Evaluation of clinical placement Name of student: Period of clinical placement: Name of home institution: Name of clinical placement institution: The evaluation is completed at the end of the clinical placement and is based on the student s overall experiences at the clinical placement and with the clinical counsellor. If the student is in clinical placement at two different institutions, two evaluations must be completed. Definition of gradings: EXCELLENT displaying high level of command of all aspects of the relevant material, with no or only a few minor weaknesses VERY GOOD displaying a high level of command of most aspects of the relevant material with only minor weaknesses GOOD displaying good command of the relevant material but also some weaknesses FAIR displaying some command of the relevant material but also some major weaknesses ACCEPTABLE performance meeting only the minimum requirements for acceptance NOT ACCEPTABLE performance not meeting the minimum requirements for acceptance THE STUDENT S EVALUATION OF OWN PROFORMANCE AT CLINICAL PLACEMENT Own clinical performance The student s own evaluation of successfully achieving learning objectives in clinical placement.
