crew (ERC). Radios dedicated to this channel are in each area control room, gatehouse and with the ERC members on site. Channels 1 and 15 have repeater stations for extended coverage. The repeater stations are situated above boiler 4 in the 514 power station area. These are mains powered, but have battery backup for around 24 hours. The repeaters stations will emit a sub-audible tone or a faintly audible "siren" on channel 15 during voice traffic in the event of a mains failure. If noticed, report it to shift maintenance immediately Fire Wardens & Security Channel Channel 4 is for fire wardens and security only Queensland Nickel Site Based VHF Channels Queensland Nickel has its own licensed and dedicated VHF frequencies for internal communications. These specific frequencies are assigned numbers from 1-7 inclusive and are dedicated to voice communications within special applications such as crane/lifting operations. 10 Interaction/Segregation between Pedestrians, Light Vehicles and Mobile Equipment Pedestrian traffic is not permitted within areas that mobile equipment is operating and is to keep to the provided footpaths or dedicated walkways/safe entry routes. Personnel within an area that have mobile equipment operating in that area, or any other personnel requiring communication with an mobile equipment operator, are to initiate contact either: Via the area portable UHF radio (on the area specified channel); or, By attracting visual contact with the operator, from the front of the mobile equipment at a safe distance- no closer than ten metres; or Approaching the control room, attached to the area of mobile equipment operation. The necessary communication with the operator can then take place. When requiring to enter 311 operations area ensure permission with the 311 operations supervisor is obtained and the 311 Traffic Management Plan is read, understood and signed. It is a requirement that an area portable UHF radio is carried at all times whilst in the operations area Overhead Powerlines & Other Electrical Services Operation of any vehicle within the vicinity of powerlines and other utilities is not permitted, unless operating in accordance with the relevant work management standards, as listed, but not limited to the following: YHSS067 Overhead Powerline Exclusion Zone Standard YHSS066 HVIA Standard YHSS046 Lifting Operations YHSS052 Job Safety Analysis YHSS047 Permit To Work & Isolation 4.11 Restricted Areas Ore Receival - Area 310 and Solar Drying Area 311 The 310 area at the western boundary of Queensland Nickel and the 311 area are subject to Australian Quarantine & Inspection Service (AQIS) federal regulation with respect of the imported nickel ore railed to the Queensland Nickel facility, sourced from overseas. The 310 area is also subject to Queensland Rail (QR) Trackside requirements. Only vehicles attached to these areas, or contractors contracted to work in these areas are permitted to access these areas. All other vehicles shall not enter this area, or be subject to strict AQIS restrictions in relation to quarantine. Contact the Ore Drying Superintendent for further details Hazardous Areas Gas Plant - Area 420 The gas plant, area 420, is classified as a hazardous area due to low flash points and quantities of various classes of flammable liquids and gasses stored and processed within the area. No vehicles are to access any part of the gas plant unless they comply with YHSS047 Permit To Work & Isolation Standard YHSS048 Hot Work Standard The only exception to the above are bulk road transport, licensed and registered, dangerous goods carrying semitrailers & B doubles while conducting unloading Published Date: 12 June 2012 Rev: 3.0 Next Review Date: 31 May 2014 Page 10 of 12

11 activities within the confines of the unloading facility, and using the gas plant service road to access the unloading facility. 5. TRAINING All employees and contractors shall hold a current Queensland (or equivalent from another Australian State) licence or certificate for the light vehicle or mobile equipment they intend to operate. Personnel wishing to learn to operate Mobile Equipment shall only begin this process after an appropriate training plan has been developed which includes identification of supervision required, use of log book and completion of accredited training and assessments. 6.
