Treatment Consent Form For Lipo-Light and/or Lipo-Light Pro Body Contouring I duly authorize the technicians of Anti-Aging Centers to perform the Lipo-Light and/or Lipo-Light Pro procedure for the purpose of body contouring, lymphatic drainage, improving the appearance of cellulite and skin tightening. If I do not make an effort to address my diet and exercise, I am aware that the results achieved may not be retained.
