Level two health guidance The second level offers guidance to better navigate toward wellness goals and get help when you need care. Plus there are tools to help guide you as you make important care decisions. Future Moms Future Moms is a program designed to promote early and regular prenatal care to help expectant mothers have healthy pregnancies and deliver healthy babies. Specialized nurses work with mothers-to-be to create a pregnancy program that is tailored to their lifestyle, home environment and specific needs. To enroll in Future Moms, call 866-664-5404. 24/7 NurseLine Call 24/7 NurseLine and receive medical information from registered nurses, any time day or night. Your doctor is your best source of information about your health, but our 24/7 NurseLine is a great place to start. You ll find the toll-free 24/7 NurseLine phone number on your member ID card. Childhood obesity The Kids In Charge of Kalories (KICK) Program is a free program specially designed for parents who want to help their children (between the ages of 6 and 12) learn about healthy eating choices and the benefits of physical activity. Help your kids eat smart and be more active through the KICK Program s free phone support and educational materials such as the KICK family workbook. We provide support for: Asthma Diabetes Coronary Artery Disease Heart Failure Chronic Obstructive Pulmonary Disease (COPD) You will be assigned an RN Health Coach to help identify what lifestyle issues are important to you. Then we will link you with a team of health professionals to work with you on healthy behavior change for those issues. Your team may include your RN Health Coach, dietitians, respiratory therapists, health educators, social workers and exercise physiologists. Each Health Coach is specially trained to help you set individualized goals and to help guide and support your health care decisions. Health coaching addresses the whole person, not just the disease. We provide action-oriented education and support. The result is an empowered, confident person, ready to tackle difficult health behavior changes successfully. All women enrolled in an Anthem Blue Cross HMO plan have the freedom to use their primary care physician or select a participating obstetrician/gynecologist (OB/GYN) directly, without a referral. Ask your medical group for a list of participating OB/GYN referral physicians. PPO plan members may self-refer to doctors, including OB/GYN specialists, as part of their plan benefits. 9
Breastfeeding Support Line Many new mothers need information and education about breastfeeding in order to make informed decisions about how and when to feed their babies. To help new moms with breastfeeding questions and concerns, we ve introduced the Breastfeeding Support Line, a free nurse information line available 24 hours a day, 7 days a week. Call the Breastfeeding Support Line at 800-231-2999. Subimo s Healthcare Advisor You can use this website if you are preparing for a medical procedure or managing a medical condition. It can be a strong decision-making tool, helping you find information that is clear and easy to understand.
