Who is directly responsible for implementing the corrective action for each violation. Specific time frames for each violation as to when the correction will be completed or implemented. 4 II. ALLEGATION(S) There is water in the basement of the facility. The framework on the basement doors is rusted out. III. METHODOLOGY 07/13/2011 Special Investigation Intake 07/14/2011 Special Investigation Initiated - On Site 08/10/2011 Contact - Document Sent to Mr. Prasser. 08/10/2011 Exit Conference- sent to the Administrator. 08/10/2011 Contact- Telephone The secretary informed me that the Administrator was on vacation. ALLEGATION: I also left a message for the License Designee regarding the exit conference. There is water in the basement of the facility. The framework on the basement doors is rusted out. INVESTIGATION: On July 14, 2011, I completed an unannounced on-site inspection at the facility. Upon arrival, I met with the home manager, Home Manager #1. I discussed the allegations with HM #1 and she informed me that there was water in the basement, but it s usually locked. HM #1 indicated that she was not exactly sure why there was water in the basement, and most of the time, it s only a problem when there are heavy rains. It should be noted that it had recently stormed. During the on-site inspection, I observed the outside of the building. I noted that the downspouts did not direct the water away from the facility, causing it to pool in a lower section in the driveway, and ultimately leak into the basement. I completed a walkthrough of the facility and observed the basement. Prior to entering the basement, I had to walk through a puddle of water that was pooling at the basement door entrance. The basement was damp, and there was standing water in some areas of the basement. The basement walls were also soiled, and appeared to have some water damage. The basement doors and frames were also rusty, and the doors were difficult to open and close. 2

5 APPLICABLE RULE R Maintenance of premises. (1) A home shall be constructed, arranged, and maintained to provide adequately for the health, safety, and wellbeing of occupants. ANALYSIS: CONCLUSION: There was water was leaking into the basement, causing the basement to be damp. There was also standing water in the basement and there was evidence of water damage on the walls. The doors and frames leading to the basement were rusty. VIOLATION ESTABLISHED IV.
