FIELD TRIP APPLICATION FOR K-8 SCHOOLS MUSEUM EDUCATION PROGRAM OVERVIEW: Thank you for your interest regarding a field trip opportunity with the 49ers Museum. The 49ers Museum, presented by is a two-level, 20,000 square foot space dedicated to showcasing the past, present, and future of the 49ers organization and its commitment to education and innovation. We are very proud of the educational program we have created. 49ERS MUSEUM EDUCATION PROGRAM: Our Museum experience provides learning platforms that connect, inspire and engage students in grades K-8 through relevant content, interactive explorations, group/peer collaboration, application of skills learned and student reflection. Specifically, the 49ers Museum Education Program will use football as a platform for: Teaching content-rich lessons in STEM (Science, Technology, Engineering, and Mathematics) where students engage in finding real solutions to real-world problems Ingraining invaluable life skills Encouraging physical activity COST & APPLICATION: All field trip offerings are at no cost to K-8 schools interested in attending. With such an amazing opportunity, one can imagine the vast amount of interest we receive from schools. Because of high demand, we have limited slots available for the 2015/16 school year and have initiated an application process that enables us to manage field trip requests in a manner that effectively directs volume and ensures the quality/integrity of the learning experience. THE 49ERS MUSEUM EDUCATION PROGRAM CURRICULUM: Is aligned to National & California Common Core and Next Generation Science Standards Engages students in a variety of technologically-rich learning experiences that are student-centered and can be adapted to various development levels and learning audiences Integrates STEM learning through project-based activities that transfer student knowledge and skills to realworld problems Is aligned to 21st Century Learning Skills (critical thinking, problem-solving, communication, collaboration & creativity) and focuses on pertinent life skills Increases student motivation for learning and enhances student understanding of STEM concepts Your field trip experience includes Customizable Curriculum Options: Educators will choose from an array of engaging lesson options; each of which is aligned to standards and sure to capture the imagination of students Grades K-3/7-8 Education Programs Grades 4-6 Education Program Levis Stadium Tour: Students will tour and learn about how the stadium is a showcase for technology innovation and environmental sustainability Guided Museum exploration: Students will experience the 49ers history and the game of football through an interactive experience built on innovative technology Movement Lab: In alignment with the NFL PLAY 60 campaign to encourage kids to be active for 60 minutes a day in order to help reverse the trend of childhood obesity, we are providing a kinesthetic movement lab to support physical activity and to get students excited about exercising

FIELD TRIP APPLICATION GUIDELINES: To qualify for a field trip, your school must: 1. Serve students in the K-8 grades in California. We highly encourage schools located in the greater Bay Area to apply. 2. Submit the 49ers Museum Field Trip Application. 3. After being accepted to attend a field trip, complete a 49ers Museum consent form for each student/chaperone participating. The consent form will be sent four weeks prior to your visit. 4. Complete an onsite program survey: Our program takes great pride in critically evaluating performance and collects data through teacher surveys for our field trip program. As outcome measurements are vital to this program, we would appreciate schools taking the time to complete onsite surveys of their field trip experience. We provide teachers the opportunity to provide video testimonials as well to support program growth and improvement. Please note that field trip experiences are limited. Field trip availability is not guaranteed. Please submit your field trip application at least six weeks prior to your anticipated visit. FIELD TRIP APPLICATION PROCESS: 1. To request consideration, apply online or download the Field Trip Application at: levisstadium.com/schools 2. If not completed online, once we receive your field trip inquiry we will send the 49ers Museum Field Trip Application and education program options electronically 3. When the application is completed, schools may submit it via fax, scan/email, or traditional mail to the 49ers Museum. Please send materials to: 49ers Museum Presented by SONY 4900 Marie P. After schools submit the field trip form, our staff will determine field trip eligibility based on schedule, staffing and program availability. 5. Field trip confirmation information: Field trip applications are reviewed bi-weekly. Please wait at least two-six weeks before checking on visitation status. At that point in time you will receive notification of your field trip status, confirmation of reservation request date, and field trip preparation materials. Our staff will work diligently to ensure the field trip experience that schools desire meet their individual needs in every way possible and that schools are provided all the information and support necessary to make this a memorable visit. *If your class is not provided an opportunity to visit, you will have a choice of one of the following options: 1. Your class will be placed as high priority on our 2015/16 waiting list (in case there are cancellations) 2. Your class can be placed on the 49ers Museum field trip schedule for the following calendar year 3. Your class will be provided other additional options to visit the 49ers Museum and Levi s Stadium Thank you for your interest in the 49ers Museum, we look forward to reviewing your request and serving your students and school community!
