navigate the winds of change Topic Change Benefit changes for 2015 Topic Change Health Tracks & Managing Health Conditions Health Care Flexible Spending Account (FSA) No cost for generic drugs for certain health conditions Those currently managing health conditions (including diabetes, high cholesterol or high blood pressure) who are assigned to any Health Track for 2015, will qualify for payment assistance for generic prescriptions related to managing their specific chronic condition. You ll pay $0 for generic prescriptions to treat these specific conditions. 2015 Health Care FSA carryover Effective January 1, 2015, you can carry over up to $500 in unused funds from your 2015 Health Care FSA balance at the end of the year. You can use these funds for eligible expenses incurred in the following year. However, the grace period for incurring reimbursement for eligible expenses will no longer be available, meaning expenses must be incurred by December 31. You will still have until May 31 of the following year (May 31, 2016, for 2015 expenses) to request reimbursement for expenses incurred during the prior calendar year. Important - for 2014 Health Care FSA contributions: Be sure to use all of your 2014 Health Care FSA contributions, as the new carryover provision does not begin until 2015. The grace period will still be available for 2014 contributions. This means you have until May 31, 2015, to request reimbursement for eligible expenses incurred through March 15, 2015, if you are a participant at the end of the calendar year. Health Benefits Medical and Dental Plan premium increases Premiums for the Medical and Dental PPO Plans will only increase 3% for 2015. This increase is lower than the past two years, where premiums increased at 5% each year. Out-of-pocket maximum increases In 2015, medical and pharmacy co-payments and coinsurance will both contribute to a single annual out-of-pocket maximum. The out-of-pocket maximum will increase to accommodate this change. In-Network Individual $4,500 Family $9,000 Out-of-Network Individual $6,000 Family $12,000 (UnitedHealthcare Choice Plus) Transplant coverage at Houston Methodist facilities Transplant procedures will only be covered if performed at a Houston Methodist facility. Bariatric surgery coverage change Certain types of bariatric surgery such as lap bands will no longer be covered in 2015. your 2015 Health Tracks Steer toward better health Your Biometric Screening results this fall will place you on a personalized Health Track for 2015, which is a personalized path full of specific activities focused on improving or maintaining your health. In mid-december, we will contact you with more information about your Health Track and exactly what you need to do in 2015 to earn great rewards including the lowest Medical Plan premium option in 2016. Track Your Biometric Screening results Activities Health Conditions Track tackles the biggest health concerns around: cholesterol, blood pressure and glucose. Nicotine Track helps you kick the habit. Body Mass Index (BMI) Track focuses on decreasing your BMI, which can act as an indicator of healthy or unhealthy weight. Healthy Track focuses on maintaining good health. OR Achieve designated fitness goals. OR Decrease your BMI by two points or meet 27.0 target (whichever occurs first).
