II. Purpose of Addendum On 08/12/2014, Ms. Catherine Craig, the Kids Club and preschool program administrator and licensee designee, requested a modification of the capacity of Dolsen Kids Club. III. Description of Findings and Conclusions The terms of the original license have been modified on 09/12/2014, 10/07/2011, and 10/14/1996. On 08/12/2014, the Licensee Designee, Ms. Catherine Craig, requested that the capacity at Dolsen Elementary Kids Club be raised to 72 children. Although the school is licensed for preschool through school-age children, currently only schoolage children are in attendance. There are no changes in the approved use-space. The approved rooms are as follows: Room B1 is 29 x 28, providing 812 square feet. This is sufficient space for 23 preschool and/or school-age children. Room B117 is now labeled Room A1. It is 26 x32-4 x7, providing 804 square feet. This is sufficient space for 23 preschool and/or school-age children. The cafeteria is 63 x 33 + 8 x 39, providing 2391 square feet. This is sufficient space for 68 preschool and/or school-age children. 2

The gymnasium is 81 x 60, providing 4860 square feet. This is sufficient space for 139 children. This space exceeds the space required for the requested capacity of 72 preschool and/or school-age children. At this time there is no preschool program in the building. Children in the Kids Club program will use both the cafeteria and gymnasium, depending on the type of activity they choose to engage in. In the event that one of these spaces are not available, the other approved space may be used. This space was last inspected by a Qualified Fire Inspector on 08/12/2008, and received an A rating. Public school buildings are exempt from fire safety rules and no further inspections are required. The school was last inspected by an Oakland County Sanitarian on 02/11/2009, and also received an A rating on its environmental health inspection. The school utilizes the city water and sewage system. The program does no food preparation, and is not adding an infant component, so further inspections are not necessary. The playground was inspected on 09/27/2014 by a qualified playground inspector. It is approved for use by children between the ages of 5-12 years old. If the school was built before 1978, a lead risk hazard inspection is required before January 2, 2017. However, this applies only if any program other than the Kids Club School-Age program is housed in the building. At this time it is not required. Dolsen Elementary School has several multi-sink, multi-toilet bathrooms typical of elementary school buildings. Those closest to the gymnasium and cafeteria contain 10 toilets and 8 sinks. These exceed the 5 toilets and sinks required for use by 72 children. Kids Club operates from 7:00 AM-9:00 AM and 4:00-6:00 PM daily throughout the school year and is available to all of the elementary school children. Ms. Craig provided me with an updated equipment and toy inventory. The schoolage children have a multitude of items to use, for both indoor and outdoor play. Ms. Elizabeth Kucharczyk will continue as a school-age program director for Kids Club. She was approved as a program director under previous licensing rules on 01/22/ 2001. At that time she was found to have the appropriate educational qualifications and met the requirements of the program director rules in effect at that time. She was approved on 01/22/2001. Ms. Kucharczyk has an Associate Degree as a Child Care Worker from Washtenaw Community College. The program had been granted an exemption to the administrative training requirement for Ms. Kucharczyk, but that was rescinded when the licensing rules changed 01/02/2014. She was a program director for 5 years before 12/07/2006, and is exempt from this 3

requirement per R400.8113(15). As there is no preschool program this year, Ms. Angelo Ugo, who had functioned as the program director of the preschool, does not currently work at the school. Ms. Catherine Craig, the Early Childhood Director, is the licensee designee. She possesses both a Bachelor s and Master s Degree in Early Childhood Education and was approved as a program director on 02/10/1989, under previous licensing rules V.
