Page 3 AFLAC A wide variety of specialty insurance products from AFLAC is available to ALL employees. These include Disability, Accident, Cancer and many more. All are provided at group rates that can save you lots of money! Automotive Discount Save 10% at Victory Auto Locations on your car care needs. You must have a discount card, which is available from your program administrator. Company Newsletter An informative and entertaining newsletter, The Orion is published monthly. Included in the newsletter are notes about upcoming training, names of newly hired staff, birthdays, anniversaries and other company information. Dental Insurance - We offer Group Dental Insurance to all of our regular Full-Time employees through Principal Financial contributing 80% of the monthly cost. The employee pays 20% of the monthly cost (only $2.73 per paycheck for a single employee!). An employee becomes eligible for Dental Insurance on the 1st of the month following 1 full calendar month of employment. Employee of the Month Each month two lucky employees (nominated by their supervisor) is singled out for their excellent work. This employee receives a $100 gift certificate to any place of their choice, along with a write up in the newsletter, a company mug, and pen. Employee Assistance Program Need help with personal issues? The EAP is a resource for support and information during difficult times. This is offered through a partnership with or insurance companies, but is available to ALL employees. The toll free number is: 1-800-432-5155

Page 4 Employee Referral Program Any employee who refers a person to us for employment is eligible for a bonus! $25.00 if we hire the person, $50.00 when the person has been with us for 3 months, and $100.00 if the person remains employed for one year! Family and Medical Leave - Meridian Services and Zenith Services will grant eligible employees up to a total of 12 work weeks of unpaid leave of absence under the Family and Medical Leave Act (FMLA), using a rolling 12 month period that looks backward for qualifying family and/or medical reasons. Fitness Club Discounts All Full Time employees enrolled in the company s health insurance plan are eligible to receive a $20 discount per month at a participating fitness club, if you attend at least 12 times per month. Flexible Spending Account Eligible employees can participate in a Flexible Spending Account, which allows employees to set aside compensation, on a pre-tax basis to pay for Out-of-Pocket Medical Expenses and/or Child Care Expenses. Employees may set aside up to $3,000.00 annually for Out-of-Pocket Health Care Expenses and up to $5,000.00 annually for Dependent Care Expenses. For Full-Time employees enrolled on Medical and Dental Insurance, the plan allows for Medical and Dental premiums to be deducted from an employee s compensation on a pre-tax basis saving you money! Fresh and Natural Foods Discount All staff receive a 10% discount at all Fresh and Natural Foods locations. Funeral leave - Employees may be granted leave with for the purpose of making arrangements for or attending funerals. For full time employees, in the event that the funeral is for an employee s spouse, domestic partner, child, parent, sibling, grandparent or grandchild, the company will pay up to 2 days Funeral Leave with supervisor approval.

Page 5 Health Insurance - We offer Group Health Insurance to all of our regular Full-Time employees. Our provider is one of this state s largest and best medical insurers Health Partners. We offer two plans to choose from to fit your needs and we contribute $200.00 for the employee, $350.00 for the employee and one dependent, and $500.00 for full family coverage. The employee pays the monthly cost above the maximums. An employee becomes eligible for Health Insurance on the 1st of the month following 1 full calendar month of employment. Holiday Pay - Full time employees receive seven paid holidays per year: New Years Day, Easter (or Floating), Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas Day. All nonexempt hourly employees who work a holiday observed by the organization earn time and one half during these observed holidays. MERSC - MERSC, a nonprofit professional association, is dedicated to helping member companies improve employee satisfaction and enhance employee well being. Receive discounts at over 100 businesses, including hotels, oil changes, theaters, museums, hair salons, etc you name it!! This benefit is for ALL employees all you have to do is show your pay stub to get the discount. A list of all the businesses can be found at www.mersc.org Mileage and other Expense Reimbursement - Reimbursement for the use of personal vehicles for company business is available to all employees. Other expenses incurred by an employee during the course of employment may also be reimbursable such as parking fees and activity fees. Notary Services We have two notaries in the Metro Office who will gladly notarize your forms for free!

Page 6 Nurse s Line - This is offered through a partnership with our insurance companies, and is available to all employees currently enrolled in Health Partners. This allows you to speak with a registered nurse about health questions, learn self care tips or get health related information. The toll free number is 1-800-551-0859. Paid Time Off - All regular Full-Time employees customarily working 35 or more hours weekly are eligible to receive, accrue, and use Paid Time Off. All regular exempt employees customarily working less than 35 hours weekly are eligible for prorated Paid Time Off, based on their regular schedule. This increases as you are employed longer and is available for use upon completion of six months of regular employment. Paid Training - We offer many different types of training opportunities to employees. These may include but are not limited to regular inservice training, regular supervisor training, new employee and supervisor orientation and training, and outside in-service training including American Red Cross CPR and First Aid and Vulnerable Adult Act. Promotional Opportunities All employees are encouraged to apply for promotional opportunities. Whenever possible, our company favors promotion from within. PTO Share This program allows you to share your PTO with someone who may really need it. Those who face a serious medical or other hardship and do not have enough PTO to cover their lost time may be eligible to receive PTO donated by co-workers. Retirement Plan Plan for the future! We offer a 401(k) Retirement Plan through Principal Financial to Full-Time and Part-Time Meridian Services employees and a Simple IRA account, using Putnam investments to Full-Time and Part-Time Zenith Services employees. Employees must have at least one year of service with the organization, are 21 years of age or older, and work 1000 or more hours a year. The organization will match 50 percent of the first 6 percent of the employee s contributions to the plan

Page 7 Short Term Disability/ADD/Life Insurance - We offer a paid Short Term Disability, Life, and Accidental Death and Dismemberment Insurance plan to all of our Full-Time employees through Principal Financial. The cost is paid by the company. An employee becomes eligible for these plans on the 1st of the month following 1 full calendar month of employment. Special Leave Compensation Our Company may allow you to take time off for a variety of reasons including but not limited to: child school conferences, jury duty, military leave, voting time off and more. Vision (VSP) All Full Time employees that are enrolled in our dental plan are also eligible for our vision discount plan which includes 20% off the cost of an eye examination, 20% off the cost of complete pair of prescription eyeglasses, 20% off the cost of cosmetic lens options, and 15% off the cost of contact lens exam at participating locations. Will Preparation Services All Full time employees that are enrolled in dental are eligible for this extra perk! According to a recent survey, 58% of adult Americans lack a basic will. Through Will Preparation Services, employees and their spouses can complete a will, living will, healthcare power of attorney, and financial power of attorney for FREE.
