Introducing Grace Bedford Bringing long-term, acute care closer to you

A comprehensive approach Introducing Grace Bedford, an exciting new addition to University Hospitals Bedford Medical Center, a campus of UH Regional Hospitals. As a certified long-term acute care facility the first of its kind in Cuyahoga County Grace Hospital has earned a reputation for clinical excellence and patient satisfaction. Now, UH Bedford campus has partnered with Grace Hospital to provide a 360-degree spectrum of acute care at one convenient location in our community. While UH Bedford campus continues to offer exceptional short-term acute care, Grace Bedford located on the third floor of the main UH Bedford campus building complements our comprehensive capabilities with exceptional long-term acute care expertise.

Giving long-term care a new meaning Grace at UH Bedford campus is not a nursing home. Fully accredited by The Joint Commission, Grace Bedford focuses on the same level of acute care that UH Bedford campus does, specifically for patients who require a longer period of more intensive care. Grace at UH Bedford campus truly is a hospital within a hospital. Its mission is to provide specialized care for patients and families coping with catastrophic illness and injury. For many, the initial recovery journey takes longer than four days the length of time a short-term acute care facility like UH Bedford campus may accommodate for inpatient care. These patients need more time in a facility dedicated to nurturing the person while treating their complex acute medical conditions. That facility is Grace at UH Bedford campus.

Committed to continued improvement At Grace Bedford, an average inpatient stay is 25 days. During that time, it is Grace Bedford s goal: To be the bridge between a patient s short-term hospital stay and going home, or to a less intensive level of care. To restore the patient s greatest level of independence. To involve the patient and his or her family in the recovery process. Grace Bedford s new, 32-bed acute care facility at UH Bedford campus welcomes patients: Who are very ill, but progressing. With multiple long-term acute medical needs. Who will benefit from an intensive and comprehensive team approach to care. Acute care patients at Grace Bedford most often arrive directly from the ICU at UH Bedford campus or another University Hospitals facility. After their stay, they typically go home, continue their recovery at an acute rehabilitation facility, or return to the skilled nursing facility where they were residing prior to their initial hospital stay.

Dedicated to total wellness and comfort Following a thorough referral, assessment and care planning process, the dedicated staff at the new Grace Bedford facility provides 24-hour monitoring of every patient with a high nurseto-patient ratio. Physicians and specialists see each patient every day, and they are always on call to quickly and skillfully address any emergency situation. Since long-term acute care patients face many medical challenges, family members are permitted to visit 24 hours a day. Patients can make or receive phone calls from their bed, and familiar items from home can be brought in to make a patient feel more at ease.

Our new long-term acute care capabilities Grace Bedford now complements the short-term acute care programs available at UH Bedford campus with these long-term capabilities: Ventilator weaning Ventilator dependence is common with certain illnesses and accidents, but often the extended use of a ventilator can be reduced. The Grace Bedford team is trained to help patients gain the strength to be weaned from breathing-assisted technology, including: Ventilator BiPap/CPAP Oxygen therapy Endotracheal tubes Tracheostomy Grace Bedford s success rate for ventilator weaning is greater than 70 percent. Wound care Patients with nonhealing wounds have multiple medical needs, including prolonged illnesses, nutritional deficits and pre-existing conditions like diabetes. The Grace Bedford inpatient wound program complements the acclaimed outpatient Wound Care and Hyperbaric Medicine Center at UH Bedford campus with specialized procedures to help patients gain maximum mobility and reach their highest level of functional independence. Medically complex care When a patient s bodily systems fail, Grace Bedford physicians and staff work to stabilize the patient s condition and prevent further complications. Their medically complex care expertise includes: Hemodialysis Total parenteral nutrition (TPN) Central venous lines Cardiac monitoring Inotropic therapy IV anticoagulation

Transplant and mechanical circulatory support (MCS) Grace Bedford welcomes organ transplant patients of all types, including those awaiting a transplant. MCS is a safe and effective method of supporting patients in terminal heart failure until a donor organ is obtained or destination therapy is pursued. Transplant therapy is also offered, designed to assist patients in gaining strength, endurance and mobility post-transplant. Rehabilitation services Grace Bedford provides low-tolerance occupational, physical and speech therapy on an inpatient basis. Rehabilitation specialists treat ventilator-dependent, orthopaedic, neurologic, cardiac, neuromuscular, wound care and organ transplant patients with the goal of restoring movement, speech, swallow function and greater independence.

Grace at UH Bedford campus With our two distinctive but connected hospitals, Grace at UH Bedford campus has teamed up to offer the only long-term acute health care facility in the University Hospitals system. Grace Bedford brings its own dedicated staff to the new, state-of-the-art longterm care environment, which includes: An all-rn staff, with LPN and PCA support as needed A certified wound care nurse Respiratory therapists Occupational, physical and speech therapists Clinical nutritionists Pharmacists Case management/social services Like UH Bedford campus, Grace Hospital is a nonprofit health care provider, which means that Grace Bedford is 100 percent accountable to the patients it cares for and the loved ones who care about them.

A genuine acute care collaboration Patients who come to UH Bedford campus or another UH facility for treatment and learn that long-term acute care is necessary are now able to extend their stay and make an easy transition to Grace Bedford. Conversely, if a Grace Bedford patient needs emergency treatment, he or she can typically move to the UH Bedford campus ICU or other tertiary care facility without having to worry about an Emergency Department visit. For maximum convenience and time management, all diagnostic and support services needed by Grace Bedford patients are provided by UH Bedford campus, including imaging, lab and surgical services, as well as house physician and Rapid Response Team coverage. Finally, our unique collaboration ensures that each patient s primary care physician or specialist will be a part of the treatment process. Physicians can be assured that their patients will be returned to their care at the conclusion of their long-term acute stay.

Accreditation and insurance information Grace Hospital, a registered Long-Term Acute Care Hospital, is Medicare and Medicaid certified and fully accredited by The Joint Commission. Reimbursement from Medicare and Medicaid is based on acute care days versus skilled nursing days. Grace Bedford accepts most insurances, including Medical Mutual of Ohio SuperMed.

Superior quality of care and patient satisfaction UH Bedford campus is known for its tradition of clinical excellence. That excellence is matched by the quality and patient satisfaction record that Grace Hospital brings to our facility. Grace Hospital has excelled in recent Joint Commission surveys, and consistently places in the 90th percentile on patient satisfaction, communication and clinical service surveys. Together with UH Bedford campus, Grace Bedford is also committed to supporting its community. As a result of this new caring partnership, more than 60 new jobs have come to the city of Bedford.
