STEPS TO APPLY FOR YOUR EMERGENCY 30-DAY SUB PERMIT ON-LINE (RECOMMENDATION ONLY) 1. The first step is to complete an application (41-4 form). In order for your application to be processed on-line you must have an e-mail address. On the 41-4 application there is a designated area to make a note of your e-mail address in section # 1 (please print clearly). Along with the 41-4 application form please submit proof of your BA degree or higher, basic skills exam and verification that you have completed the fmgerprint requirement for the CTC. 2. Once all the necessary data is collected the San Joaquin County Office of Education (SJCOE) will recommend you for the permit through the CTC via their website. PLEASE NOTE BEFORE STARTING: You must use one of the following web browsers: Microsoft Internet Explorer Versions 7, 8, or 9, Firefox 4.x and above or Google Chrome Make sure and turn off your pop-up blocker Your application will be deleted if you have not paid within 90 days ofbeing recommended 3. Once the recommendation is processed you will receive an e-mail from CTC requesting you to complete information on-line and pay the application fee ($72.00). You will need to pay the fee of $70.00 and a $2.00 service charge, using Visa or MasterCard (debit or credit). When paying with your Visa or MasterCard make sure that the address info entered matches exactly with the fmancial institution. Please see the attached example of the first e-mail CTC Notice- Recommendation for New Document (El). Please remember to immediately open the e-mail and follow the instructions to complete the payment process, otherwise your permit will not be granted. 4. You will then be sent an e-mail confirming your payment. Please see the attached example of the second e-mail Your Authorized Payment Receipt (E2). 5. CTC will then send an e-mail to you stating they have received your application and will be conducting a professional fitness review based on your fingerprints and the answers of your professional fitness questions. Please see the attached example of the third e-mail CTC Application Received Email (E3). 6. Once the document has been granted you will receive a fmal e-mail stating that your permit has been issued. The e-mail will also include a link to the CTC website where you can view and print your permit in 48 hours. Please see the attached example of the Notification of Credential Issuance (E4). NOTE: CTC does not mail you an official document. Most documents are granted within 10 working days, unless you answer 'yes" to any of the professional fitness questions. 1. What ifl do not have a Visa or MasterCard? Frequently asked questions about the on-line permit process: There are many retailers, who offer pre-paid Visa and MasterCard. If you are using a pre-paid Visa or MasterCard you must activate the card before using it to pay for your permit fees on-line with the CTC. 2. What ifl do not have an e-mail address? Anyone can set up an e-mail address by visiting www.msn.com, www.yahoo.com, www.google.com, etc. They're free!! These e-mail accounts can be accessed from any computer with Internet access or you may also use a friend or family member's e-mail address. 3. What ifl do not receive the e-mail to complete the payment for the on-line recommendation? You may access your application by completing the following steps: Log in to the following web address www.ctc.ca.gov Click on Online Services for Educators Select Educator Page Enter SS# and DOB and click OK You can edit your personal info if needed (select edit, change info, select save) and you may also add/change your address info if needed (select new, enter info, select save), select next Under Recommends go to Select and select Yes for the document that you were recommended for, click Complete Read the disclosure statement and select yes and click next to proceed Complete all professional fitness questions and Oath and Affidavit Click on Submit Payment to proceed to the payment section Pay $72 application fee -ONLY CLICK SUBMIT PAYMENT ONCE You will receive a payment receipt once the payment is processed 7/12an
