Organization Chart Samples for Healthcare Related Organizations The organization charts on the following pages were received in response to the following question sent to the Association for Healthcare Philanthropy moderated Yahoo group: I am interested in getting some samples of organization charts for hospital or health care related Foundations. If any of you are willing to share, please forward directly to me. Organization Chart Samples for Healthcare Related Organizations In addition to the printed charts, there was one response where there was no printed chart. Our Foundation is a separate supporting 501 (c) (3) (509 A) with its own board comprised of Medical Center staff and volunteers by designation, with the preponderance of board members representing the public. I am a voting member, as is our CEO, CFO and an employee support committee representative. The Med Staff, Auxiliary and Board of Trustees each have one voting seat.
