Payment amounts are based on the Blue Cross Blue Shield of Michigan approved amount, less any applicable and/or amounts required by your plan. This coverage is provided pursuant to a contract entered into in the state of Michigan and will be construed under the jurisdiction of and according to the laws of the state of Michigan. Specified Organ Transplants - Adds transplant benefits for the liver, heart, heart-lung, lung and pancreas in Blue Cross Blue Shield-approved facilities. Requires prior approval by Blue Cross Blue Shield. Benefits are not available for pre-existing conditions until the member has been enrolled for nine consecutive months Diabetic Test Strips and Lancets removes monthly quantity limits for these supplies. Hair Prostheses and Related Supplies - covers hair prostheses and related supplies (covered at 100% after ), when prescribed by a physician due to hair loss caused by chemotherapy or radiation therapy. In addition to DME and/or P&O providers, hair prostheses and related supplies will also be covered when purchased from other provider types as well (such as wig shops, etc). Benefits limited to $350 per member. Members must submit claims with an itemized receipt and a prescription from the physician to BCBSM s Special Manual Claims Department to process; provider electronically submitted claims will manual process. Rider EBMT, experimental bone marrow transplant coverage. Established the criteria and clarifies which conditions are payable for experimental bone marrow transplants. Donors must meet genetic marker criteria. Requires prior approval by Blue Cross Blue Shield.

Option Care will handle mail order prescriptions only for specialty drugs while many retail pharmacies will continue to dispense specialty drugs (check with your local pharmacy for availability). Other mail order prescription medications can continue to be sent to Medco. (Medco is an independent company providing pharmacy benefit services for Blue members.) If your physician rewrites your prescription for the recommended generic or OTC alternate drug, you will only have to pay a generic. If your physician rewrites your prescription for the recommended brand-name alternate drug, you will have to pay a brand-name. In select cases BCBSM may waive the initial after your prescription has been rewritten. BCBSM will notify you if you are eligible for a waiver. Select drugs may have limitations related to quantity and doses allowed per prescription unless the prescribing physician obtains preauthorization from BCBSM.
